Book excerpt: La stigmatisation et la marginalisation, associées aux troubles graves de santé mentale (schizophrénie, psychose, trouble bipolaire), ne sont plus à démontrer. Malgré les avancées des traitements pharmacologiques, l'intégration sociale de cette population demeure un enjeu important et la rencontre clinique avec l'individu étiqueté schizophrène, serait elle-même un lieu à l'intérieur duquel se rejouent les impacts sociaux de la maladie. Plus particulièrement, ces individus sont soumis à des mesures de contrôles particulières, dont l'autorisation judiciaire de soins pour cause de troubles mentaux. Le rapport à la parole joue un rôle important dans l'évaluation de la dangerosité d'un individu souffrant d'un trouble mental et le délire y tient une place privilégiée. En privilégiant l'angle de l'écoute lors de la pratique du travail social avec des individus souffrant de troubles mentaux, cette recherche explore une dimension de l'intervention bien souvent minimisée, ou même occultée, par un faire ou un dire. L'écoute du délire serait donc un exercice ceint par un ensemble de discours et de contraintes sociales, visant à répondre à un souci de sécurité et d'efficacité. La nature particulière du travail social en santé mentale se heurte à différentes idéologies et celles-ci ont un impact sur l'écoute. De plus, en tant que discipline et forme d'aide particulière, l'exercice de cette profession est appelé à se redéfinir et approfondir ses approches. Cette étude qualitative, réalisée auprès de onze travailleuses sociales québécoises, vise à explorer le dispositif de l'écoute dans la relation clinique, dans sa définition et sa mise en pratique. Ces entrevues ont abordé également l'apprentissage de l'écoute, les critères associés à une bonne écoute ainsi que l'impact de la rencontre avec un individu qualifié de délirant sur l'écoute. Finalement, ont été esquissées les bases de ce que constituerait une écoute propre à l'exercice du travail social. Les résultats de notre analyse, d'orientation phénoménologique, ont permis de jeter un éclairage différent sur l'écoute, une dimension fondamentale, mais implicite de l'intervention sociale. Par ailleurs, le contact avec le délire comme situation extrême d'écoute, nous a permis de relever différents dispositifs mis en place par les travailleuses sociales pour pallier aux particularités liées à cette prise de parole. À des niveaux intrapersonnels, interpersonnels et sociaux-culturels, l'écoute du délire comporte son lot de conséquences venant confronter la position de la travailleuse sociale dans son offre de service. Ainsi, en adjoignant une dimension éthique à l'écoute, en redéfinissant la vision idéalisée de l'écoute et en prenant en considération les différents niveaux de complexité liés à son exercice, l'écoute pourrait retrouver une dimension d'efficacité, plus particulièrement avec des individus délirants.

La perspective des travailleurs sociaux est centrale dans l'étude, comme le démontre la question de départ : quelles significations les travailleurs sociaux associent-ils à leurs activités professionnelles ? Méthodologie : Des entrevues semi-dirigées ont été réalisées avec dix (10) travailleurs sociaux de l'Institut universitaire en santé mentale de Québec (IUSMQ) d'avril à juin 2015. Ces entrevues comprenaient deux volets : (1) la pratique des travailleurs sociaux en 3e ligne psychiatrique à l'IUSMQ ; (2) un récit de pratique à l'aide d'un exemple clinique concret. Les transcriptions des entrevues ont été soumises à une analyse thématique des contenus fondée sur une grille de codification combinant des catégories prédéterminées et des catégories émergentes du matériel. Résultats et discussion : Le thème central issu de nos analyses est la notion de lien, qui serait au cœur des pratiques de travail social : les liens que les travailleurs sociaux créent, préservent ou rétablissent avec les usagers, les familles, les autres intervenants et les différents partenaires. Les travailleurs sociaux actualisent des rôles différents dans le cadre de leurs diverses activités en 3e ligne psychiatrique. Or, la compréhension de leur rôle dans les diverses équipes interdisciplinaires influence le déroulement de la collaboration ainsi que l'atteinte des objectifs établis avec la clientèle. Les récits de pratique soulignent que la gestion des cas cliniques de 3e ligne nécessite l'intervention de plusieurs partenaires et demandent l'utilisation de nouvelles approches d'intervention. Les collaborations constructives et efficaces ainsi que l'inclusion des familles dans les interventions complexes apportent un grand appui dans l'actualisation du plan d'intervention ou de sortie des usagers. Enfin, les diverses formes de soutien clinique, l'accès aux formations et la flexibilité du travail réalisé facilitent le travail au quotidien. Renault argues that social suffering should be taken seriously in social theory as well as in social critique and provides a systematic account of the ways in which social suffering could be conceptualised. He goes on to inquire into the political uses of references to social suffering, surveys contemporary controversies in the social sciences, and distinguishes between economical, socio-medical, sociological, and psychoanalytic approaches, before proposing an integrative model and discussing the implications for social critique. He claims that the notion of social suffering captures some of the most specific features of the contemporary social question and that the most appropriate approach to social suffering is that of an interdisciplinary critical theory of society.
