Book excerpt: The ability of eukaryotic cells to change their shape and to migrate directionally is highly dependent on active volume regulation in cells building up tissues as well as in individual cells. Transmembrane fluxes of water via specialized water channels, called aquaporins (AQPs), facilitate the changes of volume and shape, which additionally require a complex interplay between the plasma membrane and the cytoskeleton. AQPs have been shown to be involved in the development of inflammatory processes and diseases. The aims of the studies underlying this thesis were to further elucidate the expression and function of AQPs in both bacterial and viral infections as well as in the inflammatory disease, microscopic colitis. For this, molecular techniques qPCR, immunoblotting and live, holographic, confocal and super-resolution imaging were used. When cells of the innate immune system encounter pathogens they need to respond and prepare for migration and phagocytosis and do so through volume regulatory processes. The Gramnegative bacterium Pseudomonas aeruginosa utilizes a small molecule-based communication system, called quorum sensing (QS) to control the production of its virulence factors and biofilms. We found that P. aeruginosa with a complete QS system elicits a stronger phagocytic response in human blood-derived macrophages compared to its lasI-/rhlI- mutant lacking the production of the QS molecules N-butyryl-L-homoserine lactone (C4-HSL) and N-3-oxododecanoyl-L-homoserine lactone (3O-C12-HSL). Infection with P. aeruginosa further increases the expression of AQP9 and induces re-localisation of AQP9 to the front and trailing ends of macrophages. Moreover, the 3O-C12-HSL alone elevates the expression of AQP9, redistribute the water channel to the front and rear ends and increases the cell area and volume of macrophages. Both infection with the wild type P. aeruginosa and the treatment with 3OC12-HSL change the nano-structural architecture of the AQP9 distribution in macrophages. Viruses use the intracellular machinery of the invaded cells to produce and assemble new viral bodies. Intracellular AQPs are localised in a membranes of cellular organelles to regulate their function and morphology. C3H10T1/2 fibroblasts transiently expressing green fluorescent protein (GFP)-AQP6 show a reduced expression of AQP6 after Hazara virus infection and an increased cell area. Overexpressing AQP6 in C3H10T1/2 cells reduces the infectivity of Hazara virus indicating that AQP6 expression has a protective role in virus infections. Ion and water channels in the epithelial cell lining tightly regulate the water homeostasis. In microscopic colitis (MC), patients suffer from severe watery diarrhoeas. For the first time, we have shown that the expression of AQP1, 8 and 11 and the sodium/hydrogen exchanger NHE1 are reduced in colonic biopsies from MC patients compared to healthy control individuals. Following treatment with the glucocorticoid budesonide the patients experienced a rapid recovery and we observed a restored or increased expression of the AQPs and NHE1 during treatment, suggesting a role for AQPs in the diarrhoeal mechanisms in MC. Taken together, this thesis provides new evidence on the importance of water homeostasis regulation through AQPs during infections and inflammation and opens up a door for further investigations of roles for AQPs in inflammatory processes.

Healthcare is frequently being called upon to be more cost-efficient and still fulfil demands regarding waiting times, quality and availability. Experience from structural changes in other contexts gives reason to be positive about the potential for logistics improvements in the healthcare sector as well. From a logistics perspective patients pass different care functions, units, organisations and health facilities. It is assumed that logistics management knowledge applied in healthcare can lead to lower costs, shorter waiting times, better patient service, shorter treatment times and increased capacity. This dissertation therefore presents an exploration of how logistics management theories can be operationalised in a healthcare context to understand care chain effectiveness. Theoretically, the operationalisation is done by systems theory creating compatibility between logistics management theories and the healthcare context. As a first step, features for a logistics system forms features for achieving care chain effectiveness. High care chain effectiveness is thus a desired condition and the care delivery system is the tool to achieve it. As the final step in the operationalisation the features for care chain effectiveness are in turn used to analyse today’s practices. Empirically, the research is based on qualitative data from a single case study with multiple units of analysis. It includes four care units at one of Sweden’s university hospitals, where the data is gathered through interviews, insight into management systems and document analysis. One of the main results is the 21 areas identified for analysing today’s practices by means of features for care chain effectiveness. Another main result is the four important concepts revealed through the operationalisation: Lead time - the episode of care from order to delivery as the amount of time for patient cases between first contact with healthcare and the last.Patient order fulfilment - fulfilment of patients’ needs, broken down into several smaller steps performed over time within different care units in one or several organisations, consisting of five sub-processes - order handling, diagnosis, treatment, follow-up, and discharge.True demand – patients’ needs that is to be met and thus sets what care to deliver, i.e. the production plan and the subordinate resource plan.System boundaries - defines which care units to include when focusing on the care delivery system’s performance as a whole and should be more important than the performance and productivity of each individual care unit. A number of direct suggestions for care chain improvement can also be found in the concluding remarks, for example that objectives linked to economic influx or penalty narrow the system and that lead time data on an aggregated level is needed to cover episodes of care. The theoretical contribution of the dissertation is to the field of logistics management through the methodological development of using these theories in a new context. The managerial contribution is to healthcare managers through providing opportunities to improve care chains primarily by means of a greater understanding of care delivery systems.

