Le rapport Améliorer la couverture effective en matière de santé examine une approche particulière visant à améliorer la couverture effective : les incitations financières sous la forme d’un financement basé sur la performance (FBP), une réforme globale qui comprend généralement la rémunération liée à la performance pour les agents de santé de première ligne ainsi que l’autonomie des établissements, la transparence et l’engagement communautaire. Ce rapport de recherche s’appuie sur un vaste ensemble d’études rigoureuses et de nouvelles analyses. Par rapport au statu quo, dans les pays à faible revenu dotés de systèmes de santé centralisés, le FBP peut entraîner des gains substantiels en matière de couverture effective. Toutefois, les avantages relatifs du FBP — la composante rémunération liée à la performance en particulier — sont moins clairs lorsqu’il est comparé à deux autres approches, à savoir : le financement direct des établissements, qui permet d’octroyer des budgets de fonctionnement aux services de santé de première ligne en accordant aux établissements l’autonomie en matière d’allocations budgétaires, mais sans rémunération liée à la performance ; et le soutien financier lié à la demande pour les services de santé (c’est-à -dire les transferts monétaires conditionnels et les vouchers). Bien que le FBP se traduise souvent par des améliorations marginales, combler les lacunes importantes de la couverture sanitaire effective n’est pas encore à la portée de nombreux pays. Néanmoins, d’importantes leçons et expériences tirées de la mise en oeuvre du FBP au cours de la dernière décennie peuvent orienter le financement de la santé à l’avenir. En particulier, pour réussir, la réforme du financement de la santé devra sans doute moins se focaliser sur la rémunération à la performance tout en conservant les éléments de financement direct des établissements, l’autonomie, la transparence et l’engagement communautaire.

Further, the book will review the level of adoption of the sub-discipline in State policies and practice and also provide practical illustrations, with case studies, of how studies in the sub-discipline are central to the actualization of Africa's development agenda. Particular attention is paid to the relationship between health and development. Through its direct and indirect impacts on labor productivity, population health and wellbeing matter for the social and economic development of households and national economies. Yet, health is not uniform in space. And so is development. Comparatively on many health and development indicators, Africa fairs poorly. The variation in health may present as differences in the occurrence and spread of diseases, the distribution of and access to healthcare facilities, and/or in health outcomes among the population. Reasons for these variations range from biology to the population’s levels of exposure and susceptibility to elements in their environment, including the social interactions taking place within the environment. The field of health and medical geography focuses on the spatial patterns and processes underlying these variations and provides pathways for understanding and addressing them. More specifically, the sub-discipline of health and medical geography focuses on, among others, how places (their characteristics and processes that go on in them) and environmental factors underlie and/or influence disease patterns, exposure and susceptibility to diseases, health variations, health behavior, health outcomes, and the provision of and access to healthcare services. This volume documents perspectives and applications in health and medical geography in Africa for academics, students, health practitioners, and development policymakers.

Cette ascension peut être accélérée par un dividende démographique dû aux changements dans la structure par âge de la population. Les baisses de la mortalité infantile, suivies par la baisse de la fécondité, produisent une génération «renflement» et un grand nombre de personnes d'âge actif, donnant un coup de pouce à l'économie. Dans le court terme, une fécondité plus faible engendre une baisse des taux de dépendance des jeunes et une plus grande participation de la population active féminine en dehors de la maison. La réduction de la taille de la famille signifie également qu’il reste davantage de ressources à investir dans la santé et l'éducation par enfant, augmentant ainsi la productivité future des travailleurs. Au long terme, une durée de vie accrue résultant de l’amélioration dans le domaine de la santé signifie que cette vaste cohorte aux gains élevés sera également désireuse d’épargner pour la retraite ainsi que pour la création de l'épargne et des investissements plus conséquents, conduisant ainsi à d'autres gains de productivité. Deux choses sont nécessaires pour que le dividende démographique génère un décollage économique de l'Afrique. La première consiste à accélérer la baisse de la fécondité qui est actuellement au point mort ou lente dans de nombreux pays. La seconde est les politiques économiques qui profitent de l'occasion offerte par le changement de la démographie. Alors que l'évolution démographique peut produire plus, et des travailleurs de meilleure qualité, cette main-d'œuvre potentielle doit être employée si l'Afrique se doit de récolter le dividende. Cependant, une fois en route, la relation entre l'évolution démographique et le développement humain fonctionne dans les deux sens, c’est-à -dire qu’elle crée un cercle vertueux susceptible d’accélérer la baisse de la fécondité, le développement social et la croissance économique. Les recherches scientifiques montrent trois facteurs clés pour accélérer la transition de la fécondité: la santé des enfants, l'éducation des femmes et l'autonomisation des femmes, notamment par l'accès à la planification familiale. Exploiter le dividende démographique nécessite la création d'emplois pour les grandes cohortes de jeunes qui entrent en âge de travailler et qui stimulent les investissements étrangers jusqu'à faire augmenter l'épargne intérieure et l'investissement. La combinaison appropriée de politiques dans chaque pays dépend de leur stade de transition démographique. Etant la dernière région à subir la transition démographique, l’Afrique peut tirer les leçons des réussites et des échecs des autres régions dans l'exploitation d'un dividende démographique. Le succès exige (i) l'accélération de la transition démographique; (ii) une économie dynamique produisant des revenus et des investissements plus élevés pour une jeunesse mieux éduquée et en meilleure santé.
