Book excerpt: Contexte : La fièvre du nourrisson de plus de trois mois est un symptôme fréquent en médecine générale. Plusieurs études quantitatives ont évalué les connaissances et attitudes des parents face à un épisode fébrile. Il est intéressant de connaître les outils et méthodes d'éducation des parents par les médecins généralistes. Méthode : Etude qualitative par entretiens semi-dirigés auprès de médecins généralistes des régions lilloise et valenciennoise. L'analyse des données a été effectuée à l'aide d'un double codage ouvert et axial. Résultats : Seize médecins généralistes ont été interviewés permettant la saturation des données. Le déroulement de la consultation d'un nourrisson fébrile a été expliqué par les médecins, de l'interrogatoire aux prescriptions en passant par l'examen clinique. Les points abordés sont les mesures physiques et le traitement antipyrétique, les consignes de surveillance et les signes devant amener les parents à consulter. La majorité des médecins pense faire de l'éducation thérapeutique. Les outils utilisés sont principalement la répétition des informations et la vérification des acquis par des questions ouvertes, la réassurance des parents et la remise d'informations écrites sur l'ordonnance ou le carnet de santé. D'après eux, les facteurs limitant l'éducation thérapeutique sont l'angoisse des parents, leur expérience, leur niveau intellectuel, le milieu social et le manque de motivation et d'adhésion au traitement. Le manque de temps des médecins est également largement cité. Les rôles de l'éducation thérapeutique selon les médecins généralistes sont de rendre les parents autonomes à la prise en charge initiale de la fièvre du nourrisson, de reconnaître les signes de gravité, de corriger leurs erreurs et leurs croyances trouvant leur source dans les médias et l'entourage familial. Conclusion : Les médecins généralistes définissent l'éducation thérapeutique par les informations et les conseils qu'ils donnent aux parents, ce qui correspond à une infime partie de l'éducation thérapeutique et sous-tend une relation médecin-patient directive.

Book excerpt: The Violence of Modernity turns to Charles Baudelaire, one of the most canonical figures of literary modernism, in order to reclaim an aesthetic legacy for ethical inquiry and historical critique. Works of modern literature are commonly theorized as symptomatic responses to the trauma of history. In a climate that tends to privilege crisis over critique, Debarati Sanyal argues that it is urgent to rethink literary experience in terms that recall its contestatory potential. Examining Baudelaire's poems afresh, she shifts the focus of critical attention toward an account of modernism as an active engagement with violence, specifically the violence of history in nineteenth-century France. Sanyal analyzes a literary current that uses the traditional hallmarks of modernism—irony, intertextuality, self-reflexivity, and formalism—to challenge the historical violence of modernity. Baudelaire and the committed ironists writing in his wake teach us how to read and resist the violence of history, and thereby to challenge the melancholy tenor of our contemporary "wound culture." In a series of provocative readings, Sanyal presents Baudelaire's poetry as an aesthetic form that contests historical violence through rhetorical strategies of complicity, counterviolence, and critique. The book develops a new account of Baudelaire's significance as a modernist by dislodging him both from his traditional status as a practitioner of "art for art's sake" and from his more recent incarnation as the poet of trauma. Following her extended analysis of Baudelaire's poetry, Sanyal in later chapters considers a number of authors influenced by his strategies—including Rachilde, Virginie Despentes, Albert Camus, and Jean-Paul Sartre—to examine the relevance of their interventions for our current climate of trauma and terror. The result is a study that underscores how Baudelaire's legacy continues to energize literary engagements with the violence of modernity.

It highlights continued progress made towards meeting international targets for malaria control to be achieved by 2010 and by 2015. International funds disbursed for malaria control increased from $200 million in 2004 to $1.5 billion in 2009. Since 2008, more than 289 million insecticide-treated mosquito nets have been delivered to sub-Saharan Africa, enough to protect three quarters of the 765 million persons at risk of the disease. Over the last decade, 11 countries in the Africa Region and 32 countries in other regions showed reductions of 50% or greater in either confirmed malaria cases or malaria admissions and deaths. Malaria control is making a major contribution to reducing mortality in children less than 5 years old and to attaining the health-related Millennium Development Goals.

Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
