Ces optimisations passent par une amélioration de la planification opératoire et une optimisation des dispositifs de transfert. La majorité des chirurgiens semble aujourd'hui avoir recours au set-up clinique, éventuellement complété par les analyses céphalométriques ; et assurer le transfert chirurgical par des gouttières de fabrication manuelles ou parfois réaliser des chirurgies « à l'estime » selon la difficulté des cas. Les planifications virtuelles ont donné naissance à plusieurs dispositifs de transfert de conception assistée par ordinateur ; telles que les gouttières de fabrication assistées par ordinateur, les guides de coupe-plaques d'ostéosynthèse préformées et la navigation chirurgicale. Cet article a pour but de classifier et décrire ces dispositifs de transfert émergents, et d'en souligner leurs principaux avantages et inconvénients.

Book excerpt: Résumé : La planification d'un traitement orthodontico-chirurgical a traditionnellement eu pour support des données bidimensionnelles, telles qu'une téléradiographie associée à une photographie de profil, mais cette méthode connaît de nombreuses limites. L'objectif de ce travail est de déterminer dans quelle mesure l'introduction de la troisième dimension contribue à l'amélioration de cette simulation. La fusion des données issues idéalement d'une tomographie volumique à faisceau conique, d'une photogrammétrie du sujet, ainsi que d'un scan intra-oral, offre la possibilité de simuler les mouvements osseux ou d'anticiper le résultat de la position des téguments dans les trois directions de l'espace et permet également un accès plus aisé à l'aide peropératoire. Une revue systématique de la littérature a été menée afin d'évaluer la fiabilité des planifications tridimensionnelles. La synthèse a identifié des études de faible niveau de preuve et mis en évidence une hétérogénéité des protocoles mis en oeuvre. La précision moyenne de la simulation tégumentaire se situerait entre 0 et 2 millimètres pour la plupart des études, associée à des écarttypes parfois élevés, alors que la précision moyenne de l'aide peropératoire serait de l'ordre du millimètre ou du degré. Aucun logiciel 3D ne paraît être supérieur aux autres. De plus, les simulations ne se révèlent pas être plus fidèles aux résultats obtenus selon un type particulier d'intervention chirurgicale. Enfin, si les technologies disponibles s'avèrent être prometteuses, elles requièrent un travail de validation à l'aide de banques de données tridimensionnelles pré- et postopératoires.

La transition vers une planification dite virtuelle reste une entreprise exigeante à mettre en œuvre sur divers aspects. L'objet de notre travail a été de cerner les modalités de la mise en place d'un flux numérique en chirurgie orthognathique au sein d'un service hospitalier universitaire de chirurgie maxillo-faciale. Nous proposons un protocole de flux numérique et discutons son applicabilité à l'hôpital public. Un état des lieux des pratiques en France est effectué. Force est de constater que l'application d'un flux numérique nécessite un véritable engagement de la part de l'équipe chirurgicale en termes de temps et d'énergie, mais également de la part de divers acteurs hospitaliers pour la mise en conformité matérielle, logistique et règlementaire. L'équipe maxillo-faciale devra se former à la planification 3D, la modélisation et l'impression 3D, ainsi que réorganiser son protocole de gestion des patients de chirurgie orthognathique. Elle devra également garder le savoir-faire traditionnel en cas de faille rencontrée pendant le flux numérique. Une fois la mise en conformité réalisée, l'établissement de santé pourra être vecteur de nouvelles études contrôlées randomisées pour affiner l'indication de la planification chirurgicale virtuelle.
