Miles in 1908 re ported his experience in the management of cancer in this ana tomic part, that the combined abdominoperineal resection be came the recognized and accepted approach for the treatment of this cancer. Miles reasoned that proctectomy removed the cancer, bearing a segment oflarge bowel but also the regional lymphatics into which the cancer spread, not only proximally, but laterally and distally as well. In his monograph in 1926 he stated: . . . There are, I hold, two main principles to be observed in the surgical treatment of cancer of the rectum and indeed of all cancers wherever they are found, first, ope ration should be based on a knowledge of the demonstrable facts of pathology, and, second, the most extensive operation possible in conformity with that knowl edge should be performed on all patients no matter how small or early the local manifestation of the disease may seem to be . . . This philosophy was appropriate in his time but today, with increased knowledge regarding the biological behavior of cancer of the rectum, the recognition of the importance of the extent of the primary lesion and staging of cancer, and the availability of multiple modalities in the management of neoplastic disease, the philosophy has appropriately changed.

Pour autant la chirurgie reste la pierre angulaire du traitement du cancer du rectum. Objectifs : Décrire les résultats chirurgicaux, oncologiques et fonctionnels d'une technique de proctectomie totale avec exérèse totale du mésorectum (ETM) et anastomose colorectale latéroterminale ultra basse mécanique destinée au traitement des tumeurs du bas et moyen rectum. Matériel et méthode :Une population de 138 malades opérés à 1 'ICM Val d'Aurelle entre 2005 et 2011 selon une technique reproductible a été étudiée de façon rétrospective. Une analyse de résultats chüurgicaux, oncologiques et fonctionnels a été réalisée. Les facteurs associés à la survenue de complications postopératoires ont été identifiés par une estimation des odds ratio (avec les intervalles de confiance à 95%). La survie a été estimée par la méthode de Kaplan-Meier, Les facteurs associés à la confection d'une stomie de protection ainsi qu'à la survenue de séquelles fonctionnelles ont été recherchés par application d'un test du Chi. Résultats La technique a montré une excellente faisabilité avec un taux de conversion stable au cours du temps deI3,8%. Le taux de stomie était de 43,5%. Le taux de complications post opératoire était de 26,8% dont 89,2% ne nécessitant pas de traitement chirurgical. Le temps d'hospitalisation moyen était de 12 jours. Parmi les patients non stomisés, seuls 3,8% avaient une stomie lors d'une reprise chirurgicale, Le taux de récidive locale était de 4,4% et la survie globale à 5 ans de 83%. Il y avait significativement plus de complications postopératoires dans le groupe des patients pour lesquels une stomie était réalisée. Tandis que l'âge, le statut M initial, le statut pT et le caractère complet de la résection (RO et CRM> 1 mm) étaient significativement associés à la survie globale. Les atteintes urinaires et sexuelles étaient présentes dans 12,2% et 54,2% des cas respectivement. Le traitement néo-adjuvant était associé à la survenue de problèmes urinaire et le sexe et le BMI étaient associés à la survenue de séquelles sexuelles. Conclusion Nous montrons dans cette étude qu'une ETM avec anastomose colo-rectale basse peut être réalisée selon une technique fiable et standardisée avec possibilité de sursoir à une stomie de protection dans près de 60% des cas sans sur-risque pour le patient. Elle permet un excellent contrôle local et des taux de séquelles fonctionnelles limités

Le but de notre étude était d'évaluer l'évolution de la mortalité opératoire et de ses causes après exérèse rectale pour cancer dans un centre spécialisé. METHODE : Les patients opérés d'un cancer rectal entre 1980 et 2011 ont été analysés pour évaluer les causes de décès opératoire (J60). La mortalité était classée selon son origine médicale ou chirurgicale. Les facteurs influençant la mortalité ont également été étudiés par analyse multivariée (la période étudiée, l'âge, le score ASA, les antécédents cardio-vasculaires, les antécédents respiratoires, le type de chirurgie, la voie d'abord et la réintervention). RESULTATS : Parmi les 1386 patients ayant eu une exérèse rectale, il y avait 1119 (81 %) résection antérieure, 216 (15,5 %) AAP, 44 (3 %) Hartmann et 7 (0,5 %) pelvectomies. La mortalité était de 2,1 % : 3,3 % la première quinzaine d'années, 1,6 % la deuxième. La mortalité opératoire était principalement d'origine médicale dans la première décade (1980 - 1990 : 80 % médical vs 20 % chirurgical) alors qu'elle était surtout d'origine chirurgicale dans la dernière décade (2001 - 2011 : 33 % médical vs 67 % chirurgical). Les facteurs indépendants de mortalité étaient la période d'étude, l'âge supérieur à 80 ans, les antécédents cardio-vasculaires et la réintervention. CONCLUSION : La mortalité opératoire des exérèses rectales a diminué dans le temps pour tendre actuellement vers 1 %. La diminution des causes de décès d'origine médicale suggère une meilleure prise en charge des patients. Une diminution de la morbidité chirurgicale est désormais nécessaire pour poursuivre cette amélioration. L'adaptation de la prise en charge chirurgicale à chaque type de patient pourrait permettre cette avancée grâce à une "chirurgie à la carte".
