Book excerpt: With the recent approval of the first eosinophil-depleting therapeutic agents targeting the IL-5 pathway for treatment of severe eosinophilic asthma, eosinophils and eosinophilic disorders are in the limelight. Indeed, setbacks during clinical development of these compounds have revealed how much remains to be known about eosinophil biology in vivo, and have nurtured profuse research both on basic eosinophil biology and on pathogenic disease mechanisms, in order to better delineate the most meaningful targets for innovative therapeutic strategies. On one hand, variable degrees of eosinophil depletion observed in some compartments during IL-5-targeted treatment indicate that certain eosinophil subsets may not rely on this cytokine and/or that other important pro-eosinophilic mediators and signaling pathways are operative in vivo. On the other hand, it is increasingly clear that disorders involving eosinophils such as asthma are the final outcome of complex interactions between diverse cell types and mediators, beyond eosinophils and IL-5. These include type 2 helper T (Th2) cells and innate lymphoid cells, mast cells, and a variety of factors that either activate eosinophils or are released by them. Although a considerable amount of research has focused on asthma because it is a common condition and because management of severe asthma remains a major challenge, several rare eosinophilic disorders with more homogenous features have proven to be extremely useful models to reach a better understanding of the involvement of eosinophils in tissue damage and dysfunction, and of the micro-environmental interactions operating within the complex network of eosinophilic inflammation. Unraveling this interplay has resulted in advances in the development of molecular tools to detect disease subsets and to monitor therapeutic responses, and in identification of promising new therapeutic targets. This Research Topic dedicated to eosinophilic conditions covers aspects of the biology of eosinophils and closely related cells of particular relevance for drug development, reports on translational research investigating pathogenic mechanisms of specific eosinophilic disorders in humans that will likely result in significant changes in the way patients are managed, and presents an overview of the current advancement of targeted drug development for these conditions, with a special focus on asthma.

Analyses of original data suggest a series of first evidence for direct link between inflammation and developmental phases of immune dysfunction in multistep tumorigenesis and angiogenesis. The only evidence presented on initial events for interactions and synergies between activated host and recruiting cells toward tumorigenesis. Effective immunity was defined as balance between two highly regulated and biologically opposing arms, Yin and Yang of acute inflammation, an amazingly precise signal communications between immune and non-immune systems requiring differential bioenergetics. Unresolved inflammation is a common denominator mapping aging process and induction of ‘mild’, ‘moderate’ or ‘severe’ immune disorders including cancers. Our knowledge of the fascinating biology of immunity in health or chronic diseases is fragmentary, chaotic and confusing, particularly for cancer science. Lack of progress in curing majority of chronic diseases or cancer is primarily due to the fact that scientists work on isolated molecules/cells or topics that are funded and promoted by decision makers in medical/cancer establishment. Despite existence of over 25 million articles on cancer-related topics, cancer biology and cure remain mysteries to be solved. After a century of cancer research, the failure rates of therapies for solid tumors are 90% (+/-5). Current reductionist views on cancer science are irresponsible, shut-gun approaches and create chaos. Outcomes are loss of millions of precious lives and economic drain to society. Very little is known about initial events that disturb effective immunity whose function is to monitor and arrest growth of cancerous cells or defend against other external or internal hazardous agents that threaten body’s survival. The author demonstrates the serious need for systematic understanding of how immune disruptors and aging process would alter effective immunity. Outcomes of proposed orderly studies are expected to provide logical foundations for cost-effective strategies to promote immunity toward a healthier society. The policy makers and medical/cancer establishment are urged to return to the common sense that our Forefathers used to serve the public.

Book excerpt: Despite not being a disease in and of itself, antibiotic resistance could be considered the global epidemic of modern times, since it produces the failure to prevent and treat many infectious diseases. This can ultimately lead to untreatable microbial infections becoming more widespread and this will significantly increase morbidity and mortality. This worldwide problem is estimated to cause millions of deaths per year and could become an even more significant menace to humanity than established illnesses, such as cancer. In February 2017, the World Health Organization (WHO) published a list of antibiotic-resistant “priority pathogens” – a catalogue of 12 families of bacteria which pose the greatest threat to human health - and Acinetobacter baumannii is leading the list. The most critical group includes multidrug-resistant bacteria, which pose a particular threat in hospitals, nursing homes, and among patients whose care requires devices such as ventilators and blood catheters. This group includes Acinetobacter, Pseudomonas, and various Enterobacteriaceae and they are often associated with deadly infections, such as bloodstream infections and pneumonia. Furthermore, these bacteria have become resistant to a large number of antibiotics, including carbapenems and third generation cephalosporins – the best available antibiotics for treating multidrug-resistant bacteria. A. baumannii is a particularly worrisome example and demands attention: This pathogen turned into a menace to humans during the late 70s, likely as a result of intense antibiotic use in hospital settings, and became one of the microorganisms that are challenging the antibiotic era. Its extreme genome plasticity, combined with mechanisms of horizontal genetic transfer, have played a key role in the evolution of this microorganism, as well as its adaptability to unfavorable environments. However, its pathophysiology, as well as the mechanisms leading to its success as a pathogen, are not that simple to unveil. However, what is clear is that the triad of host-pathogen-environment is crucial in selection and establishment of multidrug-resistant clones and outbreaks. Indeed, there are still many aspects of this pathogen that require a deeper understanding - not only regarding mechanisms of resistance but also its global pathophysiology. For example, basic understanding of transmission mechanisms; knowledge of ‘external’ factors modulating persistence of the pathogen; genetic effects on host susceptibility and infectiousness; mechanisms of pathogenicity and their dynamics; and genetic variation of the pathogen affecting virulence and transmissibility are some aspects that would require further study. Furthermore, the importance of other members of the genus as important nosocomial pathogens, such as Acinetobacter nosocomialis, has been increasingly recognized during the last few years.
