Book excerpt: Les différentes langues qu'utilise la personne bilingue dans sa vie quotidienne sollicitent des aires cérébrales communes, principalement situées dans l'hémisphère gauche, dont les activations varient selon plusieurs facteurs. L'ensemble des représentations mentales non linguistiques, appelé « système conceptuel », est unique pour toutes les langues. Il est associé à des représentations sémantiques (caractéristiques physiques nommables) spécifiques à chaque langue, qui ont leur propre système lexical et phonologique. Les études montrent qu'un lien fort existe entre bilinguisme et fonctions exécutives (FE) en termes de contrôle des langues (contrôle cognitif). Par ailleurs, la prise en charge de ces FE dans le cadre d'aphasie monolingue apporte une meilleure récupération des troubles langagiers, notamment dans l'anomie, symptôme récurrent dans l'aphasie. Pourtant, aucune étude n'a encore porté sur les liens entre contrôle exécutif, bilinguisme et aphasie au niveau de la rééducation. Cette étude s'attachait donc à proposer une thérapie intensive de 20 séances, basée sur l'inhibition, la flexibilité et la mémoire de travail chez des patients aphasiques bilingues. L'objectif a été d'évaluer, au travers d'une étude de cas, les effets de ce protocole sur l'anomie et sur les capacités exécutives. Un patient a suivi la thérapie pendant qu'une autre poursuivait sa rééducation habituelle. Si les capacités discursives (dans les deux langues) et exécutives du sujet ayant suivi le protocole se sont améliorées, la thérapie a eu peu d'effets sur l'anomie en dénomination. Chez la patiente qui a suivi ses séances habituelles, aucune évolution n'a été constatée ni en langage ni aux épreuves exécutives. Ainsi, bien que ces résultats doivent être traités avec prudence, ce travail met en avant l'importance de prendre en considération le patient aphasique bilingue dans sa globalité en tenant compte du bilinguisme dans les troubles langagiers mais aussi des FE lors de la prise en soin.
