Elle est efficace en terme d'apprentissage et améliore les compétences. Cependant la plupart des études ne se sont concentrées que sur les bénéfices immédiats et la rétention des compétences à court terme. Objectif principal: Étudier l'intérêt et l'apport pédagogique à court, moyen et long terme de la simulation médicale par rapport à une formation classique. Méthodes: Tous les internes de 3ième année du 3ième cycle en anesthésie-réanimation et médecine d'urgence ont été convoqués. Tous ont bénéficié d'une formation théorique avec un rappel des dernières recommandations sur la prise en charge de l'arrêt cardiaque et du choc anaphylactique. Ils ont été randomisés en 2 groupes et ont reçu une formation pratique sur simulateur haute-fidélité à la prise en charge soit de l'arrêt cardiaque (groupe ACC), soit du choc anaphylactique (groupe CA). Chaque groupe a été évalué à 6 semaines (T0), 6 mois (T1) et 1 an (T2) sur deux scénarii : une fibrillation ventriculaire réfractaire (FV) noté sur 20 points et un choc anaphylactique de grade 3 (CA3) noté sur 30 points. Chaque groupe a servi de groupe témoin à l'autre pour la pathologie dans laquelle il n'a pas reçu de formation spécifique sur simulateur. Les résultats sont exprimés en moyenne avec leurs écarts-types avec « p »

La simulation médicale est en pleine expansion en France actuellement en anesthésie-réanimation, médecine d'urgence, néonatalogie, obstétrique et soins infirmiers. Le rapport de mission du GRANRY et MOLL a mis en évidence une disparité des moyens et pratiques entre les centres. La simulation offre un milieu centré sur l'apprenant qui permet au formateur de lui donner toute son attention. Au cours d'une séance de simulation, l'apprenant construit et déconstruit de façon simultanée son savoir et teste son efficacité. Cependant, la simulation induit parfois une charge émotionnelle difficile à gérer liée à un investissement important sur le plan personnel. Le vécu de la séance peut se révéler extrêmement stressant. Ce paramètre doit être pris en compte dans l'enseignement, tout particulièrement lors du débriefing. Ce travail décrit et expose les résultats d'une enquête descriptive sur les techniques pédagogiques employées par les formateurs en simulation médicale au cours de leurs débriefings. Les centres contactés ont été ceux du CHU de Toulouse, du Laboratoire Expérimental de Simulation de Médecine Intensive de l'Université de Nantes, du Centre d'Apprentissage des Attitudes et Habiletés clinique de Montréal (Québec), du Centre de formation des hôpitaux de Genève, Suisse. L'importance de la disparité des pratiques de débriefing doit inciter à plus formaliser cette phase de la formation par simulation. La vidéo est un outil pédagogique pertinent pour évaluer les répercussions du stress, la communication verbale, les aptitudes psychomotrices et la prise de décision ; elle doit être intégrée pour remplir certains objectifs du débriefing notamment pour travailler les interactions de groupe.

How can we examine pragmatic competence to make inference of its development among L2 learners? In what ways do research findings inform teaching and assessment of pragmatic competence? This book explores these key issues in Japanese as a second/foreign language. The book has three sections. The first section offers a general overview and historical sketch of the study of Japanese pragmatics and its influence on Japanese pedagogy and curriculum. The overview chapter is followed by eight empirical findings, each dealing with phenomena that are significant in Japanese pragmatics. They target selected features of Japanese pragmatics and investigate the learners' use of them as an indicator of their pragmatic competence. The target pragmatic features are wide-ranging, among them honorifics, speech style, sentence final particles, speech acts of various types, and indirect expressions. Each study explicitly prompts the connection between pragmalinguistics (linguistic forms available to perform language functions) and sociopragmatics (norms that determine appropriate use of the forms) in Japanese. By documenting the understanding and use of them among learners of Japanese spanning multiple levels and time durations, this book offers insight about the nature and development of pragmatic competence, as well as implications for the learning and teaching of Japanese pragmatics. The last section presents a critical reflection on the eight empirical papers and prompts a discussion of the practice of Japanese pragmatics research.
