Book excerpt: During 1983 and 1984, Systems Control Technology, Inc. conducted a survey of civil helicopter pilot organizations from throughout the United States who were involved in a wide range of helicopter operations for the purpose of determining the hazards of helicopter operations and the root causes of the high rate of helicopter accidents. The survey was administered through personal interviews, meetings, and questionnaires. The derived questionnaire data included census data, profiles of the pilots work environment and procedures and their own perspectives on the hazards of helicopter operations and root causes of helicopter accidents. These data were compared with historical National Transportation Safety Board accident reports and accident briefs to determine more specifically the root causes of helicopter accidents. The results of the analysis include a list of hazards and probable root causes of accidents, as well as technological, training and standardization remedies to the causes.

The helicopter was en route to the drilling ship Discoverer Spirit. The captain, copilot, and eight passengers aboard the helicopter were killed, and the helicopter was destroyed by impact forces. The flight was operating under the provisions of 14 Code of Federal Regulations Part 135 on a visual flight rules flight plan. Night visual meteorological conditions prevailed at the time of the accident. The National Transportation Safety Board determines that the probable cause of this accident was the flight crew's failure to identify and arrest the helicopter's descent for undetermined reasons, which resulted in controlled flight into terrain.

Book excerpt: A full, independent, public inquiry must be convened to address whether commercial pressure from oil and gas companies affects the safety of offshore helicopter operations. This inquiry must also examine the role of the Civil Aviation Authority. MPs highlight how the Air Accidents Investigation Branch (AAIB) investigation into the Sumburgh crash uncovered a number of serious issues. In particular, AAIB found pre-flight safety briefing did not accurately explain how to use the type of Emergency Breathing System (EBS) supplied on the helicopter. The Committee also concludes: (i) Regulatory inertia at the European Aviation Safety Agency (EASA) is exposing offshore workers to unnecessary risk by slowing down the implementation of safety improvements; (ii) There is no conclusive evidence that Super Puma variant helicopters (which make up some 60% of the UK offshore helicopter fleet) are less safe than other helicopters; (iii) The CAA review of the Sumburgh crash found that since 2008 more incidents were reported in Norway than in the UK despite the Norwegian fleet being smaller; (iv) The Government must push the European Aviation Safety Agency to implement changes recommended by the AAIB more rapidly. In addition, the Committee urges (i) AAIB to keep crash survivors better informed on the progress of investigations in future; (ii) CAA to meet survivors to hear their experiences and to consider their suggestions for safety improvements.

Book excerpt: On September 27, 2008, about 2358 eastern daylight time, an Aerospatiale Helicopter (Eurocopter) operated by the Maryland State Police (MSP) encountered instrument meteorological conditions was diverted to Andrews Air Force Base (ADW), Camp Springs, Maryland. About 3.2 miles north of the runway 19R threshold at ADW, during an instrument landing system approach, the helicopter impacted terrain and crashed. The pilot, one flight paramedic, one field provider, and one of two automobile accident patients being transported were killed. The helicopter was substantially damaged when it collided with trees and terrain in Walker Mill Regional Park, District Heights, Maryland. The National Transportation Safety Board determined that the probable cause of this accident was the pilot's lack of experience and lacking support of supporting institutions.
