Toutefois celle des traumatismes du massif facial, en particulier mandibulaires reste affaire d’écoles et de moyens. Nous avons donc réalisé une étude rétrospective à la fois technique et économique d’une technique de contention mandibulaire par fixateur mandibulaire artisanal utilisé dans notre service. Celle-ci a permis d’inclure 37 patients sur une période de 16 ans. Le coût de cette technique ainsi que ses résultats ont ensuite été discutés en les comparants aux autres techniques permettant la prise en charge de ces patients. Dans notre étude le fixateur externe ‘artisanal’ s’avère être une technique fiable, peu coûteuse, présentant des résultats aussi bien esthétiques que fonctionnels très satisfaisants. Elle démontre donc son intérêt, tant sur un plan technique, qu’économique, face aux prises en charge existantes. Ainsi, nous pensons que, le fixateur externe artisanal que nous proposons devrait intégrer l’arsenal thérapeutique de tous nos centres en tant que technique alternative ou complémentaire de prises en charge de traumatismes mandibulaires complexes.

Book excerpt: Introduction : en 1936, Gustave Ginestet a été le premier à développer un dispositif externe pour la prise en charge des fractures complexes de mandibule. En 1949, Joe Hall Morris a élaboré un système externe d’ostéosynthèse bi-cortical. Le modèle de J.H. Morris nous a conduit à créer un fixateur externe « fait maison ». Celui-ci est basé sur la solidarisation de broches avec une sonde d’intubation, remplie avec une résine auto-polymérisante. L’objectif de cette étude était de réaliser une revue des patients ayant bénéficié de notre fixateur externe durant les 24 dernières années. Matériel et méthodes : nous avons inclus dans cette étude rétrospective, tous les patients traités avec la pose notre fixateur externe, dans notre service de Stomatologie et Chirurgie Maxillo-faciale, de 1995 à 2019. 65 patients étaient répartis dans 2 groupes. 39 patients étaient alloués au groupe ostéosynthèse temporaire (OT) et 26 dans le groupe consolidation osseuse (CO). Les critères de jugement secondaire comprenaient, l’ouverture buccale et les malocclusions. Les critères esthétiques étaient essentiellement focalisés sur la cicatrisation cutanée, celle-ci était évaluée par le chirurgien et le patient. Résultats : 23 patients ont acquis une consolidation osseuse spontanément durant la période où le fixateur était en place. Deux patients ont présenté une pseudarthrose dans le groupe consolidation osseuse. La plupart des patients de l’autre groupe ont bénéficié d’une prise en charge secondaire par greffe osseuse, reconstruction par lambeau libre et distraction ostéogénique. Nous avons relevé peu de complications avec notre technique d’ostéosynthèse durant la période de l’étude. Conclusion : notre fixateur externe inspiré par le système J.H. Morris est une option viable pour la prise en charge des traumatismes du tiers inférieur de la face.
