Une analgésie postopératoire efficace, gérée par le patient à domicile avec un minimum d'effets secondaires est une clé essentielle de la réussite de l'ambulatoire. L'intervention éducative thérapeutique est rarement utilisée dans le contexte de la douleur postopératoire (DPO), car elle cible plutôt les affections chroniques alors que les patients pourraient en bénéficier pour des situations aiguës. L'objectif de notre étude est d'évaluer l'impact d'une intervention éducative préopératoire standardisée et structurée sur la gestion de la DPO en ambulatoire comparé à la pratique courante. Il s'agit d'une étude prospective, randomisée, monocentrique, en simple aveugle devant inclure 410 patients. Les patients inclus sont des adultes, ASA I à III, prévus pour une chirurgie orthopédique programmée en ambulatoire entrainant une douleur modérée à sévère. Les patients sont randomisés en 2 groupes parallèles : un groupe contrôle et un groupe intervention. L'anesthésiste délivrera aux deux groupes l'information sur la DPO selon le schéma habituel proposé par l'HAS en 2010. En plus de la consultation habituelle, les patients du groupe intervention recevront l'intervention éducative standardisée et structurée comprenant des informations orales et visuelles et se déroulant en plusieurs étapes : rappel d'une douleur vécue, quantification et qualification personnelle de la douleur, discrimination, étalonnage sur l'échelle de douleur, informations sur la DPO, les traitements antalgiques, le principe d'anticipation de la douleur et de traitement systématique. Elle s'appuie sur l'utilisation d'une échelle numérique de douleur allant de 0 à 10 avec des exemples de perceptions correspondant à chaque niveau. Le critère de jugement principal est le soulagement de la douleur évalué par le TOTPAR (TOTal PAin Relief) pendant les 5 premiers jours à domicile. Les critères de jugement secondaires sont le TOTPAR à J10, J20 et J30, ainsi que les prises d'antalgiques, l'incidence des douleurs sévères, les effets secondaires, la prévalence des douleurs neuropathiques (échelle DN4), la qualité du sommeil (questionnaire PSQI) et le confort et vécu du patient (questionnaire IPO), à J30. En améliorant l'information du patient sur l'évaluation et le traitement de la DPO, celui-ci pourrait être en mesure de mieux gérer sa douleur à domicile. L'étude PELOPS va permettre d'évaluer l'impact d'une intervention éducative structurée sur le soulagement de la DPO. A terme, le but est d'améliorer le parcours de soins du patient opéré en ambulatoire et de discuter de la mise en place de consultations préopératoires dédiées à l'explication et à la gestion de la DPO.

Matériel et Méthodes : Il s'agissait d'une étude prospective randomisée mon centrique (CHU de Rouen) entre mai 2017 et mars 2018. Un groupe recevait son ordonnance en consultation d'anesthésie (groupe CA) et un groupe en postopératoire immédiat (groupe PO). Le critère principal de jugement était l'incidence d'au moins un épisode douloureux d'intensité supérieure à 3 sur 10 à domicile à .11. La douleur pendant l'hospitalisation, la douleur à 17, l'observance du traitement, les Nausées Vomissements Postopératoires (NVPO) ont été relevés et une analyse multivariée recherchait les facteurs de risque de douleur à domicile. Résultats :186 patients ont été inclus, 94 dans le groupe CA et 92 dans le groupe PO. Dans les 24 premières heures, 94 patients (50%) présentaient une douleur modérée à sévère avec 35 patients (36%) dans le groupe CA et 59 patients (64%) dans le groupe PO (p=0,n002). Le groupe CA était plus observant que le groupe PO à il (84% versus 69%, p=0,02). L' ncidence de la douleur à J7 et des NVPO étaient similaires entre les groupes. La délivrance anticipée était retrouvée (ORa=0,3 [0,1-0,51, p=0,0003) comme facteur protecteur d'apparition de douleurs à domicile. Conclusion : Ces résultats suggèrent que la délivrance anticipée de l'ordonnance d'antalgiques pourrait diminuer l'incidence de la douleur postopératoire à domicile.

