Dans le monde anglo-saxon, c’est la publication, en 2013, de la cinquième édition de l’outil d’évaluation et d’accompagnement Diagnostic and Statistical Manual of Mental Disorders (DSM-5) qui est en vigueur. Elle classe le trouble du spectre de l’autisme dans les troubles neuro-développementaux. Sémantique précisée, l’autisme renvoie à une catégorie de troubles neuro-développementaux qui recouvrent des situations cliniques diverses. Il rend compte de situations de handicap hétérogènes qui impliquent la proposition de réponses variées et adaptées aux spécificités propres et singulières de chaque situation vécue. Cette diversité est précisée sous forme de catégories ou sous forme dimensionnelle. L’idée s’est développée que ces catégories sont de simples variantes d’une même pathologie et constituent le continuum d’un même trouble : le trouble du spectre de l’autisme (TSA). À PROPOS DE L'AUTEUR Christian Alin est Professeur émérite des universités en Sciences de l’éducation à L’ESPE de l’université de Lyon1 et membre du laboratoire L’VIS - EA-7428, Lyon. Basée sur la logique de l'évaluation anthropologique, cette recherche évalue premièrement la cohérence entre les caractéristiques spécifiques de l'autisme et les différents dispositifs de prise en charge. Ensuite, nous proposons une évaluation classique de l'efficacité de la prise en charge à l'école au moyen de différentes observations (ouvertes et structurées) d'interactions pédagogiques et éducatives entre enseignants et élèves. Les trois "découvertes" principales de cette recherche sont : 1. La conception personnelle du responsable de l'établissement ou de l'enseignant sur l'autisme est un facteur qui influence de manière déterminante les conditions d'une prise en charge. 2. On observe des complémentarités entre les nouvelles approches éducatives spécialement destinée aux enfants autistes et la pédagogie générale comme la pédagogie Montessori dans la pratique de l'enseignement. 3. La prise en charge des enfants autistes à l'école est une modalité pertinente en considérant ses effets éducatifs notamment dans l'instauration d'un rapport pédagogique enseignant / élève, la prise de conscience par l'élève autiste de sa stratégie d'apprentissage et la diminution des difficultés d'apprentissage.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?

Although internationally recognised for her pioneering therapeutic work with autistic children, her approach is considered by some to be controversial, as her psychogenic view of childhood autism challenged the belief that it is biological and genetic. Autistic States in Children is widely regarded as a vitally important work for understanding the causes of autism in young children. Vividly describing her clinical encounters with autistic children, Tustin argued that autistic states were above all self-protective ones. In her observational studies, she noted how autistic children's interaction with physical objects, such as keys, toy cars, or other play items, had a rigid and ritualistic quality, far removed from the typical kind of fantasy play seen in other children. Such objects are not used by autistic children for their intended purpose, Tustin argued, but rather in sensation-dominated ways that interfere with mental development. She also drew a fundamental distinction between two autistic groups: an ‘encapsulated’ group, which is withdrawn and non-verbal, and an ‘entangled’ group, who are hyperactive and chaotic but have some language. Indeed, one child out of 20 experiences one or more seizures before the age of 5, and one in a hundred develops epilepsy as a chronic disorder. Approximately half of these children with epilepsy display academic difficulties and/or behavioral disorders. Moreoever, it is now believed that a sizable proportion of children with learning disability suffer from undiagnosed epilepsy. While a great number of textbooks have been devoted to various medical aspects of chi- hood epilepsy (diagnosis, genetics, etiology, drug and surgical treatment, etc.), there have been no comprehensive accounts of the cognitive consequences of this condition. Advance of medical knowledge has shown that childhood epilepsy should not be considered as a single disorder but encompasses a whole range of different conditions that exhibit specific clinical EEG and outcome characteristics. It is not becoming apparent that these various clinical entities have different cognitive expression that yet need to be specified. The purpose of this book is to provide a complete up-to-date analysis of this multi-faceted pathology.
