Book excerpt: Lyme borreliosis (LB) is caused by spirochetes within the Borrelia burgdorferi sensu lato complex and is the most common tick-transmitted disease in the northern hemisphere. The transmission of the spirochetes to humans in Europe is done by the Ixodes ricinus ticks, which can also transmit the relapsing fever species Borrelia miyamotoi. LB may cause clinical manifestations in the skin, in the central nervous system, in joints, and in the heart. Diagnosis of LB is mainly based on the patient´s medical history, self-described symptoms, and clinical signs in combination with the detection of Borrelia-specific antibodies (serological methods). In some cases/issues, detection of Borrelia-specific deoxyribonucleic acid (molecular methods) may be used as a complement to serology. All diagnosed LB infections are treated with antibiotics to prevent disease progression, and most patients fully recover without further sequelae. The overall aims of this thesis were to evaluate molecular and serological tools for laboratory diagnosis of LB, with a special focus on Lyme neuroborreliosis (LNB), and to identify potential improvements. The results presented in this thesis showed that the immunoglobulin (Ig) G assays, currently in use in northern Europe for detection of antibodies in serum, had high diagnostic sensitivity (88 %) together with comparable results both between and within assays. For the IgM assays, the diagnostic sensitivity was lower (59 %) with more heterogeneous results. Small variations in diagnostic performance for IgM and IgG were mainly presented for samples within the borderline zone. These results support the theory that separate testing of IgM antibodies in serum has low diagnostic value. However, simultaneous detection in serum and cerebrospinal fluid (CSF) for both IgM and IgG antibodies was essential for the diagnosis of LNB, at least for certain assays. So far (to our knowledge), no systematic evaluation and optimisation of the pre-analytical handling of CSF samples before molecular testing has been performed. By use of the precipitate concentrated by moderate centrifugation, extraction of total nucleic acid followed by reversetranscription to complementary deoxyribonucleic acid, in combination with the absence of polymerase chain reaction (PCR) inhibitors, detection of Borrelia garinii, Borrelia afzelii, Borrelia burgdorferi sensu stricto, and B. miyamotoi was possible. These four species are all known to be pathogenic to humans. The results revealed a high analytical sensitivity and specificity for the optimised pre-analytical conditions. The thesis also presents results showing that the real-time PCR protocols currently used in Scandinavia have high analytical sensitivity, specificity, and concordance. This indicates that the low diagnostic sensitivity for detection of Borrelia in CSF was not a result of poorly designed and evaluated PCR protocols, but was possibly due to the low number of spirochetes in the samples. However, to further evaluate the diagnostic performance for detection of Borrelia in CSF by PCR, clinical samples need to be evaluated based on our new recommendations for the pre-analytical handling of CSF samples. In conclusion, this thesis presents results revealing that both molecular and serological tools for detection of Borrelia have, in general high sensitivity and specificity with results comparable between different protocols and different laboratories. It also presents recommendations for pre-analytical handling of CSF samples before PCR-analysis, and shows the benefits in diagnostic performance by simultaneous detection of IgM and IgG antibodies in serum and CSF for accurate diagnosis of LNB. Even though the techniques mentioned above have high analytical performance, the ability to discriminate an active infection from a previous one is limited and further studies need to be carried out. These studies need to focus on finding diagnostic tools that can help physicians to determine ongoing infection to ensure adequate treatment. It is also desirable to improve the standardisation of the diagnostic tools and to find methods that can discriminate between different Borrelia species. Borrelios är den vanligaste