La sanction ne se réduit pas à la privation de liberté : elle se paie aussi en termes de santé mentale. Ainsi, on se suicide six à sept fois plus en prison qu'à l'extérieur, de nombreux détenus vivent des situations de détresse qui se surajoutent à celles qui, avant leur incarcération, les plongeaient dans diverses formes d'addiction. De ce point de vue, on conviendra que les médicaments psychotropes visent à réduire la douleur psychique des individus incarcérés. Mais ces produits ont la particularité d'avoir, en outre, une fonction institutionnelle, en contribuant au calme de l'institution pénitentiaire et à la sécurité des personnels de surveillance. C'est en cela que la consommation des tranquillisants et des hypnotiques en prison intéresse les sociologues : comment peut-on vivre en prison ? Quelle est la place du support médicamenteux dans le déroulement de l'incarcération ? Quelle est la finalité du soin dans ce type d'institution ? Qu'en est-il de la liberté de l'acte de prescription, compte tenu du positionnement du service médical en prison ? Comment se gère la distribution des médicaments, longtemps assurée par les seuls personnels de surveillance ? L'enquête conduite par les auteurs révèle une consommation de médicaments psychotropes en prison très largement supérieure à celle de la population générale. Elle met également en avant le rôle des personnels de surveillance dans la distribution des fioles, pratique qui inquiète les détenus et mécontente les "gardiens" dans un contexte de pénurie d'infirmiers. Depuis 1994, de nouveaux textes officiels visent à intégrer la médecine pénitentiaire dans le dispositif général de notre système de santé. Ces nouvelles mesures peuvent-elles changer la réalité quotidienne des détenus, modifier leur demande et le circuit du médicament en prison ?

Les détenus ont également été sollicités pour diminuer ce mésusage des médicaments dans le cadre de la promotion à la santé, programme soutenu par l’Agence Régionale de Santé (ARS).
