L'élimination des bactéries parodonto-pathogènes est donc déterminante dans leur traitement. Le détartrage et le débridement non chirurgical sont incontournables, cependant, ces thérapeutiques peuvent avoir certaines limites qui imposent le recours à une antibiothérapie. L'application locale des antimicrobiens est une alternative thérapeutique complémentaire au traitement parodontal conventionnel. Elle repose sur une diffusion contrôlée du principe actif incorporé dans des dispositifs à libération lente. Elle permet d'obtenir de fortes concentrations antibiotiques sur les sites cibles. Ce travail a pour but d'évaluer l'efficacité de l'antibiothérapie locale à travers une revue de la littérature.

Quelques généralités sur les parodontopathies seront tout d'abord rappelées en insistant tout particulièrement sur le facteur étiologique bactérien (description de la flore parodontopathogène et notion de biofilm). L'arsenal thérapeutique à disposition du praticien pour combattre ces maladies sera ensuite décrit (rôle du traitement parodontal mécanique et des prescriptions d'antimicrobiens). La seconde partie s'attardera sur les antibiotiques et les antiseptiques à travers une description des molécules prescrites en odontologie et leurs mécanismes d'action sur les bactéries, le tout afin de mieux cerner leur apport à la thérapeutique parodontale. La dernière partie traitera des intérêts et des limites des antimicrobiens. Ces substances présentent notamment des effets secondaires et peuvent présenter une action limitée du fait du développement de résistances bactériennes. Ces phénomènes de résistances seront détaillés avant d'envisager quelles sont les alternatives actuelles ou à venir à disposition du praticien pour mieux traiter la maladie parodontale.

Book excerpt: La maladie parodontale est une maladie d'origine infectieuse directement induite par les bactéries de la plaque dentaire, et modulée par différents facteurs (tartre, facteurs hormonaux, génétiques....). Le traitement mécanique par le détartrage-surfaçage peut être complété par un traitement chimique par antibiotiques. A l'aide d'études cliniques récentes, les différents antibiotiques utilisés, les bénéfices qu'ils apportent, leurs modalités de prescription et leurs indications cliniques sont comparés. Puis ces données sont confrontées aux résultats des revues de synthèses et méta-analyses réalisées à partir d'études antérieures. On ne peut trouver de véritable consensus concernant l'utilisation des antibiotiques dans le traitement de la maladie parodontale. Cependant certaines analogies permettent de prodiguer quelques conseils. SRP en une fois si possible, amoxicilline + métronidazole dans le cas de parodontite agressive, pas de traitement chimique sans traitement mécanique, application d'antibiotiques locaux au niveau des poches profondes ne répondant pas au traitement initial. Cette étude bibliographique nous permet tout de même de proposer un protocole de prise en charge de la maladie parodontale, mais il sera peut être modifié aux vues des fururs études à ce sujet.