fästingöverförda sjukdomen på norra halvklotet och orsakas av bakterier inom Borrelia burgdorferi sensu lato gruppen. Överföringen av bakterier till människa i Europa sker via Ixodes ricinus fästingar, vilka även överför bakterien Borrelia miyamotoi som ger återfallsfeber. Borreliainfektioner uppvisar kliniska uttryck i huden, i det centrala nervsystemet och i leder. En borrelia-diagnos baseras främst på patientens medicinska historia i kombination med kliniska tecken, egenbeskrivna symptom samt påvisning av Borrelia-specifika antikroppar (serologiska metoder). Vid vissa frågeställningar kan påvisning av Borrelia-bakteriens arvsmassa (molekylärbiologiska metoder) användas som komplement till antikroppstester. Alla diagnostiserade borreliainfektioner behandlas med antibiotika för att förhindra utveckling av sjukdomen och merparten av patienterna blir fullt återställda. Det övergripande syftet med avhandlingen var att utvärdera metoder för påvisning av Borrelia-specifika antikroppar samt Borrelia-specifik arvsmassa, men fokus på neuroborrelios, samt identifiera potentiella förbättringar. De metoder som används för påvisning av immunoglobulin (IgG)-antikroppar (uppträder sent i en infektion) i serum i norra Europa uppvisar hög känslighet (88 %) med jämförbara resultat både mellan och inom en analysmetod. Vid påvisning av IgM-antikroppar (uppträder tidigt i en infektion) i serum uppvisas lägre känslighet (59 %) och mer olikartade resultat. Små variationer i den diagnostiska förmågan att påvisa IgM och IgG-antikroppar beror till stor del på att flera prover erhållit gränsvärden d v s ett värde som inte kan anses som positivt men inte heller som negativt. Resultaten från denna studie indikerar att påvisning av IgM-antikroppar i serum har lågt värde vid diagnostik av Borrelia. Dock bör parallell analys av både IgM och IgG-antikroppar i serum och ryggmärgsvätska utföras vid påvisning av neuroborrelios. I dagsläget (till vår kännedom) har ingen systematisk utvärdering och optimering av det preanalytiska tillvägagångssättet vid påvisning av Borrelia-specifik arvsmassa i ryggmärgsvätska genomförts. Genom att använda pelleten (bottensatsen som erhålls genom måttlig centrifugering), framrening av total nukleinsyra i kombination med frånvaro av material som kan påverka PCR-reaktionen på ett negativt sätt (inhibitorer), kan påvisning av Borreliaarterna Borrelia garinii, Borrelia afzelii, Borrelia burgdorferi sensu stricto och B. miyamotoi ske. Dessa Borrelia-arter är alla patogena för människa. De realtids-PCR protokoll som i dagsläget används i Skandinavien har hög analytisk känslighet, tillförlitlighet och överensstämmelse. Detta tyder på att den låga känslighet som uppvisas vid påvisning av Borrelia-specifik arvsmassa i ryggmärgsvätska inte beror på dåligt utvärderade och designade PCR-protokoll, utan är troligtvis orsakad av låg bakteriemängd i proverna. För vidare utvärdering av den diagnostiska förmågan att påvisa Borrelia-specifik arvsmassa i ryggmärgsvätska med PCR, bör kliniska prover samlas in och analyseras utifrån de nya rekommendationerna för pre-analytiskt tillvägagångssätt vid analys av ryggmärgsprover. Sammanfattningsvis visar resultaten i denna avhandling på generellt hög känslighet och tillförlitlighet samt överensstämmelse mellan olika protokoll/test vid påvisningar av Borreliaspecifika antikroppar och Borrelia-specifik arvsmassa. I avhandlingen presenteras även rekommendationer för pre-analytiskt tillvägagångssätt vid omhändertagande och transport av ryggmärgsvätska till laboratoriet. Resultaten visar även på nyttan i att analysera ryggmärgsvätska och serum parallellt för både IgM och IgG-antikroppar för att erhålla rätt diagnos vid frågeställningen neuroborrelios. Ovan nämnda metoder har trots god prestanda svårt att i alla lägen särskilja en aktiv infektion från en tidigare genomgången, varpå vidare studier krävs. Framtida studier bör fokusera på att finna diagnostiska verktyg som hjälper läkarna att urskilja en pågående infektion så att patienten erhåller passande behandling. Det är också mycket viktigt att arbeta vidare mot en standardisering av de diagnostiska metoderna samt finna metoder som har möjlighet att särskilja mellan olika Borrelia-arter.

