C'est pourquoi, de l'étape de diagnostic à celle de l'obturation, ce travail évalue les techniques modernes ainsi que leur réel intérêt clinique. Si, les cavités d'accès sont l'étendard de cette philosophie dans la discipline, leur apport est néanmoins controversé. Il est intéressant de corréler les étapes de mise en forme et d'irrigation tant leur lien est étroit en termes d'efficacité et de réduction de préparation. Enfin, l'obturation qui conclue le traitement voit, grâce à l'apport d'une nouvelle famille de ciments, ses perspectives de simplification et de réduction des risques s'améliorer. L'endodontie minimalement invasive doit respecter les objectifs du traitement endodontique ce qui parfois se révèle compliqué. Le minimalement invasif est donc une philosophie en endodontie ne pouvant être appliquée au quotidien mais qui doit rester l'objectif du praticien.

Book excerpt: Le but de cette étude est de comparer le respect de la morphologie canalaire après instrumentation à l'aide de trois systèmes de mise en forme canalaire différents : le One Shape NEW Generation®, le Wave One® et le Revo-S® grâce à une étude en Micro-tomographie assistée par ordinateur ou micro-CT (Computed Tomography). La mise en place d'une chaîne méthodologique totalement tridimensionnelle (3D) comprenant la reconstruction, le recalage et la segmentation a permis de traiter les images acquises et d'extraire les images recalées des canaux dentaires avant et après instrumentation. Les artéfacts de segmentation dus aux calcifications et aux débris dentinaires ont été traités. Une méthode d'estimation des zones non instrumentées a également été décrite.Le transport canalaire a été calculé pour chaque coupe de chaque tiers radiculaire, en comparant la position du centroïde avant et après instrumentation. La comparaison des moyennes de transport canalaire ne montre pas de différence significative entre les 3 systèmes d'instrumentation.Cette approche méthodologique en 4 parties a permis de valider un protocole d'imagerie 3D reproductible, qui pourra être appliqué in vitro en recherche endodontique dans l'analyse des effets instrumentaux.

Dans cette thèse, nous avons créé deux films pédagogiques portant sur l'utilisation de deux instruments rotatifs en endodontie, le Protaper Next et le Wave One, utilisés dans la préparation canalaire mécanisée. L'objectif de ces films est de fournir un support visuel de ce qui est attendu en endodontie des étudiants de 3ème année pendant leurs travaux pratiques. Les vidéos détaillent le matériel utilisé, puis le protocole allant de l'ouverture de la chambre pulpaire à la mise en forme canalaire à l'aide de ces deux types d'instruments. Dans la dernière partie de ce travail, nous discutons des avantages et des inconvénients dans la préparation, la mise en œuvre et le montage de ces deux vidéos. Pour conclure, nous avons discuté le résultat obtenu et proposé des pistes d'amélioration.

A new era thus began in the early 1960s, which led to a substantial improvement in the management of nerve lesions. The results of nerve grafting techniques have demonstrated that, independent of the length of the defect, lesions can be successfully bridged. The free tissue transplants with microvascular anastomosis have also opened new, rewarding possibilities for peripheral nerve reconstruction procedures, facilitating the achievement not only of satisfactory anatomical but also of satisfactory fUhctional results. In order to evaluate the state of the art and reflect retrospectively on 25 years of microneurosurgical treatment of peripheral nerves, numerous outstanding scientists and clinicosurgical physicians were invited to Hanover in order to exchange their viewpoints and experiences. An active und fruitful discussion resulted which dealt with the many aspects of anatomy, pathology, clinical and neuro physiology, diagnosis, and with the surgery and physiotherapy which constitute modern-day peripheral nerve lesion treatment. The excit ing ongoing experimental and clinical activities have led us to support the wish and idea to publish the scientific exchange which took place during the Hanover symposium. I truly believe that the articles presented in this book cover so many interesting subjects concerned with peripheral nerve lesions that the book will serve the interested and dedicated physician involved with such cases as a reference work for the basics and also provide him with the therapeutic guidelines to assist him in his daily work.

First, the data are more up-to-date than those included in previous reports. Data up to and including 2009 are presented for almost all key indicators; financial data extend to 2011. Second, results from several analyses undertaken for the first time in 2010 are included. Examples are: (i) for each of the 22 high-burden countries (HBCs), trends in rates of TB incidence and mortality since 1990 combined with projections of whether the target of halving the 1990 mortality rate by 2015 will be achieved; (ii) estimates of the lives saved by TB control between 1995 and 2009 and projections of the additional lives that could be saved up to 2015, including separate estimates for women and children; (iii) assessment of progress in implementing and financing TB care and control against the targets included in a just-released and updated version of the Global Plan to Stop TB; and (iv) a new and compelling compilation of data showing the contribution that PP PP M can make to case detection.
