Book excerpt: Cette étude rétrospective a porté sur 22 observations d'endocardite infectieuse recueillies de janvier 1997 à décembre 1999 aux centres hospitaliers de Pointe-à-Pitre et Basse-Terre en Guadeloupe. La médiane d'âge était de 56,3 années avec des extrêmes de 28 à 77 ans. La prédominance était masculine à 68 %. L'endocardite infectieuse s'était développée sur une valvulopathie dans 17 % des cas et était primitive chez la moitié des patients. L'atteinte du cœur gauche prédominait largement avec Il atteintes mitrales et 6 atteintes aortiques. La porte d'entrée de l'infection a été trouvée chez 64 % des patients, elle était essentiellement cutanée. Trois cas ont été considérés d'origine nosocomiale. Les hémocultures étaient positives dans 81,8 % des cas avec une prédominance pour les Streptocoques (38,5 %). L'échographie a mis en évidence des végétations dans 73 % des cas. L'évolution a été principalement compliquée d'atteinte cardiaque (30 %), d'atteinte pulmonaire (27,5 %) ou d'atteinte neurologique (15 %). La mortalité globale a été de 18 %, liée à une insuffisante cardiaque dans tous les cas. On a eu recours à la chirurgie cardiaque chez 4 patients (18%)
