Book excerpt: Les réseaux de santé ont vu le jour, dans un contexte d'augmentation de la prévalence des maladies chroniques. L'accroissement des patients poly pathologiques et le cloisonnement du système de santé motivent les institutionnels a proposer l'évolution des réseaux, le plus souvent mono thématique vers une prise en charge polyvalente où la coordination des patients complexes serait le coeur de métier. Mais comment un réseau mono thématique peut il évoluer vers la polyvalence? Et comment s'y organise la prise en charge des patients? Pour tenter d'aborder cette problématique nous avons mené une étude descriptive et analytique. Notre enquête a été menée auprès des coordinateurs de réseaux devenus polyvalents. Elle porte sur cinq points principaux : l'évolution vers la polyvalence, l'organisation du réseau, l'appui à la coordination des soins, la place de l'éducation thérapeutique ainsi que les avantages et inconvénients de cette évolution. Pour réaliser l'étude, nous avons choisi de pratiquer des entretiens semi-directifs. Six réseaux ont accepté de répondre. Au terme de l'étude, il apparaît qu'il n'y a pas un modèle unique de réseau polyvalent mais plutôt des exemples dont on peut s'inspirer. Les organisations doivent s'adapter aux besoins des patients et des soignants. elles-ci proposent un ou des parcours de santé différents. L'appui à la coordination des soins des patients complexes s'appuie sur un nouveau métier : le coordonnateur d'appui. Cette prise en charge est globale et rigoureuse. Il apparait que le développement de l'éducation thérapeutique est une mission importante mais secondaire. L'étude met en lumière l'adéquation de l'organisation des réseaux polyvalents interrogés avec les recommandations de la DGOS. L'évolution vers la polyvalence doit être guidée avant tout par une analyse fine des besoins du territoire et une concertation étroite avec l'ensemble des partenaires.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.

Book excerpt: There are two kinds of football in France. American football was first played in France in 1909 during the cruise of the Great White Fleet. Then, during World War I, the American military shipped footballs, helmets, and shoulder pads alongside rifles and ammunition to the western front. A 1938 tour of two teams lead by Jim Crowley of Fordham University maintained the game until World War II, when the arrival of millions of young Americans in France motivated the U.S. military to sponsor several bowl games. During the 1950s and 1960s, when the United States occupied bases in France during the Cold War, American soldiers, sailors, and airmen played more than a thousand football games. When France withdrew from NATO, however, American bases were forced to close, leaving American football without a natural home on Gallic shores. In the 1970s American college and semi-pro teams tried once more to generate interest in the game among French nationals through a series of tours, but until a French physical education instructor vacationed in Colorado and brought equipment back to France, there was little local enthusiasm for the sport. On the back of that vacation, and from one team in Paris, organized American football in France grew to more than 215 teams with more than 22,000 active players today. Le Football tackles the struggles and successes of American football in France and discusses how, unlike baseball and basketball, football has never been an overt instrument of American cultural influence. Russ Crawford keeps the chains moving as he shows how the modern, homegrown sport developed largely independent of American encouragement into a small but successful culture.
