When these cells disseminate from tumors and enter the blood stream, they carry the tumor's genetic and proteomic information. They are particularly attractive because they can be obtained in a minimally invasive manner. Using microfluidic devices, we can isolate these rare cells from the background of blood cells. Here we study clinically relevant use cases of these CTCs. We characterize two aspects of CTCs: i) the PD-L1 expression levels on CTCs from non-small cell lung cancer patients undergoing immunotherapy ii) the secretion of proteases as it relates to cancer metastasis. In the process of molecular profiling the CTCs, we use a novel label free Vortex isolation system. A modification to the original Vortex device enabled us to capture a wider size range of CTCs at a higher capture efficiency, uncovering further heterogeneity. This simple biophysical method opens doors for a range of downstream analysis. After CTC isolation, we test the hypothesis that tumor cells secrete proteolytic enzymes to facilitate tissue invasion and spread. Given recent work suggesting CTCs can be produced by early stage tumors, MMP production by CTCs could provide a more precise biomarker of the level of metastatic activity for the disease beyond a simple CTC count. We developed a unique integrated microfluidic system able to perform three functions: i) Isolate CTCs rapidly by size. ii) Exchange the fluid around CTCs to both remove contaminants, and introduce a fluorogenic MMP-substrate. iii) Encapsulate purified CTCs into a small number of microdroplets to interrogate MMPs secreted at the single-CTC level. Total analysis from blood input to secretion assay takes minutes and preserves viability, making this system compatible with studying live cells while they retain physiologic conditions. We found that isolated CTCs from metastatic prostate cancer patients, identified through surface markers and nucleus size, release active MMPs. Assaying protease activity of circulating tumor cells will ultimately be important in uncovering the biology of metastasis and serve as a key component of future phenotypic liquid biopsies, potentially enabling companion diagnostics for MMP inhibitor therapies currently in clinical trials.

As a consequence, growing tumor cells in plastic dishes places a selective pressure that substantially alters their original molecular and phenotypic properties.The emerging field of regenerative medicine has developed bioengineered tissue platforms that can better mimic the structure and cellular heterogeneity of in vivo tissue, and are suitable for tumor bioengineering research. Microengineering technologies have resulted in advanced methods for creating and culturing 3-D human tissue. By encapsulating the respective cell type or combining several cell types to form tissues, these model organs can be viable for longer periods of time and are cultured to develop functional properties similar to native tissues. This approach recapitulates the dynamic role of cell–cell, cell–ECM, and mechanical interactions inside the tumor. Further incorporation of cells representative of the tumor stroma, such as endothelial cells (EC) and tumor fibroblasts, can mimic the in vivo tumor microenvironment. Collectively, bioengineered tumors create an important resource for the in vitro study of tumor growth in 3D including tumor biomechanics and the effects of anti-cancer drugs on 3D tumor tissue. These technologies have the potential to overcome current limitations to genetic and histological tumor classification and development of personalized therapies.

Book excerpt: This dissertation describes novel methods to detect, separate and analyze circulating tumor cells (CTCs) from peripheral blood, with a high sensitivity and throughput. The ensemble-decision aliquot ranking (eDAR) platform is a new method for isolating CTCs from cancer patients with a recovery ratio of 93% and a zero false positive rate. We have validated this method by analyzing the samples from breast, pancreatic and lung cancer patients. The capability to perform downstream analyses on the isolated CTCs were also tested and integrated into the eDAR platform. We developed a method that can monitor eight protein markers, using a sequential staining and photobleaching procedure. Single-cell isolation and culture of the trapped CTCs were also tested. Based on these, we re-designed the eDAR platform by applying a new active cell sorting scheme and a new further purification method. Because many applications in the CTC area are focused on the enumeration, we also developed an automated method for counting tumor cells in whole blood samples. This enumeration method was validated by a side-by-side comparison with CellSearch, the only FDA approved method at present, in 90 metastatic breast cancer patient samples. These tools facilitate fundamental investigation of rare cells, as well as developing non-invasive biopsy to improve the clinical treatment.

Book excerpt: This work focuses on the integration of microfluidics and dielectrophoresis(DEP) with the principles of field flow fractionation (FFF) to create a continuous-flow isolator for rare and viable circulating tumor cells (CTCs) from peripheral blood mononuclear cells (PBMNs) drawn from cancer patients. The method exploits differences in the plasma membrane capacitances of tumor and blood cells, which correspond to differences in the membrane surface areas of these cell types. DEP-FFF was first adapted to measure cell membrane capacitance, cell density and deformability profiles of cell populations. These properties of the NCI-60 panel of cancer cell types, which represents the wide functional diversity of cancers from 9 organs and leukemia, were compared with the normal cell subpopulations of peripheral blood. In every case, the NCI-60 cells exhibited membrane capacitance characteristics that were distinct from blood and, as a result, they could be isolated from blood by DEP. The heightened cancer cell membrane capacitances correlated strongly with membrane-rich morphological characteristics at their growth sites, including cell flattening, dendritic projections, and surface wrinkling. Following harvest from culture and maintenance in suspension, cancer cells were found to shed cytoplasm and membrane area over time and the suspended cell populations developed considerable morphological diversity. The shedding changed the cancer cell DEP properties but they could still be isolated from blood cells. A similar shedding process in the peripheral blood could account for the surprisingly wide morphological diversity seen among circulating cells isolated from clinical specimens. A continuous flow DEP-FFF method was devised to exploit these findings by allowing CTCs to be isolated from the nucleated cells of 10 mL clinical blood specimens in 40 minutes, an extremely high throughput rate for a microfluidic-based method. Cultured cancer cells could be isolated at 70-80% efficiency using this approach and the isolation of CTCs from clinical specimens was demonstrated. The results showed that the continuous DEP-FFF method delivers unmodified, viable CTCs for analysis, is perhaps universally applicable to isolation of CTCs from different cancer types and is independent of surface antigens - making it suitable for cells lacking the epithelial markers used in currently accepted CTC isolation methods.
