A l’échelle mondiale, une femme sur trois subit au moins au cours de sa vie une forme de violence physique, sexuelle ou psychologique, le plus souvent de la part de son partenaire intime. Les violences subies au cours de la vie dégradent fortement la santé des femmes : conséquences immédiates, à moyen et long terme, pouvant concerner toutes ces dimensions. Le rôle des professionnels de santé dans la prise en charge de ces femmes est élaboré depuis plusieurs années. Il est recommandé d’effectuer un dépistage orienté et systématique. L’objectif principal est de connaître le ressenti des jeunes femmes victimes de violences sur leur santé aux différentes étapes de leur vie. Méthodes : étude qualitative par entretiens semi-dirigés auprès de 15 jeunes femmes victimes de violences, entre 18 et 25 ans, volontaires, résidentes au foyer d’hébergement de l’association du FIT Une Femme Un Toit. Résultats : Les jeunes femmes perçoivent une santé altérée à différentes périodes de leur vie avec des symptômes de plus en plus nombreux à partir de l’adolescence et à l’âge adulte, périodes marquées par les violences. Leur santé leur paraît très abimée à ce jour. L’enfance, dans l’ensemble, semble être considérée comme la période de leur vie ou leur santé était la meilleure. L’adolescence est une période ou nombreuses sont celles qui perçoivent que leur santé se modifie et est altérée sur le plan moral et psychologique avec une accentuation des symptômes à l’âge adulte. Depuis leur prise en charge au sein du foyer d’hébergement, nombreuses sont celles qui ressentent une amélioration de leur santé morale, psychologique et/ou physique. Cependant elles estiment qu’à ce jour, les violences les ont détruites psychologiquement, et qu’il est difficile de se reconstruire. 6 jeunes femmes rapportent s’être confiées à un médecin généraliste au cours de leur vie sur les violences subies, elles étaient déjà toutes sorties du cycle de la violence quand elles se sont confiées. Elles ont évoqué de nombreux freins pour consulter ou pour se confier à leur médecin. Des attentes variées vis-à-vis des professionnels de santé : certaines évoquent ne pas avoir d’attente particulière d’autres disent attendre des médecins qu’ils leur posent des questions sur les violences subies au cours des consultations

GBV has serious consequences for women's health and well-being, ranging from fatal outcomes, such as homicide, suicide, and AIDS-related deaths, to nonfatal outcomes, such as physical injuries, chronic pain syndrome, gastrointestinal disorders, complications during pregnancy, miscarriage, and low birth-weight of children. GBV also poses significant costs for the economies of developing countries, including lower worker productivity and incomes, and lower rates of accumulation of human and social capital. Under the rubric of violence against women the two overlapping issues have received increasing attention. Much of the response to these forms of violence has focussed either on advocacy, providing adequate health and legal services and ensuring safety of victims once violence is disclosed, or on screening for violence in the context of health services. When the word "prevention" is used, it is usually in reference to secondary and tertiary prevention - helping women get out of violent situations and preventing further violence. Little attention has been given to primary prevention - addressing the root causes with the goal of reducing the number of new cases. Recent hallmark publications have called for increased investment in primary prevention. As part of the Global Campaign for Violence Prevention, WHO is giving increased attention to the primary prevention of intimate partner violence and sexual violence. Preventing intimate partner and sexual violence against women: taking action and generating evidence is an important new tool for violence prevention researchers, practitioners and advocates provides a planning framework for developing policies and programmes for the prevention of intimate partner and sexual violence. This guide outlines the nature, magnitude, risks and consequences of intimate partner and sexual violence. It outlines strategies to prevent these forms of violence against women and describes how these can be tailored to the needs, capacities and resources of particular settings. It also emphasizes the importance of integrating scientific evaluation into all prevention activities in order to expand current knowledge of what works. The manual describes interventions of known effectiveness, those supported by emerging evidence, and those that could potentially be effective, but have yet to be sufficiently evaluated for their impact. For instance, school-based programmes to prevent violence occurring in "dating relationships" have been shown to be effective in randomized trials in the USA and Canada. Evidence is emerging for the effectiveness of empowerment approaches which use microfinance combined with gender-equality training or are based on communications and relationship skills training such as the Stepping Stones training package, which has been widely used in low- and middle-income countries. Strategies that aim to prevent intimate partner and sexual violence through reducing the harmful use of alcohol also show promise. A six-step framework for planning, implementing and evaluating such prevention programmes and policies is presented. This document is primarily aimed at policy-makers, programme planners and donors from public health and related sectors who seek to advance the prevention of intimate partner and sexual violence against women. In addition to the principal audience, other interested parties will include those working in other government sectors such as education, child welfare, social care, criminal justice and departments of women or gender equality; advocates from civil society organizations; local authorities; environmental and urban planners; and researchers."
