Mais dans le cadre d’un traitement de prothèse amovible sur implants aux difficultés d’élaboration d’une prothèse amovible conventionnelle, viennent s’ajouter des complications implantaires, en particulier le positionnement de l’implant. En effet, ce dernier est doit être positionné en fonction du volume osseux disponible, mais doit tenir compte de l’axe d’insertion prothétique. Une différence trop important entre l’axe implantaire support d’attachement et l’axe d’insertion prothétique aboutirait à une impossibilité de réalisation de la prothèse ou à une maintenance importante. Dans ce travail, nous décrivons un protocole simple, fiable et reproductible nécessitant un logiciel de chirurgie guidée afin de positionner le ou les implant(s) en tenant compte de l’axe d’insertion.

Malgré la prévention et l'amélioration des techniques, le nombre de personnes édentées est croissant dans le monde. Dans le rapport de Consensus de McGill de 2002, la stabilisation d'une prothèse complète mandibulaire par deux implants para-symphysaires est considérée comme le traitement standard minimum que l'on doit proposer chez l'édenté total mandibulaire. Néanmoins le nombre d'implant minimum nécessaire pour améliorer la qualité de vie des patients peut-être discuté. La mise en place d'un implant unique médio-symphysaire pour la stabilisation d'une prothèse amovible complète mandibulaire a été décrite pour la première fois en 1997 par Cordioli et coll. Depuis, de nombreuses études démontrant l'intérêt de ce concept ont été réalisées. Les objectifs de ce travail sont de présenter les indications et les contre-indications de la stabilisation d'une prothèse complète mandibulaire par un implant unique ainsi que les avantages et les inconvénients de cette thérapeutique. Les différentes étapes prothétiques et chirurgicales sont décrites. Enfin, une synthèse de la littérature concernant le pronostic de cette thérapeutique est présentée, en le comparant au système de stabilisation par deux implants.

Compte tenu du vieillissement de la population et donc du développement de l’édentement partiel, les données indiquent que la nécessité de prothèses amovibles partielles va s’accroitre. De nombreux patients la privilégient pour diverses raisons, comme la facilité de nettoyage et la simplification du traitement, en dehors des problèmes de coût. Pour les patients âgés, elle constitue souvent la meilleure solution, notamment pour maintenir une hygiène buccale correcte. Si le patient est prêt à accepter la prothèse et à condition de respecter les principes essentiels, c’est une solution simple techniquement et fiable. Cependant, la PAP n’en garde pas moins ses inconvénients fonctionnels, prophylactiques et cosmétiques. La déception après la livraison des prothèses amovibles partielles est malheureusement fréquente et les échecs sont nombreux. D’une manière générale, la prothèse fixée sera préférée aux prothèses amovibles lorsque cela est faisable. Les implants dentaires sont habituellement utilisés dans le but de mettre en place une solution prothétique fixée. Néanmoins, pour des causes diverses comme des problèmes anatomiques, médicaux ou financiers, il ne sera pas toujours possible de mettre en place suffisamment d'implants pour permettre la réalisation d'une solution prothétique fixe pérenne. Dans ce cas, un nombre réduit d'implants placés stratégiquement permettra d'améliorer de façon significative la prothèse amovible partielle en réduisant un grand nombre de ses défauts. Nous avons développé dans une première partie les problèmes et inconvénients rencontrés avec les prothèses partielles amovibles conventionnelles, puis nous avons discuté des intérêts et des indications de les stabiliser par des implants dans une seconde partie. Le troisième chapitre est consacré à la présentation d’un cas clinique illustré d'un patient du Centre de Soins Dentaires de Rennes ayant bénéficié de ce type de réhabilitation prothétique. Enfin, notre dernière partie fait l’objet d’une étude tendancielle réalisée à l’aide de questionnaires sur la satisfaction de vingt patients porteurs de prothèses partielles amovibles stabilisées par implants.

La conservation de racines dentaires ou encore le placement d'implant dans des zones stratégiques permettent d'apporter un soutien supplémentaire à la prothèse amovible. Dans un premier temps, les caractéristiques de la réhabilitation antérieure en prothèse amovible partielle traditionnelle seront mises en avant. Une deuxième partie s'attardera sur les intérêts et les inconvénients de la conservation radiculaire ainsi que la mise en place d'implant sous la prothèse amovible partielle dans le but d'accroître la rétention, la sustentation, et la stabilité de l'appareil. Pour terminer, différents exemples d'exploitations radiculaires et implantaires seront exposés par l'intermédiaire de systèmes d'attachements sous-prothétiques.

Book excerpt: EU regulations on the safety of medical implants-such as metal-on-metal hip replacements-must be urgently tightened in response to evidence that manufacturers are seeking approval for devices in Member States with the least stringent regulatory regimes. Much greater transparency is needed about the approval process so patients and doctors can have full confidence in the implants they are using. Manufacturers and regulators must also publish more rigorous clinical data on the safety of new implants and be subject to greater scrutiny. The clinical data requirements for high-risk medical devices to be sold on the European Market are much less stringent than for new medicines. There appears to be reliance on equivalence-similarity to an existing implant-rather than clinical investigations of the implant being approved. The Committee calls for all clinical data used in the approval of a medical implant to be published without identifying patients or clinical trial participants. For products currently on the market, such data should be published immediately. There should also be a public record of every approach from a manufacturer to any notified body in the EU so that 'forum shopping' can be identified. UK regulatory body the Medicines and Healthcare products Regulatory Agency (MHRA) is criticised for its slow reaction to reports of problems with metal-on-metal hip replacements in Australia. The withdrawal of PIP breast implants also highlights the need for frequent and unannounced spot-checks of medical implant manufacturers to identify and prevent similar problems.
