Example Chapter Threading the catheter At this point both the tip of the needle and the tip of the catheter are inside the vein. You will now need to thread the rest of the catheter into the vein. The catheter is fully inside the vein when only the hub sticks out. There are several different methods to advancing the catheter The Two Handed Technique 1.You have already visualized flash, lowered your angle, advanced 1-2mm and released the tourniquet. 2.Continue holding the needle in place 3.With your non-dominant hand, grasp the hub of the catheter and slide it forward into the vein 4.Slowly withdraw the needle 5.Place your thumb on the hub of the catheter to hold it in place 6.Use your index finger to press down above the site to prevent blood from spilling out The Single Handed Technique 1.You have already visualized flash, lowered your angle, advanced 1-2mm and released the tourniquet. 2.Continue holding the needle in place. 3.Use the index finger of the hand that is holding the needle to slide the catheter past the needle 4.Continue sliding the catheter until it is fully inside the vein. With the above two techniques, be careful that you are only advancing the catheter into the vein and not the needle. If the catheter is inside the vein it should be relatively easy and painless to advance it. Unlike the needle, the catheter is soft and flexible so it will be difficult to slide it in unless you are inside the vein. If the flashback was very small, you may feel resistance... Book excerpt: Nurses establish one or two peripheral intravenous (IV) sites, depending on institution policy, in patients undergoing cardiac catheterization with possible percutaneous coronary intervention (PCI). The literature refers to both practices. The use of two IV sites during PCI can prevent concomitant administration of incompatible drugs. Having multiple sites also increases the risk of IV site-related complications, and possibly patient pain and anxiety. This descriptive study used a retrospective review of 200 charts, and described how two peripheral IV sites were used during PCI, in order to determine if establishing one IV site would be reasonable. The theoretical framework for this study was based on the Neuman Systems Model. Findings revealed that both IV lines were used 71% of the time in the 97 patients who underwent PCI. These findings demonstrate a need for additional research into the formulation of policy for nurses establishing IV lines in patients undergoing possible PCI.

Generally, even in severe shock this route of access to the cardiovascular system is available in order to apply life saving volume substitution. It also permits longterm infusions in modern intensive care, particularly continuous administration of high-osmolarity solutions in par enteral nutrition. In both fields it represents one of the most important diagnostic parameters of circulatory disorders, enabl ing the registration of central venous pressure. Its undeniable advantages are counterbalanced by the dangers inherent in all invasive methods. Since sufficient experience and precise sta tistics are now at our disposal, the time has come for a provisional survey. While a few years ago it was considered mandatory to propagate the central venous access in order to advance new therapeutic and diagnostic methods, it is now necessary to recon sider and reformulate indications for its use. In this task it is essential to weigh the expected advantages against the possible complications in each and every case of catheter application. Cri tical scrutiny must include evaluation of techniques, approaches, and finally catheter materials; this paper presents the results of such a survey.
