Book excerpt: These guidelines are an update of the 1999 World Health Organization (WHO) guidance on the use of iodine thyroid blocking (ITB) with a special focus on public health considerations of ITB implementation.These guidelines provide a recommendation on iodine thyroid blocking (ITB), via oral administration of stable iodine, as an urgent protective action in responding to a nuclear accident. This recommendation aims to support emergency planners, policy makers, public health specialists, clinicians and other relevant stakeholders, in order to strengthen public health preparedness for radiation emergencies in WHO Member States as required by the International Health Regulations (IHR) and in line with the international safety standards (GSR Part 7). The scope of the guidelines is confined to public health aspects of planning and implementation of ITB before and during a radiation emergency, such as dosage and timing of ITB administration, adverse effects of stable iodine, its packaging, storage, and distribution.

In the event of a radiation incident, radioiodine is one of the contaminants that could be released into the environment. Exposure to radioiodine can lead to radiation injury to the thyroid, including thyroid cancer. Radiation to the thyroid from radioiodine can be limited by taking a nonradioactive iodine (stable iodine) such as potassium iodide. This book assesses strategies for the distribution and administration of potassium iodide (KI) in the event of a nuclear incident. The report says that potassium iodide pills should be available to everyone age 40 or youngerâ€"especially children and pregnant and lactating womenâ€"living near a nuclear power plant. States and municipalities should decide how to stockpile, distribute, and administer potassium iodide tablets, and federal agencies should keep a backup supply of tablets and be prepared to distribute them to affected areas.

Book excerpt: In order to determine whether acute administration of iodide leads to any undesirable effects in the general population, we studied a group of patients who received iodinated radiographic contrast medium in the course of routine x-ray diagnostic procedures. We were particularly interested in investigating the possibility that administration of iodine could damage thyroid follicular cells leading to release of intrathyroidal antigens such as thyroglobulin into the blood. An increase in serum thyroglobulin might, in turn, either initiate or exacerbate thyroid autoimmunity in susceptible individuals, leading to autoimmune thyroiditis. We looked for undesirable effects due to the administration of iodide in 3 ways: a possible acute toxic effect on thyroid follicular cells was investigated by determining serum thyroglobulin immediately prior to and 24 hours after injection of iodinated contrast medium; an effect on thyroid autoimmunity was investigated by determining thyroid autoantibodies immediately prior to and 3 to 6 months after injection of iodinated contrast medium; and acute and chronic effects on thyroid function were investigated by performing thyroid function tests immediately prior to, 24 hours after and 3 to 6 months after injection of iodinateed contrast medium.

The iodine deficiency disorders (IDD) encompass a spectrum of adverse health effects including goiter, cretinism, hypothyroidism, growth retardation, and increased pregnancy loss and infant mortality. This volume summarizes the current understanding of the effects of iodine deficiency as well as iodine excess. It also discusses best practices for salt iodization, the mainstay of global IDD prevention efforts, and other forms of food fortification. The effectiveness of iodine supplementation for vulnerable populations, an evolving strategy in many regions, is also described. Low level environmental exposure to chemicals such as perchlorate and thiocyanate, which competitively block thyroidal iodine uptake, appears to be ubiquitous worldwide. There has been recent concern that such environmental exposures might pose a health hazard by inducing or aggravating underlying thyroid dysfunction. This up-to-date volume explores both the effects of iodine deficiency as well as the best strategies for IDD prevention.

Book excerpt: In an effort to determine whether iodide administration in man can induce injury to thyroid follicular cells similar to that demonstrated in lower animals, this study measures thyroglobulin, thyroxine and triiodothyronine and thyroid autoantibodies in the serum before and after administration of iodine in a variety of patients. In the last eight months primary efforts have been devoted to the development of a high specificity and sensitivity radioimmunoassay of thyroglobulin and its preliminary clinical application. Human thyroglobulin obtained from operative specimens was purified and its identity and purity established. Rabbits were injected with thyroglobulin for 30 weeks with the appearance of significant titres of antibodies within 6 weeks. The antibody developed was examined for immunological purity and used in a radioimmunoassay system with thyroglobulin labeled in this Laboratory with /sup 125/I.
