La première est consacrée à l'étude ontogénique, anatomique et fonctionnelle de l'ATM et en particulier du ménisque. La deuxième partie concerne l'aspect dysfonctionnel de l'ATM et sa mise en évidence par les examens conventionnels d'imagerie. La troisième partie se rapporte à l'imagerie en RMN, une étude théorique a été entreprise dans un premier temps pour l'étude des contrastes entre les tissus; dans un second temps, une étude expérimentale et clinique a défini la visualisation du ménisque. Les résultats obtenus permettent d'en expliciter les caractéristiques en anatomie normale et lors de troubles crânio-mandibulaires.

Des contraintes importantes au niveau de l’ATM peuvent être à l’origine de douleurs et/ou dysfonctionnements qui amènent le patient à consulter. L’examen clinique permet le plus fréquemment de poser un diagnostic. Cependant, des examens complémentaires peuvent parfois être nécessaires comme l‘orthopantomogramme et l’imagerie par résonnance magnétique (IRM). L’objectif de ce travail a été de déterminer l’apport de l’IRM lors des différentes pathologies de l’articulation temporo-mandibulaire à travers une revue systématique de la littérature existante sur le sujet.

Cette étude porte sur 29 ATM de 16 sujets dont 8 témoins, et 8 avec luxations discales. Des coupes sagittales IRM des ATM ont été réalisées lors d’un mouvement d’ouvertures contrôlées. Les coupes sélectionnées ont été analysées par un système graphique assisté par ordinateur. La surface totale de la section du disque (ST) est divisée en une surface antérieure (SA) et une surface postérieure. La valeur des surfaces est déterminée par l’ordinateur pour chaque ouverture buccale. L’hypothèse: le rapport SA/ST est un indicateur des relations disco-condyliennes. Il est donc important de corréler toutes ces composantes afin de mieux comprendre comment elles interagissent quotidiennement dans l'appareil stomatologique de nos patients. Jusqu'à présent, l'axiographie mécanique ou électronique et l'examen IRM étaient les seuls outils d'analyse de l'articulation temporo-mandibulaire. L'axiographie ultrasonique, avec l'Arcus Digma, est un nouvel instrument d'investigation de cette articulation. Une présentation objective et comparative sera faite dans cette thèse, en développant notamment les objectifs de ce nouveau dispositif, ses indications, l'aide précieuse qu'il peut apporter dans l'élaboration de nos diagnostics ainsi que ses avantages et inconvénients par rapport aux systèmes préexistants. Notre propos s'appuiera sur une comparaison in vivo de plusieurs dispositifs d'investigation de l'articulation temporo-mandibulaire afin de mieux rendre compte des possibilités et des difficultés de chaque technique.

Book excerpt: OBJECTIF : Donner des indications actualisées des techniques d'imagerie utilisées en fonction des désordres de l'articulation temporo-mandibulaire. METHODE : Une revue systématique des articles publiés entre 1990 et aujourd'hui a été réalisée sur PubMed afin de collecter ceux relatifs à des désordres temporo-mandibulaires et qui cherchent à établir le diagnostic au moyen d'une ou plusieurs techniques d'imagerie. Seules les études de plus haut niveau de preuves ont été retenues. RESULTATS : Les études sélectionnées recherchent l'intérêt des examens d'imagerie (radiographie panoramique, scanner RX, IRM, ultrasonographie...) pour poser un diagnostic de désordres temporo-mandibulaires (groupe I, Il ou III de la classification de RDC/TMD de Dworkin et LeResche). CONCLUSION : Pour le groupe I, le diagnostic est uniquement clinique ; pour le groupe II, le plus souvent la clinique suffit à poser le diagnostic et seul l'IRM permet de visualiser le disque ; pour le groupe III, les rayons X permettront de visualiser les modifications osseuses, mais on n'utilisera cette technique que dans des cas bien spécifiques et en cherchant la plus faible exposition du patient aux rayons X ; l'IRM permet aussi d'imager les lésions osseuses.

But in the light of the new experiences one may ask if the characters selected were the most useful for their purpose? Are they easy to identify, and are their definitions rigorous and unambigous ? What is known about their embryology and development, their genetic control? Was their categorisation adequate? Are they of any general medical, forensic or surgical relevance? What is the extent of intrapopulation variation in incidence ? How do they vary between populations ? This atlas endeavores to answer such questions and to serve as a reference text. The entry of each of the 84 characters summarises its nomenclature, gross anatomy, function, embryology and growth, genetics, medical relevance, sex differences, symmetry (if the character is bilateral), laterality or side preference, age variation, association with other traits, and methods of scoring (the traditional as well as that now proposed). For some traits new results are given applying the proposed methods to male adult skulls of recorded age and origin as well as the frequencies of the variants investigated by traditional methods in a selection of the populations of the world from the literature.
