Book excerpt: At the request of the Chief of Naval Operations, the National Research Council (NRC) conducted a study to determine the technological requirements, operational changes, and combat service support structure necessary to land and support forces ashore under the newly evolving Navy and Marine Corps doctrine. The Committee on Naval Expeditionary Logistics, operating under the auspices of the NRC's Naval Studies Board, was appointed to (1) evaluate the packaging, sealift, and distribution network and identify critical nodes and operations that affect timely insertion of fuels, ammunition, water, medical supplies, food, vehicles, and maintenance parts and tool blocks; (2) determine specific changes required to relieve these critical nodes and support forces ashore, from assault through follow-on echelonment; and (3) present implementable changes to existing support systems, and suggest the development of innovative new systems and technologies to land and sustain dispersed units from the shoreline to 200 miles inland. In the course of its study, the committee soon learned that development of OMFTS is not yet at a stage to allow, directly, detailed answers to many of these questions. As a result, the committee addressed the questions in terms of the major logistics functions of force deployment, force sustainment, and force medical support, and the fundamental logistics issues related to each of these functions.

The editors and contributors to this new handbook are a group of physicians, nurses, and corpsmen with extensive experience in caring for patients in the expeditionary maritime environment, designing and implementing current doctrine and policy, and publishing peer-reviewed articles focused on these topics. This handbook takes the approach of a "how to" manual for the management of combat or disaster victims, beginning at the point of injury and proceeding through each stage of care until they leave the maritime environment. This includes sections on prehospital care, triage, en-route care, and maritime mass casualty management, as well as additional chapters covering unique aspects of maritime platforms, capabilities, and missions. The bulk of the book focuses on the initial patient evaluation and resuscitation as well as the operative and perioperative phases of care including prolonged casualty care. The primary focus throughout the book is on simple, practical, and proven practices that can be easily understood and implemented by physicians and independent providers of any experience level who may find themselves in similar situations. For the clinical chapters, each begins with a clinical vignette relevant to the chapter based on actual patients or maritime scenarios experienced by the authors demonstrating the various challenges that can occur caring for injured and surgical patients at sea while deployed on maritime and amphibious platforms. When appropriate, each clinical chapter will conclude by describing the recommended management and outcome of the patient(s) presented in the vignette that opened the chapter. The style is plain and direct language, avoiding scientific jargon and unnecessary complexity whenever possible. Each chapter begins with 5 to 10 bullet points that summarize the key information or “BLUF” (bottom line up front) from that chapter and conclude with common tips and pitfalls, as well as recommended high-yield resources for the entire maritime surgical team.

The United States, for example, has been instrumental in creating the Bretton Woods system, with organizations such as the World Bank and the International Monetary Fund. The rationale is that a hegemon, being the dominant actor in international politics and economics, often stands to gain the most from the creation of global standards. For instance, while other countries might benefit from it, U.S. companies like Microsoft, Universal Studios, and Pfizer would be among the greatest beneficiaries of a strict global intellectual property regime. As the hegemons use their power to create regimes, their withdrawal similarly can also threaten the effectiveness of regimes. Regimes serve crucial functional needs in international relations. Powerful regimes are considered by some scholars as independent actors in international politics. Although ultimately states create and sustain regimes, once institutionalized, regimes can exert influence in world politics that is practically independent of state sovereignty. The International Atomic Energy Agency, for instance, has certain rights, given to it by states themselves, to monitor nuclear energy activity in countries. Insofar as they are organized by means of treaties among countries, regimes provide an important source of formal international law. Regimes themselves can also be subjects of international law. This book tries to unravel that ideology and to create an alternative vision of a just and democratic world over.