Book excerpt: Lyme borreliosis (LB) is caused by spirochetes within the Borrelia burgdorferi sensu lato complex and is the most common tick-transmitted disease in the northern hemisphere. The transmission of the spirochetes to humans in Europe is done by the Ixodes ricinus ticks, which can also transmit the relapsing fever species Borrelia miyamotoi. LB may cause clinical manifestations in the skin, in the central nervous system, in joints, and in the heart. Diagnosis of LB is mainly based on the patient´s medical history, self-described symptoms, and clinical signs in combination with the detection of Borrelia-specific antibodies (serological methods). In some cases/issues, detection of Borrelia-specific deoxyribonucleic acid (molecular methods) may be used as a complement to serology. All diagnosed LB infections are treated with antibiotics to prevent disease progression, and most patients fully recover without further sequelae. The overall aims of this thesis were to evaluate molecular and serological tools for laboratory diagnosis of LB, with a special focus on Lyme neuroborreliosis (LNB), and to identify potential improvements. The results presented in this thesis showed that the immunoglobulin (Ig) G assays, currently in use in northern Europe for detection of antibodies in serum, had high diagnostic sensitivity (88 %) together with comparable results both between and within assays. For the IgM assays, the diagnostic sensitivity was lower (59 %) with more heterogeneous results. Small variations in diagnostic performance for IgM and IgG were mainly presented for samples within the borderline zone. These results support the theory that separate testing of IgM antibodies in serum has low diagnostic value. However, simultaneous detection in serum and cerebrospinal fluid (CSF) for both IgM and IgG antibodies was essential for the diagnosis of LNB, at least for certain assays. So far (to our knowledge), no systematic evaluation and optimisation of the pre-analytical handling of CSF samples before molecular testing has been performed. By use of the precipitate concentrated by moderate centrifugation, extraction of total nucleic acid followed by reversetranscription to complementary deoxyribonucleic acid, in combination with the absence of polymerase chain reaction (PCR) inhibitors, detection of Borrelia garinii, Borrelia afzelii, Borrelia burgdorferi sensu stricto, and B. miyamotoi was possible. These four species are all known to be pathogenic to humans. The results revealed a high analytical sensitivity and specificity for the optimised pre-analytical conditions. The thesis also presents results showing that the real-time PCR protocols currently used in Scandinavia have high analytical sensitivity, specificity, and concordance. This indicates that the low diagnostic sensitivity for detection of Borrelia in CSF was not a result of poorly designed and evaluated PCR protocols, but was possibly due to the low number of spirochetes in the samples. However, to further evaluate the diagnostic performance for detection of Borrelia in CSF by PCR, clinical samples need to be evaluated based on our new recommendations for the pre-analytical handling of CSF samples. In conclusion, this thesis presents results revealing that both molecular and serological tools for detection of Borrelia have, in general high sensitivity and specificity with results comparable between different protocols and different laboratories. It also presents recommendations for pre-analytical handling of CSF samples before PCR-analysis, and shows the benefits in diagnostic performance by simultaneous detection of IgM and IgG antibodies in serum and CSF for accurate diagnosis of LNB. Even though the techniques mentioned above have high analytical performance, the ability to discriminate an active infection from a previous one is limited and further studies need to be carried out. These studies need to focus on finding diagnostic tools that can help physicians to determine ongoing infection to ensure adequate treatment. It is also desirable to improve the standardisation of the diagnostic tools and to find methods that can discriminate between different Borrelia species. Borrelios är den vanligaste