Après avoir évoqué le rythme comme composante indissociable de la parole et représenté par la syllabe, unité rythmique par essence, nous constatons que chez les enfants porteurs d'un trouble de la parole, l'alternance du tour de rôle dans l'échange est perturbé sur deux niveaux : sur le plan auditivo-verbal, ces enfants télescopent très souvent la parole qui leur est adressée et d'un point de vue visuo-moteur, ils n'allouent pas suffisamment l'attention nécessaire à la bouche de leur interlocuteur. Or les jeunes enfants qui apprennent à parler focalisent très tôt leur attention sur les lèvres, cette attention est renforcée par le jeu des neurones miroirs qui sont impliqués dans l'observation des mouvements de la bouche pour pouvoir apprendre à les reproduire. Ainsi, nous émettons l'hypothèse que l'introduction du LPC au sein de la rééducation orthophonique représenterait un moyen augmentatif pertinent et efficace puisqu'il sollicite le canal visuo-moteur perceptif en mobilisant la lecture labiale et la motricité digitale fine. Nous nous sommes intéressée à une population d'enfants entendants âgés de 4 à 5 ans présentant des retards de parole / langage. Après avoir introduit le LPC au sein des séances hebdomadaires, nous avons constaté une amélioration de plusieurs composantes, notamment la phonologie et la morphophonologie, ainsi qu'une amélioration des contacts oculaires et une amélioration de la rythmicité du tour de rôle et de parole dans l'échange. Une nouvelle étude pourrait évaluer l'exposition au LPC sur un temps plus long et de manière plus écologique en analysant des interactions parents / enfant avec un recours à la vidéo.

Book excerpt: L'evidence-Based Practice (EBP) est une approche qui puise ses racines dans l' EvidenceBased Medicine (EBM) et qui trouve progressivement sa place en orthophonie. Le principe de l'EBP est d' optimiser la prise en charge orthophonique en associant les meilleures données issues de la recherche à l'expertise clinique du praticien et aux valeurs et préférences du patient. La rigueur méthodologique de cette approche pyrmet de guider les cliniciens dans le choix des outils d'évaluation et de rééducation, mais également dans l'évaluation de l'efficacité du traitement proposé (Maillart & Durieux, 2014). Parallèlement, alors que l' acquisition de la syntaxe est fréquemment perturbée chez les enfants présentant un trouble développemental du langage, on trouve encore peu d'études démontrant l'efficacité des traitements des troubles syntaxiques (Schelstraete et al, 2011). Notre étude porte donc sur l'intérêt de l'utilisation de l'EBP dans le cadre de la rééducation syntaxique auprès d'enfants d'âge scolaire. Pour cela, nous avons construit des lignes de base (pré-tests), comportant toutes des items contrôles, pour évaluer les performances de chaque enfant sur une variable syntaxique déficitaire. Puis nous avons proposé un traitement adapté à chacun en nous appuyant sur la littérature et les préférences des enfants. Grâce à l'EBP, nous sommes en mesure de déterminer que le traitement s'est révélé efficace, grâce à l'évaluation en post-test des mêmes items. Puisque les structures cibles sont mieux maîtrisées, et non les items contrôles, nous pouvons conclure que cette progression peut bel et bien être attribuée au traitement orthophonique, et non à des facteurs externes comme la maturation de l'enfant.

Book excerpt: Cette étude de cas multiples porte sur trois sujets d'âge pré-scolaire bénéficiant d'une prise en charge orthophonique en CAMSP pour des troubles sévères du développement du langage oral. A travers ce travail, nous avons tenté de montrer l'intérêt d'une observation phonétique fine de corpus de parole pour la pratique clinique en orthophonie. Pour cela, nous avons réalisé des enregistrements de plusieurs séances individuelles de rééducation. Les documents vidéo ont été transcrits et les données sonores segmentées puis étiquetées. Une analyse qualitative et quantitative détaillée de deux corpus ainsi constitués a conduit à élargir notre objet d'étude initial (les pauses intra-énoncés " déviantes " ) et à prendre en compte les énoncés sans pause, tous les types de pauses intra-énoncés, leur dimension discursive et plus globalement, les relations entre prosodie et syntaxe dans le cadre de l'interaction. Un ensemble de pauses produites par deux des enfants ont pu être décrites et qualifiées d'atypiques en raison de plusieurs caractéristiques morphosyntaxiques, rythmiques et mélodiques. Loin de constituer une limite à notre démarche empirique, les productions orales du troisième enfant illustrent une capacité d'utilisation de moyens langagiers réduits à des fins de communication. Description de la substance sonore visant à dégager une forme linguistique, notre démarche se veut également une voie d'approche thérapeutique intéressante dans le cadre intéractif de la séance de rééducation orthophonique.

The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training. The time required to administer the examination typically ranged from 5 to 10 minutes. (Author/GK)
