Des études épidémiologiques devraient permettre de répondre à cette question à condition de pouvoir disposer d'une évaluation adéquate de l'exposition des individus étudiés à cette pollution atmosphérique d'origine automobile (PAA), exposition présente mais également passée. En France, les chercheurs du Centre Scientifique et Technique du Bâtiment (CSTB) et de l'Institut National de Recherche sur les Transports et leur Sécurité (INRETS) ont mis au point l'Indice ExTra d'exposition au trafic routier, indice qui repose sur des modèles physico-mathématiques de dispersion de polluants (OSPM et CALINE3). Cet indice permet de classer précisément les lieux de séjour des individus en fonction des concentrations extérieures de polluants auxquelles ils sont soumis, puis de reconstituer l'exposition cumulée de chaque personne devant ces différents lieux de séjour, avec une pondération par le temps passé dans ces lieux. Il n'est, bien sûr, envisageable d'utiliser un tel indice dans le cadre d'études épidémiologiques, qu'après l'avoir validé. Cette recherche avait deux objectifs : 1) valider l'indice ExTra, 2) l'appliquer dans le cadre de l'étude épidémiologique VESTA (V Epidemiological Studies on Transport and Asthma). La validation de l'indice ExTra a été effectuée sur 100 sites exposés à des niveaux contrastés de PAA, dans quatre villes partenaires de l'étude VESTA : Grenoble, Nice, Paris et Toulouse. Les coefficients de corrélation obtenus entre les valeurs des concentrations mesurées et calculées valent, par ville, respectivement : 0,90 ; 0,95 ; 0,89 et 0,89. Par ailleurs, l'application aux enfants inclus dans VESTA a consisté à estimer, à partir du modèle retenu précédemment, leur exposition à la PAA, présente et cumulée jusqu'à la date de point. Les 403 enfants ont été en moyenne exposés à 70 ± 42 æg.m -3 de NOx équivalent NO2 dont 14 ± 22 æg.m -3 attribuables au trafic de proximité. Outre la validation du modèle pour les concentrations en oxydes d'azote et la description des expositions des enfants de VESTA, les résultats obtenus en menant une analyse discriminante par arbres de classification ont permis d'émettre des recommandations sur la pertinence de l'utilisation de l'indice (plutôt que de la pollution de fond fournie par les réseaux) en fonction de l'histoire résidentielle des individus, notamment en considérant le maximum du trafic et le trafic moyen pondéré au domicile ainsi que le minimum de la distance du domicile à son vis-à-vis et le minimum de la distance au trafic du lieu de garde ou de scolarité...
