La complexité de l'entreprise de la recherche et les enjeux éthiques que soulève la conduite des essais cliniques dans les pays africains nous ont interpelés. S'interroger sur les formes de protection des populations vulnérables de ces pays du Sud part du constat suivant : la création des comités d'éthique en Afrique a été réalisée pour faciliter l'expansion de la recherche biomédicale au Sud à partir des textes de référence occidentale. L'application des principes éthiques ainsi développés s'intègre difficilement dans la culture africaine et malgré les bienfaits de la recherche médicale, il persiste pour les populations africaines, une impression d'exploitation. Dans ce contexte et pour proposer de nouvelles pistes de réflexion, il nous a semblé important de rassembler certains principes généraux de l'éthique de la recherche et les textes de référence s'y rapportant, dont l'application en Afrique ne s'articule pas harmonieusement avec les valeurs socioculturelles locales. Les notions du consentement éclairé et de l'autonomie de la personne se heurtent à certaines valeurs africaines comme la vie en communauté avec l'appartenance identitaire à son ethnie, dont les intérêts passent avant ceux de l'intéressé. Les particularités liées au contexte africain comme l'analphabétisme, l'inaccessibilité des soins de santé primaires, différentes représentations traditionnelles paraissent inadaptées aux exigences actuelles de l'éthique de la recherche. Ce contraste entre les pays du Nord et ceux du Sud est très perceptible dans ce domaine. Comment solliciter des participants à un projet de recherche pour l'évolution de la médecine moderne quand 80% de ces potentiels participants se soignent par la médecine traditionnelle ? Précisons, cependant, que l'intérêt de poursuivre la recherche médicale dans les pays du Sud ne peut être mis en cause mais la manière de la conduire nécessite une profonde révision. Ainsi pour nous rendre compte concrètement du processus de création des comités d'éthique chargés de protéger les participants à la recherche médicale, une enquête menée dans 20 pays africains a montré l'évolution de l'éthique dans cette région. La consultation des sites ANRS en Afrique et du réseau panafricain des essais cliniques effectuée ultérieurement a confirmé un fonctionnement évolutif du partenariat Nord-Sud. Mais il persiste une nécessité de contextualisation et une indépendance à renforcer. Cette tâche peut être réservée aux scientifiques et chercheurs en sciences humaines et sociales africains. Il convient d'africaniser les principes fondamentaux de l'éthique de la recherche pour que l'africain adhère à la démarche de la recherche médicale conduite dans son milieu en s'appropriant des principes révisés à partir des repères socioculturels qui l'identifient.

Book excerpt: This book addresses the epistemological, ethical and methodological challenges faced by African scholars in their research practices. The epistemological domain addresses three interrelated topics: how African university students ethnocentric choice of research topics hampers the production of knowledge; researchers adaptation of their research methods to improve validity; and the challenge of knowledge production on community museums plagued by gross inertia, irregularities, uncooperative gatekeepers and inadequate funding. The methodological domain foregrounds issues around scientific rigour, the criteria for what counts as quality research, and reporting standards. Contributors contend that the use of mixed methods provides the best scope for the effective study and evaluation of social issues. They also detail how better-funded projects tend to improve the respect of ethical standards especially as they pertain to the protection of subjects confidentiality. Self-financed researchers on the other hand tend to be less compliant, a challenge that afflict especially younger and inexperienced researchers. This volume thus contributes not only to a critical understanding of the challenges faced by social scientists in contemporary Africa, but also the prospects and mechanisms on how to improve knowledge production processes.

Book excerpt: “The Hidden Patients” looks at questions of gender in psychiatric publications on the colonial Maghreb, which described “normal” and “abnormal” forms of behaviour among the colonised and compared these findings to descriptions of Europeans who had been diagnosed with psychiatric “abnormalities”. Many psychiatric experts claimed that Muslim women rarely went “mad” and that they only accounted for a negligible percentage of the patients cared for by colonial psychiatrists. Consequently, relatively little space was dedicated to female Muslim patients in the theoretical source material, even though case studies and statistics clearly showed that it was mainly an imaginary absence and that it contradicted the everyday experiences of the psychiatrists.

Book excerpt: For the time being African philosophy is treated regularly in research and in teaching at two European scientific institutions: at the University of Vienna and at Erasmus University Rotterdam. In October 1993 there have been held two conferences of Western and African philosophers at both universities. Eleven African and nine Western scholars participated as speakers in these conferences. Four African speakers gave lectures at the Vienna and at the Rotterdam conference. The Vienna conference dealt with general questions of postcolonial philosophy in Africa. The conference at Rotterdam focused on the processes of democratization in African countries since 1989. This volume contains the papers of both conferences. En ce moment la philosophie Africaine est traité regulièrement dans les recherches et dans l'enseignement à deux instituts scientifiques Européens: à l'Université de Vienne et à l'Université Erasme de Rotterdam. En Octobre 1993 deux conférences de philosophes Occidentals et Africains ont été organisé aux deux universités. Onze savants Africans et neuf savants Occidentals ont participé à ces deux conférences. Quatre savants Africains ont présenté des communications à tous les deux conférences de Vienne et de Rotterdam. La conférence Viennoise s'occupait de questions générales de la philosophie postcoloniale en Afrique. La conférence de Rotterdam focusait aux procès de démocratisation dans des pays africains depuis 1989. Se trouvent en ce volume les contributions à les deux conférences.

Book excerpt: This first extensive study of the practice of blood transfusion in Africa traces the history of one of the most important therapies in modern medicine from the period of colonial rule to independence and the AIDS epidemic. The introduction of transfusion held great promise for improving health, but like most new medical practices, transfusion needed to be adapted to the needs of sub-Saharan Africa, for which there was no analogous treatment in traditional African medicine. This otherwise beneficent medical procedure also created a “royal road” for microorganisms, and thus played a central part in the emergence of human immune viruses in epidemic form. As with more developed health care systems, blood transfusion practices in sub-Saharan Africa were incapable of detecting the emergence of HIV. As a result, given the wide use of transfusion, it became an important pathway for the initial spread of AIDS. Yet African health officials were not without means to understand and respond to the new danger, thanks to forty years of experience and a framework of appreciating long-standing health risks. The response to this risk, detailed in this book, yields important insight into the history of epidemics and HIV/AIDS. Drawing on research from colonial-era governments, European Red Cross societies, independent African governments, and directly from health officers themselves, this book is the only historical study of the practice of blood transfusion in Africa.