fästingöverförda sjukdomen på norra halvklotet och orsakas av bakterier inom Borrelia burgdorferi sensu lato gruppen. Överföringen av bakterier till människa i Europa sker via Ixodes ricinus fästingar, vilka även överför bakterien Borrelia miyamotoi som ger återfallsfeber. Borreliainfektioner uppvisar kliniska uttryck i huden, i det centrala nervsystemet och i leder. En borrelia-diagnos baseras främst på patientens medicinska historia i kombination med kliniska tecken, egenbeskrivna symptom samt påvisning av Borrelia-specifika antikroppar (serologiska metoder). Vid vissa frågeställningar kan påvisning av Borrelia-bakteriens arvsmassa (molekylärbiologiska metoder) användas som komplement till antikroppstester. Alla diagnostiserade borreliainfektioner behandlas med antibiotika för att förhindra utveckling av sjukdomen och merparten av patienterna blir fullt återställda. Det övergripande syftet med avhandlingen var att utvärdera metoder för påvisning av Borrelia-specifika antikroppar samt Borrelia-specifik arvsmassa, men fokus på neuroborrelios, samt identifiera potentiella förbättringar. De metoder som används för påvisning av immunoglobulin (IgG)-antikroppar (uppträder sent i en infektion) i serum i norra Europa uppvisar hög känslighet (88 %) med jämförbara resultat både mellan och inom en analysmetod. Vid påvisning av IgM-antikroppar (uppträder tidigt i en infektion) i serum uppvisas lägre känslighet (59 %) och mer olikartade resultat. Små variationer i den diagnostiska förmågan att påvisa IgM och IgG-antikroppar beror till stor del på att flera prover erhållit gränsvärden d v s ett värde som inte kan anses som positivt men inte heller som negativt. Resultaten från denna studie indikerar att påvisning av IgM-antikroppar i serum har lågt värde vid diagnostik av Borrelia. Dock bör parallell analys av både IgM och IgG-antikroppar i serum och ryggmärgsvätska utföras vid påvisning av neuroborrelios. I dagsläget (till vår kännedom) har ingen systematisk utvärdering och optimering av det preanalytiska tillvägagångssättet vid påvisning av Borrelia-specifik arvsmassa i ryggmärgsvätska genomförts. Genom att använda pelleten (bottensatsen som erhålls genom måttlig centrifugering), framrening av total nukleinsyra i kombination med frånvaro av material som kan påverka PCR-reaktionen på ett negativt sätt (inhibitorer), kan påvisning av Borreliaarterna Borrelia garinii, Borrelia afzelii, Borrelia burgdorferi sensu stricto och B. miyamotoi ske. Dessa Borrelia-arter är alla patogena för människa. De realtids-PCR protokoll som i dagsläget används i Skandinavien har hög analytisk känslighet, tillförlitlighet och överensstämmelse. Detta tyder på att den låga känslighet som uppvisas vid påvisning av Borrelia-specifik arvsmassa i ryggmärgsvätska inte beror på dåligt utvärderade och designade PCR-protokoll, utan är troligtvis orsakad av låg bakteriemängd i proverna. För vidare utvärdering av den diagnostiska förmågan att påvisa Borrelia-specifik arvsmassa i ryggmärgsvätska med PCR, bör kliniska prover samlas in och analyseras utifrån de nya rekommendationerna för pre-analytiskt tillvägagångssätt vid analys av ryggmärgsprover. Sammanfattningsvis visar resultaten i denna avhandling på generellt hög känslighet och tillförlitlighet samt överensstämmelse mellan olika protokoll/test vid påvisningar av Borreliaspecifika antikroppar och Borrelia-specifik arvsmassa. I avhandlingen presenteras även rekommendationer för pre-analytiskt tillvägagångssätt vid omhändertagande och transport av ryggmärgsvätska till laboratoriet. Resultaten visar även på nyttan i att analysera ryggmärgsvätska och serum parallellt för både IgM och IgG-antikroppar för att erhålla rätt diagnos vid frågeställningen neuroborrelios. Ovan nämnda metoder har trots god prestanda svårt att i alla lägen särskilja en aktiv infektion från en tidigare genomgången, varpå vidare studier krävs. Framtida studier bör fokusera på att finna diagnostiska verktyg som hjälper läkarna att urskilja en pågående infektion så att patienten erhåller passande behandling. Det är också mycket viktigt att arbeta vidare mot en standardisering av de diagnostiska metoderna samt finna metoder som har möjlighet att särskilja mellan olika Borrelia-arter.

