Les auteurs, Philippe Faucher et Danielle Hassoun, sont tous deux gynécologues-obstétriciens. P. Faucher est praticien hospitalier, responsable de l'Unité Fonctionnelle d'orthogénie de l'hôpital Trousseau (Paris). D. Dès l'Antiquité, la limitation des naissances, quelles qu'en soient les raisons, a toujours fait partie des risques pris par les femmes. La légalisation de l'interruption volontaire de grossesse (IVG) en 1974 a été un combat de plusieurs années sous l'impulsion de Simone Veil. Depuis, la loi a évolué et autorise la prise en charge médicamenteuse à domicile jusqu'à 7 semaines d'aménorrhée (SA). L'objectif de cette étude consistait à dresser un état des lieux de l'IVG médicamenteuse en ville en termes d'épidémiologie, d'efficacité et de tolérance. L'objectif secondaire était de repérer les avantages et les freins évoqués par les femmes et les médecins quant à cette pratique. Matériel et méthodes. Une revue de la littérature a été réalisée à partir des recommandations française et internationale, de la littérature grise, des bases de données SUDOC, Cochrane et Medline. La qualité des articles a été évaluée grâce aux grilles AGREE II, CASP, JADAD, R-AMSTAR, STROBE. Les critères d'inclusion étaient : textes écrits en français ou anglais, sans critère de temps, répondants aux mots-clés prédéfinis MeSH. Les textes sans résumé ont été exclus de l'étude sauf s'ils étaient disponibles en ligne ou dans les bibliothèques universitaires de Montpellier-Nîmes. L'écriture de ce travail s'est basée sur la grille PRISMA. Résultats. Au total, 1120 documents ont été identifiés, 191 ont été analysés et 101 inclus dans cette revue bibliographique. Actuellement, sur les 220 000 IVG annuelles, plus de 60 % sont réalisées par voie médicamenteuse. Depuis la loi de 2001 autorisant la pratique en ville, plus de 15 % des femmes y ont recours. Malgré tout, l'offre sur le territoire français est très disparate. L'efficacité est supérieure à 95 % et la tolérance jusqu'à 7 SA est bonne. Les femmes apprécient la disponibilité de leur médecin, le caractère naturel, simple, rapide, sûr, non invasif de cette méthode ainsi que le confort de leur domicile et la présence de leur proche. Les médecins veulent participer à une mission de santé publique, augmenter l'offre de soins, diversifier leur activité, et répondre à la demande de leurs patientes. Néanmoins, patientes et médecins déplorent le manque d'informations/formation et de coordination ville-hôpital ainsi que les contraintes administratives et logistiques qui en découlent. Conclusion. L'innocuité de l'IVG médicamenteuse en ville n'est plus à démontrer et cette prise en charge semble acceptable. Toutefois, certains freins ne permettent pas encore sa pleine expansion. Nous avons donc proposé quelques pistes d'amélioration qui nécessitent d'être approfondies.

Book excerpt: Depuis 2004, l'IVG médicamenteuse peut se faire en ville dans le cadre d'un réseau ville- hôpital qui assure la prise en charge des éventuelles complications et échecs de la méthode. L'IVG en ville a ses particularités inhérentes à la pratique de la médecine générale. Cette étude rapporte pour une part l'expérience d'un cabinet de ville de médecine générale situé dans le nord de Paris à la limite entre le 10e , le 18e et le 1ge arrondissement brassant de fait une grande variété ethnique et sociale de patients .Entre 2008 et 2009, 100 IVG médicamenteuses utilisant le protocole « français» mifépristone 600 mg suivi 48 heures plus tard d'une prise de 400 microgrammes de misoprostol pris au domicile des patientes .La moyenne d'âge était de 29 ans avec une patientèle diversifiée mêlant des personnes en grande précarité tout comme d'autres n'étant pas dans le besoin. Le pourcentage de patientes appartenant à la patientèle du cabinet était de 20%. 54% des patientes se sont présentées à la consultation de contrôle qui a lieu 15 jours après la prise de mifépristone. La méthode médicamenteuse au cabinet du médecin généraliste permet une prise en charge de la même qualité que celle effectuée au milieu hospitalier dans un lieu plus « humanisé» et avec notamment un délai de réponse globalement plus court que les circuits classiques. L'analyse de questionnaires portant sur l'expérience personnelle des médecins vis-à- vis de leurs pratiques quotidiennes de l'IVG. La deuxième étude montre des divergences entre les textes de loi et la pratique réelle de l'ivg hors établissement de santé. Elle met en lumière le fait que si la méthode est identique à celle effectuée à 1 hôpital l'exercice en ville comporte ses spécificités auxquelles il faut s'adapter.

Book excerpt: A decade in preparation, Immigrant Entrepreneurs offers the most comprehensive case study ever completed of the causes and consequences of immigrant business ownership. Koreans are the most entrepreneurial of America's new immigrants. By the mid-1970s Americans had already become aware that Korean immigrants were opening, buying, and operating numerous business enterprises in major cities. When Koreans flourished in small business, Americans wanted to know how immigrants could find lucrative business opportunities where native-born Americans could not. Somewhat later, when Korean-black conflicts surfaced in a number of cities, Americans also began to fear the implications for intergroup relations of immigrant entrepreneurs who start in the middle rather than at the bottom of the social and economic hierarchy. Nowhere was immigrant enterprise more obvious or impressive than in Los Angeles, the world's largest Korean settlement outside of Korea and America's premier city of small business. Analyzing both the short-run and the long-run causes of Korean entrepreneurship, the authors explain why the Koreans could find, acquire, and operate small business firms more easily than could native-born residents. They also provide a context for distinguishing clashes of culture and clashes of interest which cause black-Korean tensions in cities, and for framing effective policies to minimize the tensions.
