In that relatively short period of time, study of the disease has moved from the level of early clinical description to exhaustive and extensive laboratory characterization of the human immunodeficiency virus (HIV), the immune responses directed towards it and reasons for their failure. This volume provides contributions from clinical and basic scientists who are actively involved in research in a number of areas of current interest and controversy. Further progress in the clinical management of the HIV-infected patient will undoubtedly build on the basic knowledge about HIV and its modes of pathogenesis. The intimate relationship between HIV and the human immune system provides observations and questions that are relevant to viral immunopathogenesis in general. In the first chapter the clinical features of HIV immunodeficiency are re viewed, and aspects of its changing face are discussed. Dr Tersmette then presents evidence for changing viral characteristics at different stages of the disease. This view of close competition for ascendancy between HIV and the host immune response raises questions about current approaches to therapy.

Book excerpt: From the early days of its recognized occurrence, AIDS has been per ceived as posing tremendous threats, burdens and challenges to human beings. Individuals, societies and, in a global point of view, mankind are affected by the effects of the HN infection, the nature and extent of which is still unclear in many ways. In the beginning only biomedical and epidemiological analyses of the problem were the top research priori ties, the former laden with great hopes that it may soon be possible to stop the spread of the disease and to overcome its physical impact. Yet it soon became clear that AIDS would be something to be reckoned and coped with on a long-term basis, making a thorough investigation of its impact absolutely mandatory. AIDS has serious economic consequences. Taken seriously, they can not be confined to predictions of costs intended to support the AIDS issue in the struggle for resources. Besides cost calculations - a method ologically tricky and wide-ranging topic in itself -and their application to cost-effectiveness and other analyses, economic issues include identify ing and assessing patterns of care, analyzing problems of financing, exploring impacts on markets other than health care, and modelling scenarios for future developments and strategies. At present, the eco nomic aspects of AIDS still constitute a very recent topic in European health economics and health systems research. Many projects are just about to start, and there must be a better exchange of information between research groups.

By 1985 when a test for HIV, the cause of AIDS, appeared it was realised that a vast silent epidemic had occurred which is still not under control. It also became apparent that the natural history of HIV was slow by comparison to most infections with a median time from infection to AIDS of around 10 years. Consequently as a new infection to medicine, HIV and AIDS required further study. This thesis has detailed the establishment of an effective HIV medical service in Edinburgh largely composed of injection drug users, the natural history of IDU related HIV, the clinical presentations to date, the effectiveness of the service and lastly the factors predicting heterosexual transmission. This thesis describes the history of Injection Drug Use or IDU and its associated infections including IDU related HIV and the epidemic in Edinburgh. In addition it also describes the clinical services established, the natural history of the disease and heterosexual transmission

Book excerpt: In most countries, primary prevention programmes against the HIV / AIDS epidemic have been implemented. Broadly speaking, three levels of intervention can be identified: - national campaigns directed to the general population; most of them are multi phase campaigns aimed at providing information about HIV transmission and protective behaviour; they use the various mass media channels and are mainly directed to sexual behaviour modifi cation; - community-based interventions, addressed to specific target popula tions; these populations have been typically selected according to both the high risk of infection (gay men and prostitutes) and the difficulty to reach the members of these communities (intravenous drug users); - individual testing and counselling, often supported by public funds or large non-governmental organizations. Major efforts have been devoted to the development and the implemen tation of these preventive programmes, both in terms of human re sources and financial support. On the other hand, in most countries, far less energy has been put into the evaluation of these campaigns. This gap is not explained by the fact that evaluation of AIDS/HIV cam paigns is a totally new challenge in terms of methodology: there are classical methods, developed over twenty years and used in other fields of prevention.