Book excerpt: Voltage-gated ion channels play fundamental roles in excitable cells, such as neurons, where they enable electric signaling. Normally, this signaling is well controlled, but brain damage, alterations in the ionic composition of the extracellular solution, or dysfunctional ion channels can increase the electrical excitability thereby causing epilepsy. Voltage-gated ion channels are obvious targets for antiepileptic drugs, and, as a rule of thumb, excitability is dampened either by closing voltagegated sodium channels (Nav channels) or by opening voltage-gated potassium channels (Kv channels). For example, several classical antiepileptic drugs block the ion-conducting pore of Nav channels. Despite the large number of existing antiepileptic drugs, one third of the patients with epilepsy suffer from intractable or pharmacoresistant seizures. Our research group has earlier described how different polyunsaturated fatty acids (PUFAs) open a Kv channel by binding close to the voltage sensor and, from this position, electrostatically facilitate the movement of the voltage-sensor, thereby opening the channel. However, PUFAs affect a wide range of ion channels, making it difficult to use them as pharmaceutical drugs; it would be desirable to find smallmolecule compounds with an electrostatic, PUFA-like mechanism of action. The aim of the research leading to this thesis was to find, characterize, and refine drug candidates capable of electrostatically opening a Kv channel. The majority of the experiments were performed on the cloned Shaker Kv channel, expressed in oocytes from the frog Xenopus laevis, and the channel activity was explored with the two-electrode voltage-clamp technique. By systematically mutating the extracellular end of the channel’s voltage sensor, we constructed a highly PUFAsensitive channel, called the 3R channel. Such a channel is a useful tool in the search for electrostatic Kv-channel openers. We found that resin acids, naturally occurring in tree resins, act as electrostatic Shaker Kv channel openers. To explore the structure-activity relationship in detail, we synthesized 120 derivatives, whereof several were potent Shaker Kv channel openers. We mapped a common resin acidbinding site to a pocket formed by the voltage sensor, the channel’s third transmembrane segment, and the lipid membrane, a principally new binding site for small-molecule compounds. Further experiments showed that there are specific interactions between the compounds and the channel, suggesting promises for further drug development. Several of the most potent Shaker Kv channel openers also dampened the excitability in dorsal-root-ganglion neurons from mice, elucidating the pharmacological potency of these compounds. In conclusion, we have found that resin-acid derivatives are robust Kv-channel openers and potential drug candidates against diseases caused by hyperexcitability, such as epilepsy.

Book excerpt: Biotemplating is the art of using a biological structure as a scaffold which is decorated with a functional material. In this fashion the structures will gain new functionalities and biotemplating offers a simple route of mass-producing mesoscopic material with new interesting properties. Biological structures are abundant and come in a great variety of elaborate and due to their natural origin they could be more suitable for interaction with biological systems than wholly synthetic materials. Conducting polymers are a novel class of material which was developed just 40 years ago and are well suited for interaction with biological material due to their organic composition. Furthermore the electronic properties of the conducting polymers can be tuned giving rise to dynamic control of the behavior of the material. Self-assembly processes are interesting since they do not require complicated or energy demanding processing conditions. This is particularly important as most biological materials are unstable at elevated temperatures or harsh environments. The main aim of this thesis is to show the possibility of using self-assembly to decorate a conducting polymer onto various biotemplates. Due to the intrinsic variety in charge, size and structure between the available natural scaffolds it is difficult, if not impossible, to find a universal method. In this thesis we show how biotemplating can be used to create new hybrid materials by self-assembling a conducting polymer with biological structures based on DNA, protein, lipids and cellulose, and in this fashion create material with novel optical and electronic properties.

Book excerpt: Proteins are essential for all known forms of life and in many lethal diseases protein failure is the cause of the disease. To understand proteins and the processes they are involved in, it is valuable to know their structures as well as their dynamics and interactions. The structures may not be directly inspected because proteins are too small to be visible in a light microscope, which is why indirect methods such as nuclear magnetic resonance (NMR) spectroscopy have to be utilized. This method provides atomic information about the protein and, in contrast to other methods with similar resolution, the measurements are performed in solution resulting in more physiological conditions, enabling analysis of dynamics. Important dynamical processes are the ones on the millisecond timeframe, which may contribute to interactions of proteins and their catalysis of chemical reactions, both of significant value for the function of the proteins. To better understand proteins, not only do we need to study them, but also develop the methods we are using. This thesis presents four papers about improved NMR techniques as well as a fifth where the kinase domain of ephrinB receptor 2 (EphB2) has been studied regarding the importance of millisecond dynamics and interactions for the activation process. The first paper presents the software COMPASS, which combines statistics and the calculation power of a computer with the flexibility and experience of the user to facilitate and speed up the process of assigning NMR signals to the atoms in the protein. The computer program PINT has been developed for easier and faster evaluation of NMR experiments, such as those that evaluate protein dynamics. It is especially helpful for NMR signals that are difficult to distinguish, so called overlapped peaks, and the soft- ware also converts the detected signals to the indirectly measured physical quantities, such as relaxation rate constants, principal for dynamics. Next are two new versions of the Carr-Purcell-Maiboom-Gill (CPMG) dispersion pulse sequences, designed to measure millisecond dynamics in a way so that the signals are more separated than in standard experiments, to reduce problems with overlaps. To speed up the collection time of the data set, a subset is collected and the entire data set is then reconstructed, by multi-dimensional decomposition co-processing. Described in the thesis is also a way to produce suitably labeled proteins, to detect millisecond dynamics at C? positions in proteins, using the CPMG dispersion relaxation experiment at lower protein concentrations. Lastly, the kinase domain of EphB2 is shown to be more dynamic on the millisecond time scale as well as more prone to interact with itself in the active form than in the inactive one. This is important for the receptor function of the protein, when and how it mediates signals. To conclude, this work has extended the possibilities to study protein dynamics by NMR spectroscopy and contributed to increased understanding of the activation process of EphB2 and its signaling mechanism.
