Book excerpt: Objectifs de l'étude : étudier les connaissances des primipares sur l'insuffisance de lait et évaluer l'impact d'une information donnée durant le séjour à la maternité. - Matériel et méthodes : étude prospective en deux temps réalisée auprès de primipares : d'abord l'interrogation et l'information à la maternité de 19 des 51 patientes de la population globale, puis l'interrogation des 51 patientes au moyen d'entretiens téléphoniques trois mois après l'accouchement. - Résultats : La majorité des patientes ne connaît pas les moyens de prévention de l'insuffisance de lait, les moyens d'y faire face ni les signes objectifs de son diagnostic. Plus de la moitié des patientes ont arrêté d'allaiter avant 3 mois. Le premier motif d'arrêt de l'allaitement était l'insuffisance de lait. L'information donnée en suites de couches n'a pas eu d'influence sur le taux d'arrêt de l'allaitement et la survenue de l'insuffisance de lait. La majorité des patientes a eu besoin de soutien professionnel à domicile. - Conclusions : Il y a un manque d'information de la population étudiée. L'information donnée doit être approfondie et réadaptée à la population ciblée. La diminution du taux d'arrêt prématuré de l'allaitement maternel nécessite un suivi adapté et prolongé après la sortie de la maternité

Book excerpt: Introduction : les bénéfices de l'allaitement maternel, aussi bien pour l'enfant que pour sa mère, sont reconnus scientifiquement. Pourtant, en France, les taux d'allaitement sont parmi les plus bas au monde. La durée moyenne de l'allaitement est de 4 mois et 3 semaines. Au stade de 6 mois, seulement 18,5% des femmes allaitent encore, dont 1,5% de façon exclusive. L'objectif de l'OMS (visant un taux d'allaitement exclusif dans les 6 premiers mois à 50%) relève ici de l'utopie. Objectif : l'objectif principal est d'identifier les causes à l'origine de ce sevrage prématuré. L'objectif secondaire est de recueillir des données sur l'information prénatale, l'accompagnement, la satisfaction des mères... Matériel et méthode : cette étude transversale s'intéresse à des femmes ayant allaité pendant une durée minimale d'un mois, de façon exclusive ou mixte. Le recrutement s'opère en 2016-2017 en Loire-Atlantique (maternité du CHU de Nantes, consultations médicales à Ancenis). Les données sont issues d'entretiens en face-à-face, 6 mois après l'accouchement. Résultats : parmi les 100 femmes interrogées, 54 femmes ont maintenu un allaitement au-delà de 6 mois. En comparaison avec les femmes qui ont allaité moins longtemps, elles sont plus souvent mariées (p=0,02), bénéficient d'un congé postnatal prolongé (p=0,01), profitent d'un « peau-à-peau » à la naissance de plus d'une heure (p=0,005), sont informées à la maternité de la variabilité du rythme des tétées (p=0,03). Elles sont globalement plus satisfaites du déroulement (p=0,003) et du suivi (p=0,04) de leur allaitement. Les 46 autres femmes ont arrêté d'allaiter entre 1 et 6 mois. Les raisons invoquées sont l'insuffisance de lait (39%), la reprise du travail (30,5%), un choix délibéré (15%), des complications de l'allaitement (7%), la reprise d'un traitement médicamenteux (4,5%), la dépression (2%) et la stagnation pondérale du nourrisson (2%). Discussion : plusieurs freins existent en France : un frein psycho-culturel (doute sur la capacité d'allaiter, sentiment d'entrave à la liberté, gêne vis-à-vis de l'allaitement en public, stigmatisation des allaitements prolongés...), la brièveté du congé parental (cependant, nos voisins européens bénéficiant d'une durée de congé similaire obtiennent des résultats nettement supérieurs), un manque d'accompagnement des mères, aussi bien dans la préparation en amont (information avant et pendant la grossesse), que dans le soutien à la maternité puis au retour à domicile. Il est regrettable que le sentiment d'insuffisance de lait soit le premier motif de sevrage, alors qu'il est possible de le juguler, dans la grande majorité des cas, via la réassurance et le rétablissement d'une conduite appropriée. Conclusion : dans une société où les droits des patients retrouvent leur juste place, le choix des femmes en faveur ou en défaveur de l'allaitement ne peut pas être qualifié de choix éclairé à ce jour. La formation du personnel soignant, l'information systématique des femmes, l'accompagnement à la naissance constituent des conditions sine qua non à l'amélioration de la situation française.

Le médecin généraliste peut être le premier recours de la mère allaitante pour lui apporter une information éclairée et un soutien afin de lui éviter un sevrage non souhaité. Notre objectif est d'évaluer les connaissances, attitudes et pratiques des médecins généralistes de la Savoie face à cette insuffisance de production lactée. Nous avons mené une enquête déclarative prospective, par auto-questionnaires anonymes, conduite auprès de 350 médecins généralistes de la Savoie. Nous avons obtenu 73 réponses, soit un taux de participation de 20,8%. La plupart des médecins interrogés possèdent des connaissances théoriques satisfaisantes pour corriger les erreurs de pratique d'allaitement qui conduisent bien souvent à une insuffisance de production lactée. Nombreux sont ceux qui considèrent la plainte d'un « manque de lait » comme subjective, liée à un manque de motivation et/ou de confiance en soi de la mère allaitante, dans un environnement souvent insuffisamment soutenant. Cependant, leur conduite à tenir pour relancer la lactation est parfois parasitée par des idées reçues et des pratiques culturelles erronées et non actualisées. Un besoin de formation a été mis en évidence, tant sur la physiologie de la lactation que sur les aspects pratiques de l'allaitement maternel, afin de fournir aux mères inquiètes un soutien et une information complète et de qualité.

Like the other series, these chronicles provide a survey of significant trends in contemporary philosophical discussion from 1970 to 1985. The need for such surveys has, I believe, increased rather than decreased over the last years. The philosophical scene appears, for various reasons, more complex than ever before. The continuing process of specialization in most branches, the emergence of new schools of thought, the convergence of interest (thought not neces sarily of opinion) of different traditions upon certain problems, the increasing attention being paid to the history of philosophy in discussions of contemporary problems, and the growing signifi cance for philosophical discourse of the social, political and cul tural situation in various regions of the world are the most impor tant contributory factors. Surveys of the present kind are a valu able source of knowledge of this complexity and may as such be an assistance in renewing the understanding of one's own philo sophical problems. The surveys, it is to be hoped, may also help to strengthen a world-wide Socratic element of modern philosophy, the dialogue or Kommunikationsgemeinschaft. So far, five volumes have been prepared for the new series.