Book excerpt: The ability of eukaryotic cells to change their shape and to migrate directionally is highly dependent on active volume regulation in cells building up tissues as well as in individual cells. Transmembrane fluxes of water via specialized water channels, called aquaporins (AQPs), facilitate the changes of volume and shape, which additionally require a complex interplay between the plasma membrane and the cytoskeleton. AQPs have been shown to be involved in the development of inflammatory processes and diseases. The aims of the studies underlying this thesis were to further elucidate the expression and function of AQPs in both bacterial and viral infections as well as in the inflammatory disease, microscopic colitis. For this, molecular techniques qPCR, immunoblotting and live, holographic, confocal and super-resolution imaging were used. When cells of the innate immune system encounter pathogens they need to respond and prepare for migration and phagocytosis and do so through volume regulatory processes. The Gramnegative bacterium Pseudomonas aeruginosa utilizes a small molecule-based communication system, called quorum sensing (QS) to control the production of its virulence factors and biofilms. We found that P. aeruginosa with a complete QS system elicits a stronger phagocytic response in human blood-derived macrophages compared to its lasI-/rhlI- mutant lacking the production of the QS molecules N-butyryl-L-homoserine lactone (C4-HSL) and N-3-oxododecanoyl-L-homoserine lactone (3O-C12-HSL). Infection with P. aeruginosa further increases the expression of AQP9 and induces re-localisation of AQP9 to the front and trailing ends of macrophages. Moreover, the 3O-C12-HSL alone elevates the expression of AQP9, redistribute the water channel to the front and rear ends and increases the cell area and volume of macrophages. Both infection with the wild type P. aeruginosa and the treatment with 3OC12-HSL change the nano-structural architecture of the AQP9 distribution in macrophages. Viruses use the intracellular machinery of the invaded cells to produce and assemble new viral bodies. Intracellular AQPs are localised in a membranes of cellular organelles to regulate their function and morphology. C3H10T1/2 fibroblasts transiently expressing green fluorescent protein (GFP)-AQP6 show a reduced expression of AQP6 after Hazara virus infection and an increased cell area. Overexpressing AQP6 in C3H10T1/2 cells reduces the infectivity of Hazara virus indicating that AQP6 expression has a protective role in virus infections. Ion and water channels in the epithelial cell lining tightly regulate the water homeostasis. In microscopic colitis (MC), patients suffer from severe watery diarrhoeas. For the first time, we have shown that the expression of AQP1, 8 and 11 and the sodium/hydrogen exchanger NHE1 are reduced in colonic biopsies from MC patients compared to healthy control individuals. Following treatment with the glucocorticoid budesonide the patients experienced a rapid recovery and we observed a restored or increased expression of the AQPs and NHE1 during treatment, suggesting a role for AQPs in the diarrhoeal mechanisms in MC. Taken together, this thesis provides new evidence on the importance of water homeostasis regulation through AQPs during infections and inflammation and opens up a door for further investigations of roles for AQPs in inflammatory processes.

Book excerpt: Background: Internet use is an integral part of everyday life in contemporary society, especially among young people. It is used to perform activities in everyday life by an increasing proportion of the population. However, knowledge about access to and use of the internet by adolescents and young adults with intellectual disabilities (ID) is scarce. More knowledge is needed about digital competencies and digital participation in their everyday lives. Aim: The overall aim of this thesis was to explore and describe internet access and use, and digital participation in everyday life among adolescents and young adults with intellectual disabilities. Designs and Methods: The thesis is based on results from three studies. In study I, the focus was on access to and use of the internet in the everyday settings of school/work, at home or during free time. Data was collected through observations, conversations, and follow-up interviews with 15 participants with ID, aged 13–24 years. The data was analysed using qualitative content analysis. In studies II and III, the design was cross-sectional and comparative, using national surveys on media and internet use from the Swedish Media Council, from which comparative data from reference groups could be gained. In study II, the national survey of adolescents on internet access and use was cognitively adapted for adolescents with intellectual disabilities, aged 13–20 years, in several steps. This made it accessible to a total selection of pupils from all the special schools in four diverse municipalities in two different regions of Sweden. In study III, the national survey of parents about opportunities and risks of internet use by their adolescents was used. The surveys were sent to a sample of n=318 adolescents with ID and their caregivers/parents. The responses were higher for the adolescents (n=114) than for the parents (n=99), and the response rate of the adolescents with ID was equivalent as that of the reference group, at 36% and 38% respectively. In study II, chi-square tests