This book is based on more than 30 years of practice and reflection within the framework of the Alfred Binet Centre in Paris, France. The very great diversity of situations encountered at the Centre brings the issue of therapeutic indications to the forefront. Michel Ody focuses on the diversification of fifteen clinical situations and their theorization, ranging from basic consultation to psychoanalytic treatment. With this framework as his starting-point, he looks at the common features between the therapeutic consultation – a consultation that becomes therapeutic – and the analytic treatment, as well as what differentiates them. This implies examining, at the technical level, the different forms of interventions and interpretations presented as well as their metapsychological articulation. Ody draws on decades of clinical expertise to set out not just the basic considerations and problems typically encountered in work with this patient group, but clear guidelines for methodology and technique. Psychoanalysis can be an intellectual process, dependent on the ability of the patient to express themselves verbally, which can make working clinically with children challenging. Objectif : L'objectif principal de cette étude était d'évaluer les facteurs associés à l'alliance thérapeutique à la première consultation chez les mères d'enfants âgés de 18 à 48 mois ayant un trouble émotionnel et/ou un trouble du comportement. Méthodologie : Les mères remplissaient avant la première consultation des auto- questionnaires évaluant la présence d'un épisode dépressif, le soutien marital, l'impact des événements de vie, leur statut vis-à vis de l'attachement, et la présence d'une psychopathologie. A la fin de la première consultation, les mères évaluaient l'alliance thérapeutique et les thérapeutes la relation mère-enfant et la gravité clinique de l'enfant. Les facteurs associés à l'alliance thérapeutique de 163 mères ont été étudiés par une régression linéaire multiple. Résultats : La présence d'un épisode dépressif chez la mère, d'un bon soutien émotionnel du partenaire ou d'un trouble du sommeil chez l'enfant étaient associées à une alliance thérapeutique plus élevée. Des difficultés dans la relation mère-enfant et la présence du père à la consultation étaient associées à une alliance thérapeutique plus basse. Discussion : Lors de consultations de jeunes enfants suivis pour trouble émotionnel et/ou du comportement, l'alliance thérapeutique semble plus difficile à construire dans les situations de trouble de la relation mère-enfant, et celles de faible soutien émotionnel de la part du partenaire. Ces situations nécessitent une attention particulière du thérapeute.
