Book excerpt: La prise en charge du patient BPCO est actuellement un enjeu majeur de santé publique, ceci du fait de la fréquence de cette pathologie chronique, de sa prévalence en constante augmentation et de l'important coût de santé généré. Le traitement médicamenteux s'avère insuffisant. Ainsi, il est proposé des programmes de réhabilitation respiratoire qui apportent d'importants bénéfices, en améliorant la dyspnée, la tolérance à l'exercice, la qualité de vie et le recours aux soins. Mais aujourd'hui se pose la question du devenir des patients à la fin des programmes de réhabilitation menés dans les structures spécialisées. Il semble alors nécessaire de proposer des réhabilitations à long terme afin qu'un maximum de patients puisse poursuivre un programme d'éducation physique qui leur soit adapté pour leur permettre de maintenir leurs acquis, voire de les augmenter. Les programmes de maintien à long terme de réentraînement à l'effort sont recommandés par la SPLF. Il a été possible de créer en Haute-Vienne un club " Poumon et Santé ". Celui-ci permettra en partenariat avec la FFEPGV la mise en place d'un programme de réentraînement à l'effort au long cours, suite à une réhabilitation respiratoire. Dans un avenir proche, un autre club " Poumon et Santé " devrait être créé en Creuse, pour faire suite au réentraînement mis en œuvre dans la structure hospitalière de Sainte Feyre, et son programme de réhabilitation respiratoire hospitalière. Nous espérons aussi que d'autres départements en France utiliseront ces clubs. Le but dans l'avenir est de créer un réseau de réhabilitation respiratoire en région limousine, associant la prise en charge en centre, en ambulatoire, à domicile et au sein d'associations sportives et ludiques représentées par la FFEPGV. Ceci afin de permettre une prise en charge optimale des patients tout au long de leur maladie et de coordonner le travail des médecins généralistes, des pneumologues, des pharmaciens, des kinésithérapeutes de ville et de structure et des infirmières, qui devront tous être parties prenantes du réseau.
