Book excerpt: Acute lower limb ischaemia is caused by sudden obstruction of an artery due to an embolus or thrombosis.Chronic lower limb ischaemia is a slowly progressing disease process that is usually caused by an obliterating arterial disease.Acute lower limb ischaemia must be diagnosed and the patient immediately referred for further management. Chronic lower limb ischaemia must be diagnosed and its severity (stage) assessed, as the severity of the condition will determine management approach. Mild ischaemia is asymptomatic, but is suggestive of an increased overall cardiovascular risk. Moderate ischaemia causes intermittent claudication which, as such, is a benign but troublesome complaint. Claudication may be treated conservatively, but if the condition threatens the work or functional capacity of the patient, a referral to a vascular surgeon is indicated. Critical ischaemia manifests itself as pain at rest and may lead to a non-healing chronic leg ulcer and eventually to gangrene. Patients with critical limb ischaemia require urgent referral to a vascular surgeon. If limb ischaemia is complicated by an infection, an emergency referral is indicated. If ischaemia is suspected it should be verified with Doppler ultrasonography and pressure measurements. Examination of the patient, see . Prophylaxis against coronary heart disease and ischaemic cerebrovascular disorders should be provided. This has the greatest impact as regards the patient’s prognosis. The arteries of the lower limbs are never the only site of atherosclerosis in a patient with occlusive arterial disease.

Book excerpt: Jean Natali The pathophysiology and management of chronic critical limb -ischaemia (CLI) has always been a problematic area, at least partly because it involves doc tors from a wide range of the traditional medical specialities including vascular surgery, angiology, diabetology, haematology and radiology. The treatment of these patients also varies largely with local circumstances and national traditions. CLI therefore seemed a particularly appropriate subject for a new type of European consensus approach. In 1988 a series of small workshops were held by the European Working Group on Critical Limb Ischaemia to discuss the definition, pathophysiology, in vestigation and management of this condition. The process culminated in a meet ing in Berlin in March, 1989 where 120 specialists from sixteen European countries, representing the basic sciences as well as a spectrum of clinical dis ciplines, met to evolve a Consensus Document on the subject with specific recom mendations. The Document, which is reproduced in the first section of this book, does not of course necessarily represent the unanimous view of all those who participated in its compilation; however it is agreed that it does represent a con sensus or majority view. It was also noted that the comments and recommenda tions in the document should be taken as a whole, and are not intended to dictate the only correct approach to individual treatment.

This high dependency on energy-consuming processes is mainly caused by the active transport of ions, which is necessary to compensate for the transmembrane ion currents that are part ofthe complex signaling processes in the brain. Ninety-five percent ofthe brain's ATP is derived from mitochondrial oxidative phosphorylation. Since that organ' s storage capacity for oxygen is minimal, any interruption of oxygen delivery to brain cells willlead to changes in membrane excitability and, there fore, to disruption of neuronal signaling within seconds. It seems that mamma lian brain is especially vulnerable to such an interruption, since oxygen deprivation leads to activation of ion channel mechanisms in neurons that impair their communications. Thus, the function of the brain as a coordinator of vital homeostatic reflexes, and complex body reactions to external challenges, depends critically on the rate of oxygen delivery and oxygen consumption. Oxygen delivery depends on two variables described in the Fick relation ship: volume flow rate ofblood and the arterial oxygen content. A reduction in either of these two variables will have serious effects on vital brain func tions. Reduction of arterial blood flow to the brain can be caused by cardiac arrest, shock, carotid occlusion, Of hypotension (global ischemia). Oxygen content is progressively decreased in asphyxia (including drowning).

Book excerpt: Strangulating obstruction of the small intestine in the horse is associated with a high mortality rate. Although many intestinal lesions can be treated surgically, more effective strategies are needed to decrease the occurrence of complications related to the intestinal segments that are not amenable to intestinal resection. The aim of this PhD was to determine the feasibility and effect of two novel treatment modalities in equine small intestinal ischaemia. Therefore, ischaemic postconditioning and pharmacological preconditioning with lidocaine and xylazine were investigated in two experimental in vivo studies. By evaluating different variables for intestinal ischaemia reperfusion injury by use of histomorphology, immunohistochemistry and biochemical analysis, the results of this study demonstrate the applicability of these treatments in experimental small intestinal ischaemia in horses.

The disease is a great masquerader of other conditions and can be mistaken for ulcerative colitis, Crohn's disease or almost any other inflammatory disease of the large bowel. It can have shallow ulcers, deep ulcers, filiform polyps, and pseudomembrane. It can produce fistulae or toxic megacolon. The thumb printing and other acute findings are not always seen and the site of involvement may be atypical. It is therefore so important to have a book dealing with this condition extensively and in detail. This scholarly presentation based on large clinical experience significantly contributes to the knowledge of this important disease that will assume even more importance as other conditions involving the colon are successfully treated. They have broadened our in sight into the molecular basis of myocardial lipidology and the lipido-chemical basis of car diology in health and disease, and have narrowed the gap between promising pharmacological intervention in experimental animals and clinical treatment of patients suffering from an ischaemic heart. Furthermore, they illustrate the fundamental significance of the product of the union between basic and clinical science. The rapid development of knowledge in the "Lipids-Heart-Heart Disease" triad prompted us to organize the International Symposium on Lipid Metabolism in the Normoxic and lschaemic Heart, held in Rotterdam (The Netherlands) on September 22 and 23, 1986. This meeting was a coproject of the Department of Biochemistry I (Medical Faculty, Erasmus University Rotter dam) and the Department of Physiology (Medical Faculty, University of Limburg) and was held under the auspices of the GerritJan Mulder Foundation (named after a famous Rotterdam physi cian and chemist who lived from 1802-1880) whom we thank for their co-operation. Book excerpt: During the last meeting of the European Association of Neurosurgical Societies (Barcelona 7-16 September 1987), a Symposium was devoted to the use of calcium antagonists in cerebral vasospasm. As shown by its title "Prevention and treatment of delayed ischaemic dysfunction in patients with subarachnoid haemorrhage: an update", papers presented at the Symposium covered a broad spectrum from some basic data on pathophysiology of subarachnoid haemorrhage (SAH) and delayed ischaemic dysfunction, to the clinical use of nimodipine, which has been largely documented among calcium inhibitors for its cerebrovascular properties. This supplementum of Acta Neurochirurgica is based upon the papers presented. Some articles, however, have been extended to take into account further results in order to present a broad view of the subject. The Symposium started with two presentations concerning general aspects of SAH: in the first one, B. Voldby reviewed the pathophysiological events following SAH: if modifications of intracranial pressure (ICP) and decrease of cerebral blood flow (CBF) are the primary alterations, metabolic changes (particularly reduction in CMR0 ) 2 contribute rapidly to disturbances of brain circulation. A variety of mechanisms may be responsible for the neurological dysfunction: most of them are directly related to the reduction in cerebral perfusion pressure (CPP) which may result in ischaemia. This fall in CPP (either due to elevation of ICP or reduction of mean arterial blood pressure) is observed under different pathophysiological conditions, discussed in the paper by A. D. Mendelow.
