Book excerpt: OBJECTIFS: L'objectif principal de notre étude visait à définir les impacts de l'autonomie dans la formation initiale des sages-femmes de Nancy. Pour cela, nous avons étudié et évalué un format pédagogique précis : le stage de PNP en autonomie à la MRUN entre 2013 et 2015. Notre objectif secondaire était d'évaluer la préparation à ce stage, l'accompagnement au ncours de ce stage ainsi que son évaluation. MÉTHODE : Nous avons réalisé une étude descriptive, transversale et observationnelle, en réalisant une enquête par questionnaire évaluant le vécu et le ressenti des étudiants vis à vis de l'autonomie qui leur est donnée au cours du stage de PNP en 4ème année. RÉSULTATS : L'autonomie en stage peut-être déstabilisante et angoissante mais une fois ces difficultés dépassées, l'autonomie dans la formation favorise l'autonomie future en stage, aide à la maîtrise des connaissances, facilite la communication, permet un réel investissement personnel de l'étudiant dans sa formation et engendre la projection de l'étudiant dans son futur métier à responsabilité. De plus, la préparation au stage semble satisfaire les ESF. Pour finir, l'accompagnement avant, pendant et après le stage ne semble pas convenir aux ESF. Ceux-ci suggèrent un encadrement et des temps d'échange permettant un réel dialogue afin de transformer positivement cette expérience et d'en tirer un grands nombres de bénéfices pour leurs stages et leur activité professionnelle futurs.

Les sages-femmes en sont les acteurs privilégiés, notamment de la préparation à la naissance et à la parentalité (PNP) en période prénatale. Leur formation initiale se fait en alternance entre enseignements théoriques et stages. Elles assurent donc parfois une double mission : éducative auprès des femmes enceintes et formative auprès du stagiaire. Faisant l’hypothèse que les ancrages théoriques des pratiques éducatives et formatives des sages-femmes sont identiques, l’objectif de recherche est d’identifier ces ancrages en situation de PNP et d’encadrement d’un étudiant. La méthode de l’auto-confrontation simple et l’entretien de compréhension ont été utilisés. 14 sages-femmes volontaires ont participé à l’étude. Les données ont été traitées selon la méthode de l’analyse du contenu à partir d’une modélisation combinant des modèles d’éducation, d’apprentissage et de santé. Les principaux résultats valident l’hypothèse, avec cependant quelques exceptions. Les pratiques de transmission des recommandations et savoirs médicaux ou d’activité corporelle s’inscrivent majoritairement dans les combinatoires à la croisée des modèles béhavioristes, de l’éducation centrée sur l’instruction et de la santé biomédicale. Les modèles du développement du sujet ou de la santé biopsychosociale sont ponctuellement présents, alternant avec ceux qui sont prédominants. Les combinatoires basées sur le constructivisme et le modèle de santé du sujet autonome n’apparaissent qu’exceptionnellement. Les mêmes tendances sont retrouvées pour les modélisations éducation/apprentissage sous-jacentes aux pratiques formatives.

Les sages-femmes françaises ont-elles les compétences et la formation initiale nécessaires afin d’y exercer en toute efficacité et d’en garantir l’entière responsabilité ? D’après 71,10% des professionnels, il semble que la sage-femme française soit compétente afin d’assumer la responsabilité des maisons de naissance. Elle n’a rien de fondamental à envier à la formation de ses consœurs étrangères. Il semble que la différence de « pouvoir » soit essentiellement due à une différence d’autonomie.

Book excerpt: Contexte : L'apprentissage de la gestion du travail physiologique par les étudiants sages-femmes a le plus souvent lieu auprès de femmes bénéficiant d'une analgésie péridurale et au sein de maternités dont les pratiques obstétricales évoluent. Objectif : Apprécier au cours de la formation initiale, l'adéquation entre la formation théorique et pratique en matière de gestion du travail physiologique. Méthode : Nous avons réalisé une étude observationnelle quantitative, prospective et multicentrique sur une durée de 3 mois, entre octobre et décembre 2017. Nous avons inclus toutes les sages-femmes nouvelles diplômées de l'année 2017 des 35 écoles de France. Nous avons créé un questionnaire en ligne comportant une vignette clinique et des échelles d'autosatisfaction. Nous avons ensuite effectué une analyse statistique des résultats grâce au logiciel épidémiologique Open-Epi. Résultats : Nous avons relevé que 68,6% des sages-femmes ont « plutôt » le sentiment de connaître le déroulement du travail physiologique et que l'enseignement théorique est « tout à fait » adapté pour seulement 5,8% d'entre elles. Par ailleurs, les nouvelles diplômées sont « plutôt » satisfaites de leur formation clinique à la gestion du travail physiologique mais plus de la moitié d'entre elles décrivent un nombre d'opportunité d'apprentissage « plutôt pas » suffisant ainsi que des lieux de stage « plutôt pas » adaptés en matière de gestion du travail physiologique. Enfin, avec une moyenne de 7 sur 10, on relève que les sages-femmes ont globalement le sentiment d'être compétentes pour gérer le déroulement d'un travail physiologique. Conclusion : En regard de l'évolution des pratiques obstétricales et des opportunités d'apprentissage peu rencontrées en salle de naissance, l'étudiant sage-femme associe différents moyens d'apprentissage à sa formation initiale pour être compétent à la gestion du travail physiologique.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments?
