Available in PDF, EPUB and Kindle. Book excerpt: Background: Intra-cranial complications of acute otitis media or mastoiditis are rare but can result in significant morbidity and death. Objectives: To determine the range of presenting features and clinical course of adults with intra-cranial complications of suppurative ear disease. Methods: Retrospective case note review was performed to collect demographic, clinical, and radiological data. Results: 41 patients with mean age 48.4 (range 21-77) were admitted to our institution from 2007 to 2017. The intracranial complications included: venous sinus thrombosis (n=3), meningitis/encephalitis (n=20), extra-dural abscess (n=2), sub-dural abscess/empyema (n=8), temporal lobe abscess (n=4), and posterior fossa abscess (n=4). 10 patients did not have otological surgery, 10 had grommet insertion, 21 had mastoidectomy - 6 patients had cholesteatoma. Mean length of stay was 30 days. Conclusions: A majority of patients recovered fully with no long-term morbidity although 4 patients died. Cholesteatoma was strongly associated with posterior fossa abscess formation in our series.

Its management remains a challenge due to potential extracranial and intracranial complications. This study was designed to evaluate the recent experience with acute mastoiditis with intracranial complications at a tertiary paediatric centre. Methods: A retrospective case note review was carried out for patients admitted to Alder Hey Childrens Hospital between January 2006 and December 2016. Results: 30 patients were idenitified with mastoiditis and associated intracranial complications, with 17 males and 12 females. The average age was 4 years and 2 months (range 2 months u2013 15 years. The most common presenting compliant was otalgia and vomiting (63%), with only 27% patients presenting with mastoid swelling. 53% of patients were investigated with a combination of CT and MRI scans. 73% were diagnosed with sinus thrombosis, 40% cerebral abscess and 33% subperiosteal abscess. 78% of the patients required surgical intervention. Of the 22 patients with sinus thrombosis, 9 received anticoagulation. 27 patients recovered fully with no significant long term sequalale following an average of 50 months follow-up. Conclusion: A wide variation in management did not result in altered clinical outcomes. It was felt that there was no real consensus over anticoagulation for patients with sinus thrombosis. A prospective national study is required to be able to reach a consensus on managing these often complex patients.

Book excerpt: Although half of all deafness and hearing impairment is avoidable, an estimated 278 million people worldwide are living with disabling hearing impairment (moderate or worse level of hearing loss in the better hearing ear). Many more have mild hearing loss and/or ear diseases. One quarter of hearing impairment begins during childhood, and 80% of all deaf and hearing impaired people live in low and middle income countries. These problems can be life-long and sometimes life-threatening; they may have profound effects on: inter-personal communication, education, employment prospects, social relationships and through stigmatization. They produce substantial economic burdens on countries. Some of the most effective and cost-effective interventions against ear and hearing problems can be implemented at the primary level by trained primary ear and hearing care (PEHC) workers or primary health care (PHC) workers or their equivalents. Used on a large scale, these interventions will have a major impact on the burden of ear disease and hearing loss. However most developing countries do not have PEHC workers and the topic is hardly addressed in the training of PHC workers. The Primary Ear and Hearing Care Training Resource manuals provide practical information and guidance and can be used as part of a training course, stand-alone training module or in a self-taught manner. They are designed to be useful to a wide range of primary health care personnel. The manuals can also be used to help communities understand common causes of deafness and hearing impairment and ways to prevent and/or treat the conditions.
