Por tanto, todos somos pacientes, no siempre enfermos. Y como tales, tenemos derechos y deberes que no siempre conocemos. "Falta formaci?n. Las organizaciones los damos a conocer, pero quiz? no llegan a todo el mundo. Por tanto, aplican el sentido com?n. Todos debemos hacerlos valer. ¿C?mo podemos conocer nuestros derechos para poder hacerlos valer? Un breve repaso normativo puede servir de ayuda. La Ley General de Sanidad de 1986 ya establec?a nuestros derechos y deberes fundamentales, que se completaron con la Ley de Autonom?a del Paciente (41/2002), cuyo desarrollo concierne a las comunidades aut?nomas. Otras de las normativas que nos afectan como pacientes son la Ley de Cohesi?n y Calidad del Sistema Nacional de Salud y el real decreto ley, que desde 2018, garantiza de nuevo el acceso a la sanidad a todos los residentes en Espa?a, tras seis a?os sin cobertura para los extranjeros. Todas estas normativas tienen su traslaci?n pr?ctica en leyes o cartas de derechos y deberes de los usuarios de los servicios sanitarios que ha elaborado cada autonom?a. La m?s completa es la carta catalana que, actualizada en 2015, recoge cerca de un centenar de derechos y obligaciones adecuados a los cambios sociales, los retos tecnol?gicos y biom?dicos y los retos ?ticos. De hecho, dos de sus diez ?mbitos se re?eren a la constituci?n gen?tica de la persona y a la investigaci?n y experimentaci?n. En el a?o 2002 la red de ciudadanos europea Active Citizenship Network (ACN) estableci? la llamada Carta Europea de los Derechos de los Pacientes. Este texto, basado en la Carta de los Derechos Fundamentales de Niza (2000) y reconocido por la Uni?n Europea, asegura en su pre?mbulo que deben ser respetados por todos los pa?ses y sistemas nacionales de salud, independientemente de sus limitaciones ?nancieras o econ?micas y de los grupos pol?ticos gobernantes. El tratamiento para garantizar la salud de la persona enferma tambi?n depende de su propia implicaci?n y capacidad para participar en la toma de decisiones terap?uticas. La persona enferma deposita en el saber m?dico su confianza y su esperanza. Pero el paciente est? lejos de ser un sujeto pasivo en el proceso de recuperaci?n. Puede, y es aconsejable que lo haga, exigir un trato personalizado, una mejor relaci?n sanitario-paciente, una adecuada y correcta informaci?n, as? como la posibilidad de participar en las decisiones sobre su salud. Conceptos como los de mejora continua, calidad o excelencia, ya asentados en otros ?mbitos de la sociedad, est?n surgiendo como referentes para los sistemas de gesti?n de las organizaciones sanitarias. Nuevos roles El modelo tradicional de la relaci?n m?dico-paciente se ha basado hasta hace unos a?os en un modelo paternalista: el m?dico, despu?s de escuchar y reconocer al paciente, emit?a el diagn?stico y establec?a un tratamiento. ?l sab?a qu? era lo mejor para el paciente y ?ste consent?a pasivamente, sin apenas recibir informaci?n sobre las distintas alternativas terap?uticas con sus ventajas e inconvenientes. En este tipo de relaci?n el paciente no era m?s que un elemento pasivo que poco ten?a que decir y que pocas veces se atrev?a a cuestionar la opini?n del profesional. Book excerpt: Thanks to remarkable advances in modern health care attributable to science, engineering, and medicine, it is now possible to cure or manage illnesses that were long deemed untreatable. At the same time, however, the United States is facing the vexing challenge of a seemingly uncontrolled rise in the cost of health care. Total medical expenditures are rapidly approaching 20 percent of the gross domestic product and are crowding out other priorities of national importance. The use of increasingly expensive prescription drugs is a significant part of this problem, making the cost of biopharmaceuticals a serious national concern with broad political implications. Especially with the highly visible and very large price increases for prescription drugs that have occurred in recent years, finding a way to make prescription medicinesâ€"and health care at largeâ€"more affordable for everyone has become a socioeconomic imperative. Affordability is a complex function of factors, including not just the prices of the drugs themselves, but also the details of an individual's insurance coverage and the number of medical conditions that an individual or family confronts. Therefore, any solution to the affordability issue will require considering all of these factors together. The current high and increasing costs of prescription drugsâ€"coupled with the broader trends in overall health care costsâ€"is unsustainable to society as a whole. Making Medicines Affordable examines patient access to affordable and effective therapies, with emphasis on drug pricing, inflation in the cost of drugs, and insurance design. This report explores structural and policy factors influencing drug pricing, drug access programs, the emerging role of comparative effectiveness assessments in payment policies, changing finances of medical practice with regard to drug costs and reimbursement, and measures to prevent drug shortages and foster continued innovation in drug development. It makes recommendations for policy actions that could address drug price trends, improve patient access to affordable and effective treatments, and encourage innovations that address significant needs in health care.

The fourth chapter gives an overview on the diagnostic assessment and management of distinct clinical manifestations of CNS (Central Nervous System) involvement in DM patients, and the main topics regarding its pathogenesis are explored. The next chapter focuses on the strategy of neuromuscular blockage for patients with myotonic dystrophy, especially focusing on premedication, induction of anaesthesia, response to muscle relaxants, the crucial points of safe anaesthesiological care and other perioperative triggers. The development of biomarkers is of high importance in the diagnosis and monitoring of diseases and thus is examined with relation to DM1 (myotonic dystrophy type 1) patients. Other chapters summarize the different rehabilitative approaches that have been proposed in this disease; the novel, interesting findings of therapeutic potential to target DM1 with small molecule kinase inhibitors and the promises his may provide to this devastating disorder; the controversial, phenotypical consequences of the interrupted alleles on DM1 patients; and the sleep disorders associated with myotonic dystrophies.

Book excerpt: It is with great pleasure that I write this Foreword to the Proceedings of the International Conference on Behçet’s Disease which was held in Berlin in June 2002. This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.

To understand this vast, but largely unperceived diversity with its untapped genetic, enzymatic and industrial potential, microbial systematics is undergoing a revolutionary change in its approach to describe novel taxa based on genomic/envirogenomic information. The characterization of an organism is no longer bounded by methodological barriers, and it is now possible to fully sequence the whole genome of a strain to study individual genes, or to examine the genetic information by using different techniques. In fact, application of genomics is helping not only to provide a better understanding of the boundaries of genera and higher levels of classification, but also to refine our definition of the species concept. In addition, increased understanding of phylogeny is allowing to predict the genetic potential of microorganisms for biotechnological applications and adaptation to environmental changes. The present Research Topic on “Microbial Taxonomy, Phylogeny and Biodiversity” compiles a collection of papers covering the use of genomic sequence data in microbial taxonomy and systematics, including evolutionary relatedness of microorganisms; application of comparative genomics in systematic studies; or metagenomic approaches for biodiversity studies.
