Book excerpt: In Japan, cadaveric donor liver transplantation is not common even though cadaveric organ transplantation was legally established in 1998. In contrast, the number of living donor liver transplantations is increasing, with more than 1700 cases at 43 Japanese institutes by November 2001. Indications for and have become living donor liver transplantation are widening in Japan similar to those for cadaveric donor liver transplantation in the United States and Europe. At the same time, split liver transplantation from cadaveric donors shares some technical aspects with living donor liver transplantation. Remarkable progress has been reported recently, and thus it was an auspicious time to hold a symposium on "Current issues in liver/small bowel transplantation" in Japan. We were honored to hold a very fruitful symposium sponsored by the Keio University Medical Science Fund and to bring together top-rank transplant surgeons from Japan and other countries. It was a productive and rewarding time for all participants. We were able to share our experience through excellent presentations followed by active discussions and insightful com ments. At the symposium, we focused on current issues in liver transplanta tion such as widening indications for viral hepatitis and malignant tumors. We also discussed technical aspects and physiological problems in split/iiving donor liver transplant, novel strategies in immunosuppression, and the current status and future prospects in small bowel transplantation. This book contains the papers from all the distinguished guest speakers, focusing on the topics discussed at the symposium.

Book excerpt: We organized the Seventh International Winter Conference on Neurodegeneration and Neuroinflammation in a small town of Karuizawa in Nagano Prefecture in Japan on January 20 to 22, 1999. Karuizawa is a nice summer as well as winter resort close to the place for winter Olympic game in the year of 1998. Over 40 scientists gathered together and made hot discussion. Neurodegeneration and neuroinflammation are two major and important neurologic disorders, in which satisfactory neuroprotective and neurorestorative treatment is not available yet. For this purpose, understanding of molecular mechanisms of neuronal cell death in these two disorders is imperative. Recently, pathologic processes common to these two groups of disorders have been identified such as the involvement of inflammatory changes, microglia, cytokines, and apoptosis. We intended to involve scientists working in these two major fields together to participate in hot and fruitful discussion for the understanding of neuronal death and for developing newer methods of treatment. The science is progressing so rapidly today and we are working in a very narrow specialized area. Sometimes, we are ignorant about important discoveries in other fields. This conference was a nice opportunity for scien tists working different areas to meet together and to exchange their experi ence. This volume is the proceedings of this Winter Conference.

Book excerpt: This title was first published in 2003: This book provides an evaluation of the Gateshead Community Care Scheme which was devised as an alternative to residential and hospital care for frail elderly people. An important feature of the scheme was the decentralization of control of resources to individual social workers acting as care managers, with defined caseloads and expenditure limits to ensure accountability. The initial social social care scheme was subsequently extended to provide both health and social care to clients from a large general practice based in a health centre. The social care team was enlarged to include a nurse care manager and part-time doctor and physiotherapist. The study examines the operation of care management in both settings, the use of devolved budgets and services developed, the outcomes for clients and carers and the costs of care. Admissions to residential care were reduced and the elderly people who received the scheme’s support experienced a better quality of care and greater well-being when compared with elderly people receiving the usual range of services. This was achieved at no greater cost. The characteristics of those for whom the scheme was most appropriate are described. In addition, the pattern of development of the scheme as it was incorporated into the mainstream of the Social Services and after the implementation of the NHS and Community Care Act are examined. Final, the implications for the development of care management are considered.

After the end of the conflict anecdotal reports of various disorders affecting troops who fought in the Gulf began to surface. This mysterious illness was given the name “Gulf War Syndrome” (GWS). This book is an investigation into this recently emergent illness, particularly relevant given ongoing UK deployments to Iraq, describing how the illness became a potent symbol for a plethora of issues, anxieties, and concerns. At present, the debate about GWS is polarized along two lines: there are those who think it is a unique, organic condition caused by Gulf War toxins and those who argue that it is probably a psychological condition that can be seen as part of a larger group of illnesses. Using the methods and perspective of anthropology, with its focus on nuances and subtleties, the author provides a new approach to understanding GWS, one that makes sense of the cultural circumstances, specific and general, which gave rise to the illness.
