Book excerpt: L'accompagnement familial, pratique en plein essor en orthophonie, s'inscrit dans le courant écologique (Bronfenbrenner, 1994). Selon le contexte de l’intervention, trois types d’accompagnement familial se distinguent : le type I « information » où l'orthophoniste transmet au Parent des éléments du dossier de l’enfant ; le type II « collaboration » où le soignant donne des ressources et amène les Parents à agir ; le type III « intervention » où le Parent est acteur de la prise en charge de l’enfant, l’orthophoniste l’aidant à développer ses capacités communicatives. Ce travail vise l'étude des différentes pratiques en accompagnement familial et des facteurs influençant ces pratiques dans les interventions orthophoniques au Québec et en France. A partir de questionnaires adressés aux orthophonistes québécois et français, les données ont été comparées et affinées par des entretiens réalisés auprès de soignants et de parents québécois et français. Les résultats, obtenus par des statistiques descriptives, montrent que les orthophonistes québécois utilisent de manière prépondérante un accompagnement familial de type II « collaboration » et III « intervention ». Au Québec, la relation orthophoniste-Parent est donc horizontale, elle repose sur le principe d’ « empowerment ». En France, les orthophonistes utilisent majoritairement le type I « informations », leur relation avec le Parent suit un schéma de soins vertical traditionnel. Les résultats ont aussi montré un impact de la formation initiale et de l’organisation des systèmes de soins sur les pratiques orthophoniques en accompagnement familial. Considérant les trois types d’accompagnement familial existants, il serait intéressant de se poser la question de la diversification de leur utilisation, en France, pour une meilleure adaptation au contexte de vie du patient et donc une meilleure efficacité des soins.

Instead, the reverse seems to be true. Obviously, the use of such tools requires considerable skill at the center of coordination, to borrow an expression from a chapter in this volume.
