That is no longer the case, and FDA's tools need to keep pace with the challenges.

The recent Salmonella contamination of peanut butter products and jalapeño and Serrano peppers, E. coli outbreaks in spinach and lettuce, and reports about cattle slaughter practices and the safety of farm-raised fish in China have all heightened anxieties about the vulnerability of the nationâ€TMs food supply. Approximately 76 million Americansâ€"one in fourâ€"are sickened by foodborne disease each year. Of these, an estimated 325,000 are hospitalized and 5,000 die. According to the World Health Organization (WHO), foodborne illnesses include “...diseases, usually either infectious or toxic in nature, caused by agents that enter the body through the ingestion of food.â€ŗ Foodborne diseases caused by major pathogens alone are estimated to cost up to $44 billion annually in medical costs and lost productivity. Major outbreaks can also contribute to significant economic losses in the agriculture and food retail industries, which account for approximately 13 percent of the U.S. gross domestic product (GDP) and are the largest industries and employers in the United States. Americans spend more than $1 trillion on food annually. This report examines the existing programs at U.S. Department of Health and Human Services (HHS) and examines ways to strategically restructure the agencies at HHS to better protect the nationâ€TMs food supply.

Book excerpt: While large, multinational corporations have supported the removal of tariffs, behind the scenes these firms have fought for protection in the form of product regulations, including testing, labeling, and registration requirements. Unlike tariffs, these regulations can raise fixed costs, excluding smaller firms from the market and shifting profits toward global giants. Narrowing the Channel demonstrates that globalization and globalized firms can paradoxically hinder rather than foster economic cooperation as larger firms seek to protect their markets through often unnecessarily strict product regulations. To illustrate the problem of regulatory protectionism, Robert Gulotty offers an in-depth analysis of contemporary rulemaking in the United States and the European Union in the areas of health, safety, and environmental standards. He shows how large firms seek regulatory schemes that disproportionately disadvantage small firms. When multinationals are embedded in the local economy, governments too have an incentive to use these regulations to shift profits back home. Today, the key challenge to governing global trade is not how much trade occurs but who is allowed to participate, and this book shows that new rules will be needed to allow governments to widen the benefits of global commerce and avoid further inequality and market concentration.

All told, contaminated food causes 48 million illnesses, 128,000 hospitalizations, and 3,000 deaths each year in the United States. With Outbreak, Timothy D. Lytton provides an up-to-date history and analysis of the US food safety system. He pays particular attention to important but frequently overlooked elements of the system, including private audits and liability insurance. Lytton chronicles efforts dating back to the 1800s to combat widespread contamination by pathogens such as E. coli and salmonella that have become frighteningly familiar to consumers. Book excerpt: Globalization of the food supply has created conditions favorable for the emergence, reemergence, and spread of food-borne pathogens-compounding the challenge of anticipating, detecting, and effectively responding to food-borne threats to health. In the United States, food-borne agents affect 1 out of 6 individuals and cause approximately 48 million illnesses, 128,000 hospitalizations, and 3,000 deaths each year. This figure likely represents just the tip of the iceberg, because it fails to account for the broad array of food-borne illnesses or for their wide-ranging repercussions for consumers, government, and the food industry-both domestically and internationally. A One Health approach to food safety may hold the promise of harnessing and integrating the expertise and resources from across the spectrum of multiple health domains including the human and veterinary medical and plant pathology communities with those of the wildlife and aquatic health and ecology communities. The IOM's Forum on Microbial Threats hosted a public workshop on December 13 and 14, 2011 that examined issues critical to the protection of the nation's food supply. The workshop explored existing knowledge and unanswered questions on the nature and extent of food-borne threats to health. Participants discussed the globalization of the U.S. food supply and the burden of illness associated with foodborne threats to health; considered the spectrum of food-borne threats as well as illustrative case studies; reviewed existing research, policies, and practices to prevent and mitigate foodborne threats; and, identified opportunities to reduce future threats to the nation's food supply through the use of a "One Health" approach to food safety. One in three children nationwide—and half of Black children—now encounter CPS during childhood. In Investigating Families, Kelley Fong provides an unprecedented look at the inner workings of CPS and the experiences of families pulled into its orbit. Drawing on firsthand observations of CPS investigations and hundreds of interviews with those involved, Fong traces the implications of invoking CPS as a “first responder” to family misfortune and hardship.
