Book excerpt: Franco Basaglia (1924-1980) was an Italian psychiatrist and activist who proposed the dismantling of psychiatric hospitals and pioneered new ideas about mental health and its treatment. Basaglia was also one of the principal proponents of Italy's Law 180, which effectively closed down large mental hospitals in Italy. His ideas and his disciples have had a decisive influence in the move away from institutional care in many parts of the world, particularly in continental Europe and South America. However, Basaglia is strikingly absent from the literature in Germanic and Anglophone psychiatry. Most of the literature about Basaglia in the last 40 years has been published by his followers and supporters and has often been largely positive, with little exploration of differing responses or possible limitations of his model. Basaglia's International Legacy: From Asylum to Community provides an overview of current thinking and the international influence of Franco Basaglia. This resource draws on the combined knowledge of clinicians, policy makers, historians, and social scientists, including a handful of Basaglia's collaborators. It provides an in-depth understanding and critical analysis of the various applications of his thinking worldwide. Organised into three broad sections, chapters examine Basaglia's work and influence in Italy; in the 'Basaglian' countries of Europe and South America; and in those countries where his influence has either been rejected or significantly modified. The Editors bring together the contributions and draw out the important messages (both positive and negative) for current clinical practice and development within international mental health services.

L'association Santé mentale et communautés naît d'une sorte de révolte réfléchie vis-à-vis à la fois du destin du psychotique et du type de réponse qu'incarnait l'asile psychiatrique. A cette époque, sa fonction de protection de la société vis-à-vis de la folie prime sur sa fonction soignante. Par ailleurs, les perspectives thérapeutiques vis-à-vis de la psychose se précisent grâce aux apports de la psychanalyse, en particulier de l'école kleinienne. l'espoir apparaît de pouvoir faire sortir le patient de la forteresse intérieure dans laquelle il semble muré... L'association veut lutter contre le rejet du fou, et soigner celui-ci en le réinsérant progressivement dans son milieu naturel. Elle est donc conduite à expérimenter des soins à domicile articulés à de petites structures intermédiaires, c'est-à-dire insérées dans le tissu urbain. Le terme de communauté renvoie bien sir à la notion de communauté thérapeutique (psychothérapie de groupe) mais aussi au partenariat avec les équipements et ressources du quartier ou de la ville (dispensaires, centres sociaux, travailleurs sociaux, enseignants). Actuellement, l'association se positionne en laboratoire du secteur psychiatrique pour pouvoir continuer à expérimenter et innover, en particulier dans le mode de rencontre avec des personnes souffrant d'autres types de pathologies mentales (suicidants, dépressifs, délinquants sexuels, etc.).

Book excerpt: Participer à la démocratisation de la société à travers la lutte contre la ségrégation des personnes souffrant d'un trouble mental a constitué le but d'un groupe composé de psychiatres et d'autres intellectuels qui a animé dans les années 1970, autour de Franco Basaglia, le débat sur les services alternatifs de psychiatrie. Visant à réformer les législations en vigueur, ces professionnels ont organisé dans différents pays des services de psychiatrie territoriale privilégiant l'intégration sociale des malades comme condition d'une prise en charge ne menaçant pas leurs droits. Tandis que la désinstitutionnalisation s'imposait progressivement, le mouvement contestait la reproduction des logiques asilaires dans des services territoriaux centrés sur l'hôpital psychiatrique. La création de nouveaux services remplaçant les services hospitaliers s'est accompagnée de la mise en place d'un savoir critique prenant appui sur les sciences sociales pour mettre en cause ces services, rendre la critique durable de façon à en tirer avantage pour les réadapter sans cesse. La critique de l'hôpital psychiatrique et du contrôle social a stimulé la création de nouveaux services que, au début des années 2000, l'Organisation Mondiale de la Santé a désignés comme « services communautaires ».Notre analyse rassemble sous l'appellation de « psychiatrie communautaire » des expériences menées en Italie et France à partir d'initiatives locales que nous comparons. Dans les deux sites, les malades sont accompagnés à leur domicile par des institutions et des services communautaires qui, se fondant sur la critique de l'asile et du contrôle social, ont remplacé les services coordonnés à partir de l'hôpital psychiatrique. Nous avons questionné le rapport entre le travail quotidien des éducateurs et le dispositif central de leur travail, le logement des personnes prises en charge. Notre travail ethnographique reconstitue le travail de prise en charge qui est guidé par des principes non cohérents avec la prise en charge des patients par une communauté territoriale. Orientés par le projet de démocratiser la société à travers la réforme de la psychiatrie, les deux services sont amenés à construire des compromis qui, dans une tout autre orientation, participent à un gouvernement par l'objectif et tendent à réorienter le travail d'assistance vers un plan exclusivement médical ou marchand. Comment le projet de démocratisation, toujours présent dans les deux sites, peut-il se maintenir dans cette nouvelle configuration et profiter de la portée créative de la critique ?
