A travers la diversité de normes qui entourent le secteur médico-social et des différentes préconisations qui sont données pour gérer la qualité en EHPAD, il sera question dans notre mémoire de nous pencher particulièrement sur la gestion du risque infectieux en EHPAD. La gestion du risque infectieux faisant partie de la démarche qualité en EHP AD préconise les bonnes pratiques professionnelles en vue de réduire les infections liées aux soins et à 1 'hygiène. Cependant, son appropriation par les praticiens du soin (infirmiers, aides-saignantes) n'est pas toujours aisée et rencontre plusieurs freins. L'implémentation de la gestion du risque infectieux nécessite donc des moyens et des outils de gestion permettant de l'intégrer au mieux dans les pratiques professionnelles afm que ces dernières puissent avoir un impact sur 1' amélioration des pratiques de soin et d'hygiène en EHP AD. J'ai donc donné dans ce mémoire une méthodologie de gestion du risque infectieux à travers différents outils pratiques. Cela aura pour but d'harmoniser les pratiques au sein l'établissement afin que cette dernière réponde aux exigences de gestion du risque. Aussi il est présenté en dernière parti ma mission et les résultats et les préconisations proposés suite à la mise à jour du DARI.

Book excerpt: Devant une population vieillissante de plus en plus polypathologique et polymédiquée, les Établissements d'Hébergement pour Personnes Âgées Dépendantes (EHPAD) occupent une place importante dans notre système de santé. La prise en charge médicamenteuse d'un résident est un macro-processus complexe, impliquant de multiples acteurs en des lieux différents. Face à des contraintes budgétaires et une pénurie de personnel, l'externalisation de la Préparation des Doses à Administrer (PDA) pour les établissements d'hébergement médicalisés dépourvus de Pharmacie à Usage Intérieur (PUI), semble être l'un des outils permettant une meilleure efficience du circuit du médicament. La première partie de ce travail consiste à dresser un état des lieux de la prise en charge médicamenteuse d'un résident en EHPAD sans PUI et positionne la PDA comme l'une des pratiques gage de sécurisation du circuit du médicament, et ceci malgré une absence de textes réglementaires. La seconde partie précise le rôle des acteurs intervenant dans la prise en charge d'un patient en EHPAD avant d'expliciter la mise en œuvre de la PDA et ses diverses méthodes existantes. Enfin, la dernière partie implique les pharmaciens d'officine dans une démarche de qualité et de sécurité de la prise en charge médicamenteuse d'un patient. Car, si la PDA n'est pas exécutée de manière procédurée, elle peut augmenter le risque de survenue d'événements indésirables évitables. La finalité de ce travail a pour vocation de proposer un certain nombre d'outils pratico-pratiques permettant d'assurer la qualité et la gestion des risques à mettre en œuvre par le pharmacien d'officine lors de la réalisation des piluliers.