were used and, when necessary, Fisher’s exact test to analyse the data. In study III, analyses were carried out using Fisher’s exact test and logistic regression to control for confounding factors. Results: This thesis show that access to internet-enabled devices is lower for adolescents with ID than for the general population, except for tablets (study II). All internet activities, except playing games, are performed by fewer adolescents with ID compared to the reference group (study II) and the time spent on the internet activities is less (study III). Both environmental challenges and personal abilities present difficulties in internet access and use (study I) and affect digital participation for adolescents and young adults with ID. Furthermore, a significantly higher proportion of parents of adolescents with ID perceive opportunities associated with internet use and playing games, and a lower proportion perceive risks with negative consequences, or have concerns about online risks, compared with the reference group (study III). Significantly more parents of adolescents with ID state that their adolescent never uses smartphones or social media compared with the reference group. Strategies used to handle the digital environment and take part in internet activities were found and described, such as getting support from others, reducing the number of internet-enabled devices used and personalising them. Gaining access to internet content and performing internet activities was facilitated by picture-, word- and voice-based strategies, which were used by adolescents and young adults with both mild and moderate ID (study I). Conclusions: The conclusions are that the results show a lag in internet access and use and in digital participation by adolescents and young adults with ID. Adolescents and young adults with ID were accessing and using the internet in similar ways to the reference group, but to a lesser extent. The impact of the participants’ environment, together with their lack of certain abilities, make the development of digital competencies difficult for them. The result that parents of adolescents with ID perceive more opportunities and fewer risks associated with the internet provides new knowledge to support positive risk-taking in internet use and enable digital participation by adolescents and young adults with ID. Support can be developed in collaboration between the adolescent/young adult, their parents and teachers, and staff in community-based services and should involve physical, social and digital environmental adaptations. These can enable the development of digital competencies and minimise the lag in digital participation in everyday life, which is needed for participation in today’s digitalised society. Bakgrund till avhandlingen: Internetanvändning är en integrerad del av vardagen i dagens samhälle, särskilt bland ungdomar. Internet används för att utföra en mängd aktiviteter i vardagen av alltfler personer. Dock är kunskapen om tillgång till, samt användning av internet i vardagliga aktiviteter för ungdomar med intellektuell funktionsnedsättning (IF) bristfällig. Ökad kunskap behövs om digitala kompetenser och digital delaktighet i vardagen för ungdomar och unga vuxna med IF. Syftet med avhandlingen: Det övergripande syftet med denna avhandling var att utforska och beskriva tillgång till och användning av internet och digital delaktighet i vardagen bland ungdomar och unga vuxna med IF. Hur studierna genomfördes: Avhandlingen bygger på resultat från tre delstudier. I studie I samlades data in via observationer av och uppföljande intervjuer med 15 deltagare med IF, i åldern 13–24 år. Fokus låg på tillgång till och användning av internet i deras vardagliga miljöer: skola/arbete, hemma eller på fritiden. Data analyserades med kvalitativ innehållsanalys. I studie II och III var designen jämförande tvärsnittsstudier. Nationella enkäter från Statens Medieråd om medie- och internetanvändning användes där jämförande data från referensgrupper kunde erhållas. I studie II gjordes kognitiva anpassningar av den nationella enkäten om medie- och internetanvändning i flera steg för målgruppen ungdomar med IF i åldern 13–20 år. Anpassningen till en lättläst version av enkäten, med bildstöd för de som behövde det, gjorde den möjlig att skicka till ett totalurval av elever från alla särskolor i fyra olika kommuner i två olika regioner i Sverige. I studie III användes den nationella enkäten till föräldrar om möjligheter och risker med internet- och medieanvändning för deras ungdomar. Enkäterna skickades till ett urval av n = 318 ungdomar med IF och deras vårdgivare/förälder. Antalet svar var fler från ungdomarna (n = 114) jämfört med föräldrarna (n = 99), och svarsfrekvensen för ungdomar med IF var i paritet med referensgruppens, med 36% för ungdomar med IF, jämfört med 38% i referensgruppen. I studie II användes chi-två tester och vid behov Fisher’s exakta test, för analys av data. Alla internet-aktiviteter, utom att spela spel, utförs av en lägre andel ungdomar med IF jämfört med referensgruppen (studie II) och tiden som spenderas på internet-aktiviteterna är lägre för ungdomar med IF (studie III). Utmaningar i den omgivande miljön, såväl som personliga förmågor leder till svårigheter med internetuppkoppling och internetanvändning (studie I), och påverkar digital delaktighet för ungdomarna och de unga vuxna med IF. Möjligheter och risker med att använda internet visade att en signifikant