M. Despris, in a lecture which he delivered at La Charite (gazette Medica/e, Med. Times) made some interesting observations. A young woman, striking a table with the palm of her hand, thrust a needle into the base of her middle finger and this, striking against the first phalanx, broke and became fixed there. When seen two days afterwards, the fragment of needle had completely disappeared amidst the in amed tissues. On pressing at the base of the mid dle finger, however, a foreign body, pressure on which occasioned pain, could be felt An incision was made at this point, and a fragment of needle, measuring 1% centimetres, removed by the forceps. Here the incision was justified by the fact that the foreign body was firmly fixed. The subject of a second case was a young woman, into whose breast a needle was driven obliquely by a blow, and entirely disappeared under the skin. Guided by the patient the presence of the needle could be ascertained but in this case an incision for its removal would be im proper, for the integuments of the breast are so mo bile that an incision made in the skin would not correspond to the foreign body. The presence of the needle having been exactly determined, we should seize it in its length and make pressure on its two ends. At one of these we may perceive a slight cracking sound, and here the patient also feels a sharper pain than elsewhere, and this is the point of the needle. If we now press firmly upon the other end this point'will be forced through the integument and can then be seized with a forceps. An incision should never be made except when the body is fixed in the tissues, as in the first case. One caution must be borne in mind, and that is, we should never attempt an extraction on the mere statement of the patient that a needle is present in the tissues, and when we are unable, to verify its presence; for sometimes persons declare that they have needles in their tissues when they have not; or when we are consulted the needle may have already migrated to another part of the body, this migration sometimes taking place very rapidly. Book excerpt: Excerpt from The American Journal of the Medical Sciences, 1885, Vol. 90 In the preparation of this paper it has been my intention to present a full report of a case of retention cyst of the pancreas, which has recently come under my observation, and, at the same time, summarize, in a com pact form, the clinical history of similar cases which will serve as a basis for some general remarks on the surgical treatment of cysts of the pan creas. Case I. Cyst of Pancreas; Laparotomy; Recovery - vo12, set. 19, laborer, German, was admitted to Milwaukee Hospital Nov. 28, 1884. He is small for his age and not robust, but he claims that, with one excep tion, he has never been sick, and that no hereditary tendency to disease exists in his family. Five weeks ago, while enjoying perfect health, he was thrown from a wagon, striking the. Ground on thelett side of the abdo men, a heavy keg falling upon his back, and increasing the force of the fall. The pain felt immediately after the accident was confined to his back, at a point where he was struck by the keg, but it was not sufficient in intensity to prevent him from following his occupation as a mason's apprentice. In a few days, however, diarrhoea set in, which persisted for two weeks, greatly reducing his strength and weight. If he had any fever during this time it was not. Sufficiently severe to attract his attention. His appetite was not impaired, and, though he vomited occasionally, neither vomiting nor the diarrhoea seemed to be aggravated by the time of eating or the kind or variety of food. After two weeks he noticed a tumor in the left hypochondriac region, which was round, smooth, and painless. The tumor increased rapidly in size, and soon gave rise to a sensation of fulness in the stomach, and later on, to regurgitation and vomiting soon after meals. Appetite slightly impaired. At this time the patient was treated for a short time by my friend, Dr. F. H. Day, of nvauwatosa, Wis., who resorted to symptomatic treatment, and observing no improve ment, referred him to me for diagnosis, and, in case it should be deemed advisable, for surgical treatment. Take a bandage Of muslin, as broad as the toe is long, and about a foot and a half in length, and place the initial extremity under neath the affected toe. Carry it up on the sound side of the toe, over the toe, down over the ingrowing nail. And beneath the toe which lies against the ingrowing nail; then up on the far side, and back over the top of that toe; over the top of the affected toe again. Down on its sound side.' and under it. Thus fixing the initial extremity of the bandage. Then, lifting the adjacent toe in the loop you have made around it, pull it as far as possible up over the top Of the affected toe. And fasten it there by carrying the bandage beneath both toes and making circular turns around the two, held in that position until the bandage is ex hansted. Having made as firm pressure as possible without causing pain, fasten the band age with a pin on top and between the two toes.
