Book excerpt: First published in 1998, Reproducing Narrative sets out to interrogate a number of medico-legal reproductive discourses. Recognizing that these dialogues are heavily imprecated in broader social, political and economic discourses it is contended that responses to reproductive issues are influenced and possibly determined, by non-reproductive concerns both at a parochial and more general level. Whilst a number of such influential narratives are recognized the book concentrates on the narratives of gender which appear implicit within the discourses and practices considered. Given the productive nature of discourse and the traditional premising of gender on sexual difference it becomes apparent that the explicit figuring of the female reproductive body becomes a means of realizing the implicit gender narratives within these discourses. Privileged medico-legal discourses become understood as a technology of gender - an important site at which gender is constituted.

It embraces all-important aspects in the field of in vitro fertilization and alternate assisted reproduction. Papers on controversies and diversities of methods to stimulate the ovaries, imaging techniques, basic research and state-of-the-art papers on ovarian physiology, the role of GnRH and its analog, clinical aspects of IVF treatment and cryopreservation, up-to-date techniques in assisted reproductive technologies that are quickly developing in conjunction with IVF, were included. When should IVF be preferable to surgery? What are the ex pected up-to-date world results and what are the psychological, moral, ethical and religious implications? These are all the concerns of the treating team and are addressed here. Male factor infertility remains a frustrating problem, but advances in the understanding of sperm-egg interaction, sperm evaluation and preparation are reported. Micromanipulation emerges as a possible alternative to bring some relief to this problem, but it also promises to be central in promoting the field of prenatal genetic analysis.

Book excerpt: In vitro fertilization (IVF), gamete intra-fallopian transfer (GIFT), and other technologies of assisted conception have been heralded by the medical community and the media as "the answer" for infertile couples. This timely collection of articles discusses medical and social options for couples facing infertility; the effectiveness, safety, costs, and benefits of the new reproductive technologies; and some of the legal and ethical issues surrounding the use of these services. Although now in widespread use, the new reproductive technologies have not yet been fully evaluated. No attempts have been made to determine the need for such services, to compare their effectiveness with other therapies to restore fertility, or to assess the risks associated with such treatments. Since there is no agreed-upon standard definition of infertility, over-diagnosis is often spurred by exaggerated claims of success by medical sources and the media. Over-treatment exposes women to unnecessary risks and has a staggering impact on national health budgets. Bringing together key issues in health policy analysis, this volume argues for a public health approach to infertility, maintaining that far too little attention has been given to the important social, ethical, and legal issues involved. These internationally focused essays address critical issues that have arisen from the proliferation of infertility technologies: Should there be any social criteria for IVF recipients? Does society have a responsibility to deal with the long-term consequences of the technology? What is to be done about the diversion of money, resources, and health professionals' talents away from pressing community health needs into ahigh technology benefiting only a few? What are the best ways to handle ethically questionable practices such as inducements to women to donate their eggs to IVF clinics and misrepresentation of success rates?

It seemed to me at that time that significant advances in the field called for the publication of such a volume. The following is from the preface to that volume: During the past 15 years considerable progress has been made in the field of infertility diagnosis and management. It is perhaps a paradox that much of this increased knowledge has come about because of Western medicine's preoccupation with the search for a means to control reproduction. As a result, we have achieved new insights into the physiologic mechanisms involved in reproduction, and we have found better methods for measuring physiologic changes in reproductive health and disease. To these advances can be added improvements in the utilization of endoscopic and surgical techniques, in the diagnosis and treatment of infections and endometriosis, and in the treatment of hormonal disorders. During this period, too, through workshops and conferences and in journals and texts, these latest advances have been made available to physi cians, an outstanding example being the two volumes of Progress in Infer tility, edited by Drs. Jan Behrman and Robert Kistner. As necessary as these publications are, they do not offer an overall view of infertility diag nosis and management.
