Elle est une des principales causes de douleur dans les suites de couches, donc la prise en charge de sa douleur est primordiale. La circulaire du 30 avril 202, impose la mise en place de protocoles de prise en charge de la douleur. l'objectif de cette étude était d'évaluer la prise en charge de la douleur de l'épisiotomie les 4 premiers jours de postpartum au CHU de Rouen. Méthodologie : Étude prospective monocentrique à la clinique obstétricale du CHU de Rouen chez 72 patientes primipares ayant bénéficié d'une épisiotomie entre octobre 2009 et janvier 2010. La douleur a été évaluée les quatre premiers jours de postpartum à l'aide de l'échelle visuelle analogique. Ont également été étudiées : l'information délivrée aux patientes, l'application de protocole d'analgésie du service, la satisfaction de la prise en charge de la douleur. Résultats : Il existe un pic de douleur de 5.5 sur 10 entre J0 et J1, avec une médiane de douleur durant le séjour de 3.5 sur 10. 21% des patientes ont eu des EVA supérieures à 3 au quatrième jour de leur séjour. Le protocole existant n'est que peu appliqué (54%). l'information délivrée aux patientes est peu satisfaisante néanmoins l'indice de satisfaction de la prise en charge de la douleur est de 7.5 sur 10 en moyenne. Conclusion : La prise en charge de la douleur n'est pas optimale, il serait important que le protocole soit suivi pour pouvoir évaluer son efficacité.

Book excerpt: Objectif : l’objectif principal de ce travail était de comparer l’épisiotomie à la déchirure spontanée au niveau de la douleur et de la cicatrisation périnéale en post-partum immédiat suite à un accouchement eutocique. L’objectif secondaire était d’identifier les éventuels facteurs de risque de la douleur et des problèmes de cicatrisation périnéale précoces. Matériels et méthodes : nous avons effectué une étude prospective de cinq mois au sein de la Maternité du C.H.U. de Grenoble, recrutant 185 patientes ayant accouché de manière eutocique et présentant soit une épisiotomie (58) soit une déchirure spontanée (127). Au troisième jour, la douleur et la cicatrisation ont été évaluées par : les scores E.V.A., la consommation d’antalgiques et d’anti-inflammatoires pour le périnée et l’échelle de cicatrisation R.E.E.D.A. Résultats : nNos résultats ne montrent pas de différence au niveau de la douleur et de la cicatrisation périnéale entre les patientes ayant présenté une épisiotomie et une déchirure spontanée, au troisième jour du post-partum. Les patientes présentant des éraillures multiples présentent davantage de problèmes de douleurs et de cicatrisation en post-partum immédiat. Conclusion : il n’y a aucun bénéfice à réaliser une épisiotomie au cours d’un accouchement eutocique dans le but de diminuer la douleur et d’améliorer la cicatrisation périnéale dans le post-partum immédiat. Les problèmes entraînés par les éraillures multiples en post-partum immédiat nécessitent une prise en charge adaptée.

Book excerpt: OBJECTIFS DE L'ETUDE : Observer la prise en charge actuelle de la douleur post-épisiotomie et évaluer son efficacité à la Maternité Régionale de Nancy. POPULATION ET MÉTHODES : Etude prospective unicentrique incluant 65 femmes ayant subi une épisiotomie sur la période de septembre à décembre 2014. Cette étude s'est faite à l'aide de questionnaires distribués en suites de couches de J0 à J3 du post-partum. L'intensité de la douleur était évaluée par cotation sur l'échelle numérique. Les facteurs de risque influençant la douleur, la prise en charge reçue ainsi que la satisfaction des patientes ont également été recherchés. RÉSULTATS : La douleur moyenne au cours du séjour est de 4,3/10. Le pic de douleur se situe à J1 avec une moyenne de 4,9/10. Toutes les patientes n'ont pas bénéficié de la même prise en charge. Nousavons observé un manque d'anticipation de la douleur, les médicaments étant donnés systématiquement seulement à 62,3%. Nous avons également constaté que les AINS ne sont que très peu utilisés (26% contre 74% paracétamol seul), et que l'automédication concerne une partie importante de notre population (57%). De plus, l'évaluation orale quotidienne de la douleur ne concerne que 11% des patientes. Globalement, les patientes sont satisfaites avec une moyenne de 6,9/10 pour leur prise en charge. CONCLUSION : La prise en charge de la douleur post-épisiotomie n'est pas optimale. L'uniformisation des pratiques permettrait une amélioration de la qualité des soins.

Yet in 1992-93, some five hundred people died from cholera in the Orinoco Delta of eastern Venezuela. In some communities, a third of the adults died in a single night, as anthropologist Charles Briggs and Clara Mantini-Briggs, a Venezuelan public health physician, reveal in their frontline report. Why, they ask in this moving and thought-provoking account, did so many die near the end of the twentieth century from a bacterial infection associated with the premodern past? It was evident that the number of deaths resulted not only from inadequacies in medical services but also from the failure of public health officials to inform residents that cholera was likely to arrive. Less evident were the ways that scientists, officials, and politicians connected representations of infectious diseases with images of social inequality. In Venezuela, cholera was racialized as officials used anthropological notions of "culture" in deflecting blame away from their institutions and onto the victims themselves. The disease, the space of the Orinoco Delta, and the "indigenous ethnic group" who suffered cholera all came to seem somehow synonymous. One of the major threats to people's health worldwide is this deadly cycle of passing the blame. Carefully documenting how stigma, stories, and statistics circulate across borders, this first-rate ethnography demonstrates that the process undermines all the efforts of physicians and public health officials and at the same time contributes catastrophically to epidemics not only of cholera but also of tuberculosis, malaria, AIDS, and other killers. The authors have harnessed their own outrage over what took place during the epidemic and its aftermath in order to make clear the political and human stakes involved in the circulation of narratives, resources, and germs.