La douleur et les nausées/vomissements post-opératoires représentent les principales complications non chirurgicales, et sont sources de comorbidités, hospitalisations, et coûts supplémentaires. Peu d'études ont étudié la douleur post-opératoire dans les semaines suivant une chirurgie ambulatoire, et celles portant sur le sujet ont établi que les douleurs non contrôlées (EN > 4/10) touchent entre 20% et 30% des patients à J1-J2, puis entre 10% et 30% de J3 à J7. Mais de nombreux progrès ont été réalisés sur l'analgésie multimodale et la réhabilitation post-opératoire depuis. Une réévaluation de la prévalence et des causes de la douleur en post-opératoire de chirurgie ambulatoire parait donc utile. Cette thèse constitue le rapport de l'étude prospective longitudinale et descriptive DOL-UCA (« Prévalence et évaluation de la douleur en post-opératoire de chirurgie ambulatoire ») conduite au sein du service de chirurgie ambulatoire de l'hôpital Pitié-Salpêtrière à Paris. L'objectif principal était d'évaluer les trajectoires douloureuses en post-opératoire de chirurgie ambulatoire. Les objectifs secondaires étaient d'apprécier le retentissement douloureux, d'identifier et confirmer les facteurs de risque de douleur, de décrire la consommation antalgique, opioïde, et le mésusage, et d'examiner l'efficacité des prises en charge d'analgésie multimodale en post-opératoire. Le recrutement était étalé sur 3 semaines et concernait tous les patients majeurs, capables d'exprimer leur consentement, et opérés en ambulatoire qui étaient ensuite suivis durant 3 mois. Les caractéristiques des patients et les données chirurgicales et anesthésiques étaient recueillies par des questionnaires en pré- et per-opératoire, puis les variables post-opératoires étaient collectées par des entretiens téléphoniques quotidiens de J1 à J7, puis mensuels de Ml à M3 post-opératoire. Une douleur non contrôlée était définie par une intensité > 4/10 à l'EVA. La douleur et les antalgiques était évalués par le BPI, l'anxiété pré-opératoire par le questionnaire APAIS, la dramatisation pré-opératoire par le questionnaire PCS, et les capacités d'adaptation pré-opératoires par le CSQ-F. Le retentissement douloureux évaluait les activités quotidiennes avec le BPI, le sommeil avec le questionnaire de Vis- Morgen, la récupération avec le QoR-15, la qualité de vie avec FEQ5DL5 et le SF-36, et la satisfaction avec l'EVAN-G, l'EVAN-LR, et des échelles numériques. Enfin, le mésusage opioïde était recherché par le questionnaire POMI. Quatre-vingt patients ont été inclus après chirurgies orthopédiques, gynécologiques, urologiques, ORL, et stomatologiques. L'âge médian était de 51 ans, les femmes représentaient 71% de la population, l'IMC médian était de 25,4 kg/m2. En pré-opératoire les patients étaient 45% à présenter un syndrome anxio-dépressif, 44% une douleur chronique pré-existante, et 25% une consommation antalgique. Malgré cet échantillon réduit, les résultats montraient des incidences de douleurs non contrôlées inférieures à celles des études antérieures, un faible retentissement douloureux sur les activités quotidiennes, le sommeil, la qualité de vie post-opératoire, et la satisfaction, excepté pour la récupération post-opératoire, mais aussi une excellente prise en charge antalgique, et quasiment aucun mésusage opioïde. L'anxiété pré-opératoire et la douleur pré-existante étaient corrélées en analyse multivarié avec les douleurs non contrôlées à J7 et M3 respectivement, de même, que l'anxiété, les douleurs neuropathiques pré-existantes, et la consommation opioïde chronique pré-opératoires pour les intensités douloureuses à J7. Bien qu'elle manquât de puissance, cette enquête constituait une réévaluation des douleurs et pratiques antalgiques, ainsi qu'une approche novatrice de l'accompagnement et du suivi des patients douloureux en post-opératoire de chirurgie ambulatoire. Elle a mis en évidence la nécessité d'un suivi rapproché à l'aide des nouvelles technologies de la santé, et a servi d'audit au lancement de l'application pour smartphone POST'UP.
