Book excerpt: Job satisfaction largely determines the productivity and efficiency of human resource for health. It literally depicts the extent to which professionals like or dislike their jobs. Job satisfaction is said to be linked with the employee s work environment, job responsibilities and powers and time pressure; the determinants which affect employee s organizational commitment and consequently the quality of services. The objective of the study was to determine the level of and factors influencing job satisfaction among public health professionals in the public sector. This was a cross sectional study conducted in Islamabad, Pakistan. Sample size was universal including 73 public health professionals, with postgraduate qualifications and working in government departments of Islamabad. A validated structured questionnaire was used to collect data from April to October 2011. Overall satisfaction rate was 41% only, while 45% were somewhat satisfied and 14% of professionals highly dissatisfied with their jobs. For those who were not satisfied, working environment, job description and time pressure were the major causes.

Furthermore, an appropriately structured questionnaire was excerpted from the available literature, as well as expert opinions relating to patient satisfaction with healthcare professionals. The questionnaire data was then interpreted through SPSS-20. Background: Appropriate and improved strategies for quality assurance in the Punjab Public sector hospitals, Pakistan can be evolved with the appropriate understanding of factors influencing quality of medical service. Patient satisfaction is itself a critical controversy for the healthcare professionals/Government. Patient satisfaction is a complicated attitude because a horde of variables have been pinpointed as its interpreters. However, the study is aimed to ascertain the factors affecting the quality Healthcare services providers within Lahore Public Sector Hospitals, Punjab, Pakistan. Objective of the Study: The study was conducted chastely in the public interest and in order to facilitate the Govt. of Punjab, Pakistan to deal with its maximum potential to bring about ultimate satisfaction level of the patients with a foremost objective of providing fairly, attainable and effective healthcare services. The study was conducted - To analyze factors impacting patient satisfaction in Public sector hospitals - To rank the most important factors affecting patient satisfaction - To evaluate the necessary steps taken by the hospital administrators for patient satisfaction. Conclusion: The study ascertained the 30 most important factors resulting in poor level of patient satisfaction. Results identified that majority of the factors out of 30 factors lie in Medium and high Severity zone (with a rating between 3.4 to 4.2 out of total 5). These factors also require foremost attention by the Government. After pointing out the 30factors and their severity level, the study revealed the top 12 most important factors on the basis of Impact Factor and Relative Importance Index.

Worker shortage, skill mix imbalance, maldistribution, negative work environments, and weak knowledge bases challenge nearly all countries. Especially in the poorest countries, the workforce is under assault by a triple threat of HIV/AIDS, out-migration, and inadequate investment. Effective country strategies should be launched and backed by international reinforcement. These include urgently mobilizing one million more health workers for Africa, and focusing efforts on the unremunerated community-level health workers, the majority of whom are women. Ultimately, the crisis in human resources is a shared problem requiring shared responsibility for cooperative action. Alliances for action are recommended to strengthen the performance of all actors while expanding space and energy for new ones.

This report examines how Ethiopia s Health Extension Program (HEP) has contributed to the country s move toward Univeral Health Coverage (UHC), and to shed light on how other countries may learn from Ethiopia s experiences of HEP when designing their own path to UHC. HEP is one of the government s UHC strategies introduced in a context of limited resources and low coverage of essential health services. The key aspects of the program include the capacity building and mobilization of more than 30, 000 Health Extension Workers (HEWs) targeting more than 12 million model families, and the mobilization of health development army ? to support the community-based health system. Using the HEP-UHC conceptual model and data from Demographic and Health Surveys, the study examines how the HEP has contributed to the country s move toward UHC. During the period that the HEP has been implemented, the country has experienced significant improvements in many dimensions: in terms of socioeconomic, psychological, behavioral, and biological dimensions of the beneficiaries; and in terms of the coverage of health care services. The study finds an accelerated rate of improvements among the rural, less-educated, and the poor population, which is leading to an overall reduction in equity gaps and improvements in the equity indicators including the concentration indices - that suggest a more equitable distribution of resources and health outcomes. The HEP in Ethiopia has demonstrated that an institutionalized community approach is effective in helping a country make progress toward UHC. The elements of success in the HEP include the emphasis on community mobilization which identifies community priorities, engages and empowers community members, and supports their ability to solve local problems. The other aspect of HEP is the emphasis on institutionalization of the activities, which addresses the sustainability of community programs through high level of political commitment, and effective coordination of national policies and leveraging of support from partners. These findings may offer useful lessons for other low income countries facing similar challenges in developing and implementing a sustainable UHC strategy.

The short-form instrument, the PSQ-18, contains 18 items tapping each of the seven dimensions of satisfaction with medical care measured by the PSQ-III: general satisfaction, technical quality, interpersonal manner, communication, financial aspects, time spent with doctor, and accessibility and convenience. PSQ-18 subscale scores are substantially correlated with their full-scale counterparts and possess generally adequate internal consistency reliability. Moreover, both the magnitude of the correlation coefficients and the overall pattern of correlations among PSQ-18 subscales are highly similar to those observed for the PSQ-III. These preliminary analyses support the use of the PSQ-18 in situations where the need for brevity precludes administration of the full-length PSQ-III.
