The potential use of data analytics to address these health system issues has raised significant interest in both research and practice. Health Analytics is central to informing and realizing the systematic quality improvements and cost reductions required by healthcare reform. Fundamentally, the contribution of IS and analytics research in healthcare is to identify and study the impact of interventions that can make a significant difference to the quality and cost of care. This dissertation is concentrated on patients with heart failure (HF). HF is the number one killer in the world, and is the largest contributor to healthcare costs in the United States. Moreover, HF is one of the six conditions used by the Centers for Medicare and Medicaid Services (CMS) to exercise fiduciary control over health systems by monitoring both the quality and cost of care. Specifically, my larger research question is "How can we identify and inform impactful transition of care interventions that manage costs and improve resource allocation efficiencies while providing improved quality of care for heart failure patients?" We adopted a mixed-method approach to study the impact of transitional care in a healthcare system for patients with heart failure. This dissertation includes three essays. In the first essay, I use qualitative methods to study the nature, sources and impacts of information coordination problems as HF patients' transition through the patient flow in a health system. I propose a set of interventions based on my analysis of information and control errors along the continuum of care to inform the design of appropriate interventions that improve the cost and quality of care. In the second essay, I empirically evaluate the impact of these interventions on cost and quality of care measures such as all cause readmissions, heart failure readmissions, ER visits, length of stay, and cost of care. Analysis suggests that multicomponent complex transitional interventions have significant impact on reducing 30-day readmission and ER visits. The third essay is dedicated to understanding the impact of heart failure patient's self-care behaviors. I developed and validated an assessment tool for patients with heart failure to monitor and score their condition accurately. Together, these essays investigate impactful transition of care interventions that can help healthcare organizations improve quality of care and manage costs from the clinical, administrative and patient perspectives."--Abstract from author supplied metadata.

Book excerpt: In the last few years, legislations such as the Patient Protection and A ordable Care Act, also known as Obamacare, have emphasised the need for improving the quality of health care. Part of the programs introduced by this Act is the Hospital Readmissions Reduction Program (HRRP) which reduces payments to hospitals with excess readmissions. Hospitals are, therefore, constantly looking for ways to help reduce their readmission rate, and an idea of patients that are at a higher risk of getting readmitted is extremely bene cial. In this thesis, we rst look at ways to predict the chance of a patient getting readmitted, and investigate predicting future healthcare costs, to understand how much money the patient would have to spend on hospital expenses during the readmission visit. We, then design a multi objective medication recommendation framework. We would like to echo the sentiments of the medical community here, that, while reducing readmissions is important, there are also other factors that go into ensuring a successful patient discharge experience, such as reducing mortality rate and the patient's length of stay at the hospital during the subsequent visit. We propose a novel framework that recommends personalized medications to patients by analyzing the complex interplay among a multitude of factors, such as, demographic factors, medical diagnoses, clinical factors, and how they contribute to the three objectives - thirty day readmission, subsequent length of stay and mortality rate. We then nd the best medication combination to simultaneously reduce all three objectives by performing Multiobjective optimization. Our proposed framework is exible enough to include or exclude additional factors, as well as layers, and can even obey constraints provided by the domain experts (i.e., doctors) in the design of this hierarchical network. We present a case study validated by a domain expert as well as comprehensive experimental results based on the proposed approach to demonstrate the e ectiveness of our proposed framework. Our work is validated on the largest (1000+ bed) South Puget Sound healthcare system: MultiCare Health on their EMR data including medication transactions. While our primary e ort is to design medication recommendations for heart failure patients, the proposed framework could be adapted for other diseases as well. We also develop a web based service called Pathway Finder with the objective of visually exploring and discovering clinical pathways that reduce the patients chance of readmission. We, therefore, try and play our small part in improving the quality of health care in the United States.

This research evaluated the economic impact of integrating a Remote Patient Monitoring (RPM) system into the conventional heart failure disease care process. The first part of the work used a Markov chain model to represent patients' discrete health states in the disease management process and the transition probabilities between the states over time. The model presented 8 health states of a heart failure patient in the disease management process. These states are home/Post-Acute Care (PAC), doctor visit, Urgent Care (UC) visit, transfer from other health facility, Emergency Room (ER) visit, hospitalization, 30-day readmission, and death. Using the Markov model, Benefit-Cost Analysis (BCA) was conducted to estimate the change in monetary benefit and the cost of the RPM practice compared to those in a conventional healthcare system. The cost of PAC states was estimated using a parametric analysis. In order to compare BCA for control and RPM patients, it was necessary for analytical tractability to assume that the proportion of patients in each of the Home/PAC states was the same in both the cases. The BCA demonstrated the societal economic value of RPM during the first month of intervention. The results of the four months of intervention and post-intervention period indicated that RPM was not economically attractive during those periods. The stakeholder analysis highlighted the economic gains for both the healthcare service payer and heart failure patient during the first month of intervention. An RPM patient gained an increased economic value due to delayed death. During the four months of intervention, post-intervention, and one-year follow-up period, RPM generated increased cost for the payer but created economic gain for the heart failure practice, hospital, and the patient. This research indicates that RPM is a promising enabler of a new model of care with better outcomes for the heart failure disease management; however, further research is required to find an optimal RPM design with economic feasibility.

They can be costly, labor intensive, and slow, and can return results that are seldom generalizable to every patient population. Furthermore, many pertinent but unresolved clinical and medical systems issues do not seem to have attracted the interest of the research enterprise, which has come to focus instead on cellular and molecular investigations and single-agent (e.g., a drug or device) effects. For clinicians, the end result is a bit of a “data desert” when it comes to making decisions. The new research infrastructure proposed in this book will help the medical profession to make ethically sound and well informed decisions for their patients.