högre andel föräldrar till ungdomar med IF uppfattar möjligheter förknippade med internetanvändning och att spela spel, och en lägre andel upplever risker med negativa konsekvenser, eller oroar sig för risker med internet jämfört med referensgruppen (studie III). Signifikant fler föräldrar till ungdomar med IF uppfattar dock att deras ungdomar aldrig använder smartphones och sociala medier jämfört med referensgruppen. Strategier, som ungdomar och unga vuxna med både lindrig och måttlig IF använder för att hantera den digitala miljön och delta i internet aktiviteter identifierades (studie I). Strategierna innebär; att få stöd från andra; att minska andelen enheter som används för internetanvändning och att främsta använda enheter som är utformade för/av person; samt att använda stödstrategier som är baserade på ord- bild- och röststöd för att kunna utföra internet-aktiviteter. Kunskapen som avhandlingen har bidragit med: Sammantaget visar resultaten en eftersläpning i tillgång till och användning av internet, liksom i digital delaktighet för ungdomar och unga vuxna med IF. Även om deltagarna har tillgång till och använder internet, är det i lägre utsträckning än referensgruppen. Faktorer i miljön tillsammans med deltagarnas personliga förmågor gör utvecklingen av digital kompetens svår för deltagarna. Resultatet att fler föräldrar till ungdomar med IF uppfattar möjligheter och färre uppfattar risker med internet är ny kunskap som kan stödja positivt risktagande i internetanvändning och möjliggöra digital delaktighet för ungdomar med IF. Book excerpt: Generalized anxiety disorder (GAD) is a chronic condition characterized by an excessive and uncontrollable worry. GAD has a highly negative impact on all aspects of life, and the costs for society are considerable. Cognitive behaviour therapy (CBT), delivered face-to- face or online, has been shown to be effective in treating GAD. However, there are still room for improvement, with treatment content and format of delivery being two areas to further investigate. Internet-based treatments are often based on a generic CBT model of GAD and include treatment strategies such as cognitive restructuring and relaxation. Furthermore, the impact of therapist support has shown inconclusive results. Overall, this thesis aimed to evaluate the effects of a new internet-based treatment for GAD, based on acceptance, mindfulness, and valued action, delivered with different types of support. Study I was a small pilot trial with a within-group design. The study included 16 participants diagnosed with GAD who received the newly developed treatment program with weekly therapist support. Statistically significant and large effect sizes were observed on self-rated worry at post-treatment, and the effects were maintained at a three-months follow-up. Study II included 103 participants diagnosed with GAD who were randomized to nine weeks of treatment with the treatment program and weekly support, or a waiting-list control group. Statistically significant moderate to large effects were observed on primary and secondary outcomes in favour of the treatment. The exception was a measure of quality of life, with no significant difference between the two groups. The treatment effects were maintained at a six-months follow-up. Study III was a pilot trial including 33 participants diagnosed with GAD. A within-group design was used to investigate the effects of the treatment program delivered with automated messages and support on demand. Significant and large within-group effects were observed on all outcome measures at post-treatment with the exception of quality of life, for which there was a small effect. Study IV was a pilot factorial design trial that compared the acceptance-based treatment program against a self-tailored treatment, as well as two types of support: scheduled support and support on demand. The study included 85 participants with GAD randomized to four different treatment groups. Significant moderate to large effects were observed in all treatment groups, with no statistically significant differences between the groups on self-rated outcome measures. Receiving scheduled support was rated as more positive than support on demand, and self-tailored treatment was rated as more positive than the acceptance-based treatment. Treatment satisfaction was high in all studies. In conclusion, the studies indicate that an internet-based treatment based on acceptance, mindfulness, and valued action is a viable option in the treatment of GAD. Syftet med avhandlingen var att utvärdera behandlingseffekterna av ett nytt acceptansbaserat behandlingsprogram för GAD, förmedlat via internet, med endera veckovist stöd eller stöd vid behov. De fyra inkluderade studierna visar att behandlingen resulterade i signifikanta effekter på oro, GAD-symtom och komorbida depressiva symtom. Behandlingen var effektivare än kontrollgrupp (väntelista). Vid jämförelse med en aktiv behandling var effekterna likvärdiga. Att förmedla behandlingen med veckovist stöd eller stöd vid behov resulterade i jämförbara effekter på symptomskattningar. Dock slutförde fler som fick veckovist stöd hela behandlingen och de var även mer nöjda både med stödet och behandlingen. Behandlingseffekterna var jämförbara med tidigare IKBT-studier för GAD och deltagarna rapporterade hög grad av nöjdhet i alla studier. Sammanfattningsvis är acceptansbaserad internetförmedlad behandling ett effektivt alternativ vid behandling av GAD.
