It consists, as you will Observe, Of a straight tube with an Oblique Opening at its distal end, somewhat in the form Of an Old-fashioned rectal speculum of this pattern. The tube is forty two millimeters in circumference and six and one-half inches in length without the Obturator. It has an auxiliary tube carrying a small in candescent lamp Of low heating power which is exposed through a small window near the distal end and on the longer side Of the tube. On the short side of the distal end Of the tube, and almost directly opposite the lamp, a groove is cut in the side Of the tube for the in sertion of the cautery. The groove is intended to give greater rigidity to the knife and greater certainty in the direction and length of the incision. The forms Of the cautery thus far used are here presented and consist in three modifications of the cautery knives, used for nose and throat and other galvano-cautery purposes. The patient further stated that his general health had always been good and that he had never suffered from any liver trouble. He had been for a long time practically incapacitated for busi ness on account of the pain and difficulty attending any attempt at standing or walking. Upon removing his shoes and stockings there was seen a peculiarly grouped eruption of pale yellow nodules, hard and shotty to the feel and deeply set in the skin. The accompany ing chromo-lithograph gives an accurate picture of the appear ance of the eruption, although, on account of the reduction in size, the lesions appear less distinctly defined and more blended the yellowish tint is perhaps a little too accentuated. The erup tion on the sides and back of the heels and in other localities, presently to be referred to, could not be shown in the picture from the point of view from which it was taken. Thur F., a Russian woman, aged 43, thickly set and strongly built, of good general health, and dark complexion, was admitted into the skin ward of the Philadelphia Hospital, December 20, 1890. Owing to the ignorance of the patient, with the fact that she could scarcely speak any English, the history obtainable was far from satisfactory. So far as could be learned from her husband who was closely questioned upon several occasions, the disease dated back four or five months. It began, so he stated, as an itching eruption of an erythematous type extending over the arms and face. At this time or soon afterward an erysipelatous patch appeared over the surface of the metacarpo-phalangeal joint of the thumb of the left hand, becoming gradually, in the course of several days or a few weeks, swollen and elevated, forming a tumor of two to three inches in diameter and one-fourth to one-half inch in altitude. This persisted and grew somewhat more prominent, and finally broke down at the superficial central part, discharging a bloody mucoid fluid. A few weeks later the ulcer had grown to two inches in diameter, with a sharply defined elevated border and a crateriform fungoid central portion. At this time and for a month or two subsequently, erythematous and erysipelatous outbreaks, involving one or several parts, occurred. The patient was finally led through the discomfort and disfigurement produced by the eruption to seek advice. At the time of admission into the hospital the conspicuous and only striking lesion was the fungoidal ulcerating tumor upon the hand. It was fully three to four inches across, rounded, the border considerably elevated, somewhat infiltrated and rather sharply defined against the surrounding healthy skin. It was painful to the touch, but the patient complained of but little spontaneous pain. The surface of the ulcer was mushroom-like, and attended with a variable sanguineous seropurulent discharge. In fact at this rime the growth presented, when taken alone, the appearance of an epitheliomatous tumor, and such it was upon first glance considered. Upon examination of other portions of the general surface, however, it was found that a mild erythematous and eczema-like eruption existed upon the chest anteriorly, and to a slight degree upon the eyelids also. These symptoms together with the history were indicative of mycosis fungoides. No special treatment was ordered for the first several days beyond an antispetic dressing to the ulcer. Book excerpt: Excerpt from The Journal of Cutaneous and Genito-Urinary Disease, Vol. 17: January, 1899 The method of procedure adopted was as follows: an observation of the case clinically; an examination of the hair or the scale microscopically; a planting of the suspected tissues; and in many cases a microscopical examination of the culture. All hairs and scales were examined at first unstained, and later, in some instances, stained specimens were prepared, but I do not believe in the latter method, for by this artificial process only the living spores are colored and the dead ones remain in obscurity. The result is a picture far from the true one - a picture which does not present the plant as it actually exists. Several methods of staining were tried but that recommended by Adamson of London was finally adopted as the most satisfactory. After experimenting with many different artificial media for the cultivation of the plant I adopted the "milieu-d'epreuve" of Sabouraud as the one soil most favorable to the local varieties of ringworm, and although many variations were made in the proportions of the ingredients, nevertheless, in the end I returned to the original formula and used it during the greater part of my work. Success seldom came to me in my cultures, for out of nearly three hundred inoculations less than fifteen percent, produced favorable results. This percentage refers to distinct cases, for when once an individual plant showed itself capable of artificial reproduction it was almost always possible to obtain sub-cultures ad libitum. These sub-cultures almost always retained the most minute peculiarities of the parent stock and to my mind afforded one of the prettiest demonstrations of the plurality of the ringworm fungi. The maltose of several different manufacturers was tried during the three years, and I found that the resulting cultures varied only slightly - the growth of the microsporon Audouini being white when sown on the maltose of Lehn and Fink of New York and buff when planted on that of Grubler of Leipsig. This was the most striking variation noted, and one can see that this was not as important as those found by European investigators in their studies. The temperature at which the cultures were left to grow was found to influence their characteristics to a considerable extent and after many trials 30 centigrade was decided upon as the one best fitted to produce a rapid and luxuriant growth. As to the reaction of the medium it was found that the more acid it was the more contaminations flourished, while with too much alkali present the vigor of the ringworm was threatened. Therefore, a slightly alkaline reaction was found to be the most favorable. During my second-year's work I discovered, to my temporary disadvantage, an excellent means of checking further growth in a culture and also of rendering hairs and scales infected with ringworm sterile.
