Book excerpt: In 2006, the Thai government established antiretroviral therapy as part of health benefits under the Universal Coverage Scheme. This descriptive study purposed to examine HIV/AIDS treatment and the changes during the transition from NAPHA to the UCS. Data were collected from Nan province. Analysis involved the assessment of factors influencing ART under the UCS policy. The findings: obstacles during the transitions were lack of infrastructure development. It also affected drug management and laboratory supply chain and logistics, but not to a severe degree. ARV drug regimen types are not vastly different under either NAPHA or the UCS, therefore; under both programs treatment procedures can be integrated efficiently. The Thai Government should consider excess demand which will likely lead to increase costs, and the social implications. The Ministry of Public Health should take greater responsibility for policy making, instead of the National Health Security Office, and the provincial public health offices also should be given greater responsibilities too.

A project rolling out in rural Thailand, the Positive Partnerships Program (PPP) has shown that targeted economic assistance can boost self-esteem, ambition and hope-all of which help reinvigorate community ties and have a beneficial impact in promoting an enabling environment for HIV prevention and treatment efforts. This Best Practice document provides a short introduction to the program and examines how and why it may be used as a flexible and adaptive model in other countries.

Book excerpt: In Thailand, the young population is living under the threat of getting HIV infected one way or the other. The main goal of this study is to understand the geographic patterns of HIV/AIDS in Chiang Rai, a northern province with the second highest rate of HIV/AIDS in Thailand. A related goal is to examine the impact of the disease on the lives of Thai youth and young adults living with HIV/AIDS. The lack of geographic study in Thailand has not only hindered theory development about the spread of the epidemic, but has also led to uncertainties in the design of intervention policies and the implementation of healthcare in Thailand. In this dissertation, spatial analysis is used to understand the patterns of HIV/AIDS mortality and HIV/AIDS cumulative incidence rates among youth. In addition, the experience of youth living with HIV/AIDS is analyzed to understand their feelings/concerns and health care perceptions, and to identify areas that are in need of further intervention by healthcare groups. This dissertation provides an understanding of various aspects of the HIV/AIDS epidemic and conditions of people living with HIV/AIDS in Chiang Rai. Given the cluster concentration and cumulative incidence rate in many areas of the province, the municipal region remains a high risk area that needs further investigation. The experience of youth with HIV/AIDS reveals uninformed reckless behavior and social discrimination. The understanding from this dissertation is critical for program planning and resource mobilization given the limited budget of Thailand to identify areas of high risk and to provide improved health care services to PLWHA.

Book excerpt: A cross sectional study looked at the association between sociodemographic factors, HIV/AIDS clinical history and health-related quality of life (QOL) in HIV/AIDS persons. It used the WHOQOL-BREF-THAI questionnaire completed by 3,596 patients during 2008 in three Thai hospitals. The questionnaire explored five QOL domains: Physical, Psychological, Social, Environment and Overall and it was for the first time administered to many HIV/AIDS patients in Thailand. Descriptive statistics and inferential statics (bivariate and multivariate : one-way ANOVA, Kruskal-Wallis Test, Pearson Correlation and independent ttest)) were used to test association between independent factors and QOL. Results: The following associations were all statistically significant. Males had higher QOL than females in Physical, Psychological and Overall domains. Younger age was associated with higher QOL in Physical and Social domains, older age in Psychological and Environment domains. Married had higher psychological QOL than single and separated. Good adherence to, more than 3 year duration of ARV treatment and not suffering AIDS related events were associated with higher QOL in all except social domains. VL
